2. L.6 FAR 52.236-27 Site Visit (Construction) ALTERNATE I FEB 1995 is amended as follows:
(a) The clauses at 52.236-2, Differing Site Conditions, and 52.236-3, Site Investigations and Conditions Affecting the Work, will be included in any contract awarded as a result of this solicitation. Accordingly, offerors or quoters are urged and expected to inspect the site where the work will be performed.
(b) An organized site visit has been scheduled for 17 November 1 December 2016 at 0900 or 9 A.M. Local Time.
(c) Participants will meet at -- Wellpinit Service Unit, Wellpinit, WA 99040

3. L.7 PROPOSAL INSTRUCTIONS is amended as follows
The offeror proposal package shall be submitted electronically in PDF format only. No facsimile, or other alternate method of submission will be accepted. Each electronic file submitted as an attachment to an email transmission shall not exceed 8 megabytes in size.
If the aforementioned electronic file exceeds 8 megabytes, divide data into separate files so as not to exceed 8 megabytes per file. Due to file size limitations, each electronic file should be attached to a separate email. Files shall be named as with 16-102-SOL-00031 Wellpinit Service Unit Proposal the addition of “email X of X” (e.g. email 1 of 2) in the subject line.
The offeror’s proposal must be submitted in three volumes: (I) a technical proposal; (II) past performance and; (III) a price proposal. Each of the volumes shall be separate and complete so that evaluation of each may be accomplished independently.

Late submissions shall be handled in accordance with FAR 52.215-1, INSTRUCTIONS TO OFFERS – COMPETITIVE ACQUISITION (JAN 2004) reference subparagraphs (c)(3)(ii)(A) and (1), and FAR 15.208.

Special attention is called to FAR 15.208 which states, “Any proposal, modification, or revision, that is received at the designated Government office (i.e., email address) in the solicitation after the exact time specified for receipt of proposals is “late” and will not be considered unless it is received before award is made; and (1) If it was transmitted through an electronic commerce method authorized by the solicitation (i.e., email), it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of proposals; or (2) It is the only proposal received.
It is the Offeror’s responsibility to ensure sufficient time is provided for electronic submission to be processed through the Government’s electronic point of entry.
Due date and Proposal Format. You are requested to submit your proposal to John.fannon@ihs.gov, Paul.reed@ihs.gov at your earliest convenience but no later than 1300 or 1P.M. Pacific Standard Time, 12 19 December 2016. Proposals submitted in response to this solicitation shall be formatted as follows:
A cover letter shall accompany the proposal and shall include:
(a) The Contract number;
(b) The name, addresses, telephone and facsimile numbers, and e-mail addresses of the offeror.
(c) Furnish any or all items upon which prices are offered at the price set opposite each item;
(d) Names, titles, phone numbers, facsimile numbers, and e-mail addresses of persons authorized to negotiate on the Offeror’s behalf with the Government in connection with this solicitation, and;
(e) Name, title, and signature of person authorized to sign the proposal.
The Contractor's proposal must be supported by necessary documentation to indicate that adequate planning to accomplish the requirement has been done. The proposal shall contain the following three sections:
(a) I: Technical Approach Submission Requirements: Offerors are required to submit a comprehensive and complete technical proposal that demonstrates their ability to perform the work described in this solicitation. Submittal Requirements include the following:
Provide a narrative describing the technical approach that meets the requirements of the RFP. Narrative shall be a minimum of three (3) and not exceed five (5) single-sided pages, and shall be no smaller than 12 pitch font in 8 ½” x 11” format. The submission shall be in narrative form only. Gantt Charts, Critical Path Method (CPM) schedules, etc.

The narrative shall include the following:

A description of the Offeror’s plan to account for (Required Bullets):
• the construction period (clearly identify the number of calendar days, explain if longer of shorter period of performance is proposed)
• construction phasing
• work restrictions
• administrative submittal requirements •critical path construction items including those submittals that must be accepted/approved before construction work can begin •long lead items (specify items)
A description of the Offeror’s critical path of their proposed schedule shall include a discussion of at least three (3) elements that have the potential to negatively impact the critical path and what measures will be taken to mitigate impacts to time and resources.
Note: Failure to address any of the submission requirements bullets and three required elements may result in an Unacceptable rating.
(b) II: Past Performance Requirements: The offeror shall provide project descriptions for a minimum of three and a maximum of five other similar or comparable efforts performed during the past three years which demonstrate relevant experience and successful performance by the offeror. The description should also indicate if the project was performed by any of the personnel proposed for this project, and the role and time commitment of the personnel on the referenced project. The description shall also state why the project is considered relevant. See Section: J.38 for the form to utilize for submission of Past Performance information.
(c) III: Price Submission Requirements: The proposal shall be a Firm Fixed price proposal for the item listed above in Section B.1 and the following shall be submitted as part of the Price Submittal:
(a) Requirement(s) as stated in Section I.8 Bid Guarantee
(b) Fully complete and submit Section K (in its entirety)
(c) Fully complete, submit and sign the SF1442 as stated in Section B.1
4. Contract Requirements: J-01 Section 2.1 Project Description is amended as follows:
8. Phase 5: Lobby and Signage. Work includes installation of a new directory kiosk at the entry, installation of three (3) pre-cast concrete site signs, and installation of primary departmental signage in the Lobby. Is this an option?
5. The subject proposal due date is hereby extended to 19 December 2016 at 1300 or 1 P.M.
PST.
All other terms and conditions remain unchanged. The proposal due date IS EXTENDED as a result of this Amendment.
