STATEMENT OF WORK
1 Sept 2017
Multi Building Renovations (Bldgs. 1005, 1008 & 1009)
SW-1 SCOPE:
The project site is located at the Air National Guard Base, Jacksonville International Airport in Duval County, Florida.
Construction will take place at Building 1005, 1008, and 1009. All work shall meet the most current applicable codes (see section SW-7).
Building 1005: This project includes building walls, adding electrical outlets, lights, motion sensors, communication boxes, television boxes, interior doors, quiet rock, drop-ceiling modifications, and applying all finishes where applicable. In addition the project also includes adding supply air ducts, diffusers, return grills and relocating electrical.
This project will include all equipment, labor, materials, and disposal of all debris as well as site cleanup to complete the necessary construction. Construction must comply with all local, state and federal building codes in addition to the manufacturer’s installation requirements.
Building 1008: This project includes the demolition and construction of two existing restrooms (Room-104 & Room- 109), remove and replace six (6) windows, and the removing of two (2) windows and patching the hole with CMU concrete blocking; both the exterior and interior surfaces is to be resurfaced and painted to match existing material. All construction must comply with state and federal regulations, to include manufacturer’s installation requirements.
Building 1009: The project scope involves demolishing and replacing existing 10’x 25’ awning, and demolishing and replacing two (2) exterior staircases. The existing staircases consist of 28 welded steel stair treads with risers on two separate exterior staircases to include 2-4’x 5’ landings at the top of each staircase. The contractor is to remove and replace the existing 5’x5’ concrete pad at the base of south staircase being replaced. The contractor is to grind off, patch, and paint existing structural support steel and railing connection points. The contractor is to provide new structural support steel and railing connection points and all mounting material to provide a complete and usable system. Staircases and mounting hardware is to be galvanized steel. The awning is to be aluminum with galvanized steel frame, mounting brackets, and hardware. This project will include all equipment, labor, materials, and disposal of all debris as well as site cleanup to complete the necessary construction.
SW-2 SCOPE OF WORK:
All requirements must be met as they appear in this statement of work. This statement of work does not supersede any Federal Acquisition Regulation stipulations that may apply to this contract.
2.1 The Contractor shall provide supervision, transportation, labor, materials, equipment, fuel, and any other needed materials for the purpose of this project.
2.2 The Contractor is subject to all base fire, safety, security, and traffic regulations.
2.3 The Government contracts on the understanding that the Contractor is complying with applicable laws concerning workplace safety.
2.4 The Contractor is subject to all citations and penalties for violation of applicable workplace safety laws.
The burden of compliance rests with the Contractor. Enforcement of workplace safety laws is the responsibility of the agency specified in the applicable law.
2.5 The Contracting Officer may notify the Contractor of hazards in Contractor operations that endanger Government personnel or property and require appropriate abatement action. This provision does not constitute assumption of liability for Occupational Safety and Health compliance on the part of the Government or negate other agency responsibility for enforcement of applicable laws.
2.6 Accident prevention principles must be thoroughly integrated into all operations. Unsafe conditions and practices, as identified by the Contracting Officer or designated representative, must be corrected immediately.
2.7 Any and all debris generated by the Contractor's operations shall be removed from the base. Disposal of all debris shall be in accordance with existing local, state, and federal regulations. The Contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations.
2.8 The Contractor shall be liable for any loss, destruction, or damage, and deterioration to Government property, including vegetation and landscaping, resulting from Contractor operations. Any damage to government or personal property shall be repaired or replaced to the Contracting Officer's satisfaction at the Contractor's expense.
2.9 All work under this project shall meet applicable County, State, Federal, Air National Guard construction codes, regulations and policies.
SW-3 SPECIFIC ITEMS OF WORK:
Building 1005:
3.1 This project includes building walls, adding electrical outlets, lights, motion sensors, communication boxes, television boxes, interior doors, quite rock and applying all finishes to include flooring. In addition the project also includes adding supply air ducts, diffusers, return grills and relocating electrical. This project will include all equipment, labor, materials, and disposal of all debris as well as site cleanup to complete the necessary construction.
Construction must comply with all local, state and federal building codes in addition to the manufacturer’s installation requirements. All finishes and or products shall match existing. If you cannot match existing contact COR for further information and approval. See drawings for new walls, electric, communication and television locations etc.
3.2 Walls:
• Construct 3 metal stud walls to form rooms 1 and 2 from finish floor 10’-0” through the acoustical ceiling, and tie to existing roof members. Secure vertical framing to existing roof deck truss system as needed for proper stabilization. Verify sizes and location off walls with COR prior to wall construction.
• Install and finish quite rock with level 5 finish for paint.
3.3 Electric, Communications and Television:
• Install 6 Quadraplex outlets @ 18” AFF and 3 duplex outlets @ 72” AFF to include all conduit, boxes, outlets and wiring. New conduit shall stub up through and above existing ceiling.
• Install 6 Communication drops using 1.5 inch conduit and boxes only (no wiring) New conduit shall stub up through and above existing ceiling
• Install 3 television drops with conduit and boxes only (no wiring) @ 72 AFF. Conduit shall stub up through and above existing ceiling. Install wood backing at each location to mount televisions.
• Install two new 2x4 lights and 2 motion sensors 1 each in rooms 1 and 2. Installation shall include all conduit, wiring, boxes and switches operate motion sensors as well new and existing lights together.
Center new and existing lights in both rooms as feasible.
• Rotate and move as feasible two existing 2x4 lights in new hallway parallel to existing hallway walls.
Installation will include all conduit, boxes, wiring.
• Relocate and Reuse 1 existing switch and 1 motion sensor from room 1 to hallway wall. This switch and motion sensor will operator hallway lights only and will not include room 3 lighting. Installation will include all conduit, boxes, wiring.
• Install 1 new switch and motion sensor in room 3 only (separate from hallway) to operate existing lights. Installation will include all conduit, boxes, wiring, switches and motion detectors.
3.4 Mechanical:
• Install two new supply air ducts with in line dampers and diffusers. 1 in hallway and 1 in room 2. Install 3 new return grills in room 1, 2 and hallway.
• Test and balance entire area to include rooms 1, 2 and 3 and hallway.
• Relocate and Reuse 1 existing thermostat from room 1 to hallway wall. Installation will include all conduit, boxes, wiring.
3.5 Finishes:
• Carefully remove existing cove base in rooms 1, 2, 3 and hallway to prepare for new cove base.
• Install two metal frame solid wood doors with side light to include all hardware.
• Install finish trim in all locations where new sheet rock walls meet acoustical ceiling. Paint to match wall color.
• Paint rooms 1, 2, 3 and hallway-Benjamin Moore Manchester Tan (HC-81) Eggshell
• Install new cove base in rooms 1, 2, 3 and hallway - Johnsonite 29-Moonrock
• Hardware for doors – CoreMAX Best / Falcon
3.6 Miscellaneous:
• Relocate and re-use existing horn strobe from Room 1 to east end of hallway. Installation will include all conduit, boxes, wiring.
• Remove, save, store and re-use ceiling tile where applicable.
Building 1008:
3.1 This project includes the demolition and construction to completely demolish and replace two (2) existing restrooms. The contractor will be required to remove all existing finishes down to the bare studs or concrete block wall, ceilings, existing concrete floor, all plumbing fixtures, light fixtures, electrical devices, fire alarm devices, exhaust fans and ductwork, toilet accessories and toilet partitions to prepare the area for the new installation while not damaging the existing below floor piping. Following demolition, the contractor will re-pipe the restrooms to match the new fixture layout, provide new floors, ceilings, walls and finishes, lighting, electrical devices, fire alarm devices (where applicable), exhaust fans and ducted to the outdoors, toilet partitions and toilet accessories, all in compliance with the code and ADA requirements. The contractor will also replace the existing galvanized steel water service to the building.
3.2 Remove all existing plumbing fixtures, floor drains, toilet accessories and partitions, light fixtures, switches and receptacles, and exhaust fans and ductwork.
• Remove existing fire alarm devices and store for reuse.
• Existing fire alarm rough-in to remain and be reused.
• Do not damage existing underfloor utilities.
3.3 Remove existing water service down to floor. Plug line and patch floor. Removal and replacement must be coordinated with base and building occupants and occur during unoccupied hours as a scheduled utility shutdown.
3.4 Provide new 2” copper water service. Connect underground near new meter (2011) outside south wall. Rise up above ground and enter building through existing concrete south wall at plumbing chase. Seal penetration with “link-seal”. Insulate exposed pipe outdoors with 1” thick flexible elastomeric insulation, painted with UV protective paint.
Run service across chase, branch, with valve, running a parallel line to connect new fixtures. Provide access door for valve. Reconnect service to existing lines at water heater. Provide valves at branches to water heater, existing building and room 109. Insulate all cold and hot water pipes and fittings with ½” mineral fiber preformed insulation and ASJ jacket.
3.5 Provide new water and waste rough-in piping, using copper for water and PVC-DWV for waste and vent in room 104 and room 109 to accommodate:
• Room-104 to include one 36” ADA shower stall with thermostatic mixing valve, ADA compliant head, hand shower and slide rail, floor drain, hand rails and other accessories as required.
• One ADA compliant wall mounted flush valve toilet with carrier. Relocate existing plumbing to meet new location of toilet. Existing carrier may be reused with new bolts and trim.
• One ADA compliant wall hung lavatory with carrier, stops, supplies, grid strainer, offset P-trap and insulation kit for trap and supply.
• One floor drain, located in shower area. Provide with Trap primer.
• One hose bib under lavatory 18” above floor.
• Provide toilet partition with door to separate shower area from remainder of room, 30” floor mounted wood bench, shower curtain, two clothes/towel hooks toilet paper holder, paper towel dispenser, soap dispenser, mirror over lavatory, full length mirror on opposite wall, and grab bars as required.
• Provide factory manufactured water hammer arrestors as required, with valve and access panel.
• Provide new water and waste rough-in room 109 using copper for water and PVC-DWV for waste and vent to accommodate:
• Room-109 to include two shower stalls, one 36” ADA compliant with ADA compliant head, hand shower and slide rail, one with standard head, thermostatic mixing valves, floor drains, hand rails and other accessories as required. Showers are located at rear of room 108.
• One ADA compliant floor mounted flush tank toilet and one standard floor mounted flush tank toilet.
Demo and move existing floor drains to new location.
• One wall hung lavatories with carriers, stops, supplies, grid strainers, one with standard P-trap and one with offset P-trap and insulation kit for trap and supply.
• Two floor drains, one located in shower area one located in toilet room area. Provide with Trap primers.
• One hose bib under lavatories, 18” above finished floor.
• Provide toilet partition with door to separate shower area from remainder of room 110 two 30” floor mounted wood benches, shower curtains, four clothes/towel hooks, toilet paper holders, paper towel dispensers, soap dispensers, mirrors over lavatories, full length mirror on end wall of room 109, and grab bars as required.
• Provide factory manufactured water hammer arrestors as required, with valve and access panel.
3.6 Provide new ceiling mounted exhaust fans ducted to the outdoors. Increase size of existing openings as necessary and provide new wall caps
3.7 Provide new floor slabs, 4” think with 6x6 – 10/10 wire mesh centered. Slope floors to floor drains.
3.8 Provide new chase wall in bathroom. Construct of 2-1/2” metal studs with ½” water resistant GWB (“green board”) one side. Extend wall to 6” above ceiling.
3.9 Remove eight (8) existing windows, replace six (6) windows and fill two (2) openings in with block and stucco to match exterior and interior finishes. New windows shall be sealed and match recently replaced windows in facility.
3.10 Tile shower floors to ceiling (Rooms 104 & 109)
3.11 Tile floor to ceiling all walls room-104
3.12 Paint all walls utilizing provided paint codes by ANG / 125th CES. Paint all patched and new hard ceiling with provided paint codes
3.13 Remove and relocate fire pull station and horn/strobe in shower bath area
3.14 Contractor will construct hard framing to support hard ceiling in bathrooms, relocate fans / strobes, vents through hard ceiling.
Building 1009:
3.1 Demolition:
1) Contractor shall demolish one complete 10’x25’ awning, framework and associated hardware located on NW corner of Building 1009. 2) Contractor shall demolish two (2) separate staircases located at the North and South end of Building 1009. Total treads (28) Total Landings (2). ANG will remove direct satellite antenna from existing awning.
• 10’x25’Awning: Contractor shall in conjunction with the awnings Fabrication Building Co. measure and design final drawings necessary for construction of the 10’ X 25’ Awning. Contractor shall erect 1-10’x 25’ awning at same location that other awning was demolished based on drawings provided by the fabricator.
The awning shall have a mocha brown roof with a reflective white underneath. The roof panels shall be 24 gauge vertical R panel with boxed eaves and include all framing, fascia, trim, gutters, downspouts and connecting hardware. The framing shall be 26 gauge high strength galvanized steel. The awning shall meet the most recent SBC (see section 4) and all other applicable codes. All shop drawings and calculations are to be signed and sealed by the structure fabricators engineer.
• Concrete pad: Contractor will demo and remove 5’-0” x 5’-0” concrete landing pad on BLDG 1009 south side stairway. Contractor will form and pour new (3000 psi) 5’-0” x 5’-0” concrete landing pad at BLDG 1009 south side stairway prior to stairway install.
• Stairs: Contractor shall in conjunction with the Stairway Fabrication Building Co. measure and design final drawings necessary for construction of new galvanized steel staircase with 4’x5’ landings. Contractor shall erect 2 new galvanized steel staircase with a 4’x5’ landing at same location that other stairways were demolished based on drawings provided by the fabricator. All shop drawings and calculations are to be signed and sealed by the structure fabricators engineer. Contractor will detached existing canopy’s above north and south side of BLDG 1009 from existing stairway and secure such canopy’s after new stairway install.
SW- 4 GREEN PROCUREMENT
4.1. Building Insulation
Insulation made from recovered materials is available for thermal insulating applications. The product is available in several forms including rolls, loose-fill and spray foam. Insulation also can include a range of recovered materials such as glass, slag, paper fiber and plastics. One manufacturer grinds post-consumer glass bottles into a substitute for the sand used in glass fibers. Others use slag for rock wool or old newspaper for cellulose insulation.

4.2. Structural Fiberboard
Structural fiberboard is a panel made from wood, cane or paper fibers matted together which is used for sheathing, structural and insulating purposes. Examples include building board, insulating formboard, sheathing and acoustical and non-acoustical ceiling tile.
EPA's Recovered Materials Advisory Notice (RMAN) recommends recycled-content levels for purchasing structural fiberboard as shown in the table below.

4.3. Product Specifications
EPA recommends that procuring agencies use ASTM Standard Specification C 208 and ANSI/AHA specification A194.1. EPA further recommends that, when purchasing structural fiberboard products containing recovered paper, procuring agencies should do the following:
• Reference the technical requirements of ASTM C 208, "Insulating Board (Cellulosic Fiber), Structural and Decorative" Exit.
• Permit structural fiberboard products made from recovered paper where appropriate.
• Permit products, such as floor underlayment and roof overlay, containing recovered paper.
In addition, EPA recommends that procuring agencies review their specifications for insulating products and revise them as necessary to obtain the appropriate "R"-value without unnecessarily precluding the purchase of products containing recovered materials.
4.4. Faucets
Plumbing codes and standards require a flow rate of 0.5 gallons per minute (gpm) for public lavatory faucets and 0.25 gallons per cycle (gpc) for metered faucets. There are currently few products on the market with flow rates lower than these requirements. In addition, there is no documentation that lavatory faucets with flow rates lower than 0.5 gpm and
0.25 gpc perform in a satisfactory manner
4.5. Shower Heads
4.5.1 The showerhead shall conform to applicable requirements in ASME A112.18.1/CSA B125.1.1
4.5.2 If the showerhead has more than one mode, all modes must meet the maximum flow rate requirement outlined in Section 3.1.1 and at least one of the modes, as specified by the manufacturer, must meet all of the requirements outlined in this specification.
4.5.3 The showerhead shall not be packaged, marked, or provided with instructions directing the user to an alternative water-use setting that would override the maximum flow rate, as established by this specification.
Any instruction related to the maintenance of the product, including changing or cleaning showerhead components, shall direct the user on how to return the product to its intended maximum flow rate.
The flow rate of the showerhead shall be tested in accordance with the procedures in ASME A112.18.1/CSA B125.1 and shall meet the following criteria:
4.5.4 The manufacturer shall specify a maximum flow rate value (rated flow) of the showerhead. This specified value must be equal to or less than 2.0 gallons per minute (gpm) (7.6 liters per minute [L/min]).
4.6. Toilets

4.6.1 This specification establishes the criteria for a tank-type high-efficiency toilet under the U.S. Environmental Protection Agency’s (EPA’s) Water Sense program. It is applicable to:
• Single-flush, tank-type gravity toilets
• Dual-flush, tank-type gravity toilets
• Dual-flush, tank-type flushometer tank (pressure-assist) toilets
• Tank-type, flushometer tank (pressure-assist) toilets
• Tank-type electrohydraulic toilets
• Any other tank-type technologies that meet these performance specifications
4.6.2 The specification is designed to ensure both sustainable, efficient water use and a high level of user satisfaction with flushing performance.
4.7 General Requirements
4.7.1 The toilet shall conform to applicable water closet requirements in ASME A112.19.2/CSA B45.1,1 except as otherwise indicated in this specification.
4.7.2 If the toilet has dual-flush capabilities, it shall conform to requirements in ASME A112.19.14.
4.8 Water Efficiency Criteria
4.8.1 Single-flush toilets: The effective flush volume shall not exceed 1.28 gallons (4.8 liters) when evaluated in accordance with the sampling plan contained in 10 CFR 429.30.
For single-flush toilets, the effective flush volume is the average flush volume when tested in accordance with ASME A112.19.2/CSA B45.1.
4.8.2 Dual-flush toilets: The effective flush volume shall not exceed 1.28 gallons (4.8 liters) when evaluated in accordance with the sampling plan contained in 10 CFR 429.30.
For dual-flush toilets, the effective flush volume is the average flush volume of two reduced flushes and one full flush. Flush volumes shall be tested in accordance with SME A112.19.2/CSA B45.1 and ASME A112.19.14.
4.8.3 Samples with average flush volume in excess of 0.10 gallon (0.4 liter) greater than their rated flush volume shall be deemed to fail testing requirements due to excessive flush volume.2
4.9. Urinals
4.9.1 Urinal fixtures and flushing devices must meet criteria in the following areas, as applicable:
• The average maximum water consumption must not exceed 0.5 gallons per flush (gpf) (1.9 liters per flush [Lpf]), as specified in Section 3.0.
• The urinal fixture must conform to the requirements specified in Section 4.0.
• The urinal flushing device must conform to the applicable requirements specified in Section 5.0.
4.10 Water-Efficiency Criteria
4.10.1 The average maximum water consumption shall be tested in accordance with the following ANSI standards as applicable: ASME A112.19.2/CSA B45.1, ASME A112.19.3/CSA B45.4, or IAPMO Z124.91, and shall meet the following criteria:
4.10.2 The manufacturer shall specify a maximum flush volume (rated flush volume) of the flushing device or urinal fixture, which must be equal to or less than 0.5 gpf (1.9 Lpf).
4.10.3 The average maximum flush volume, determined through testing, shall not exceed the value specified in Section 3.1.1.

SW-5 CONTRACTOR MANPOWER REPORTING
Contractor Manpower Reporting Application (CMRA): The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for AIR FORC via a secure data collection site.
The contractor shall completely fill in all required data fields using the following web address: http://www.ecmra.mil/.
Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the help desk at http://www.ecmra.mil/.

SW-8 PERMITS/CLEARANCES
8.1 Construction Permit – Prior to commencement of work, the Contractor shall report to Customer Service Section in the Base Civil Engineering office, Building 28, telephone (904) 741-7611, to complete a construction permit form and login the construction start date. The Contractor shall identify other anticipated permits at this time to reduce lead times. Information obtained from the construction permit will be used for Entry Authorization Letters (EAL’s) and other permitting requirements. There is no fee for the Permit.
8.2 Work Clearance Request (AF Form 103) – A clearance request (also known as digging Permit) is required prior to excavation work. Permits can be obtained from the Customer Service Section at the Base Civil Engineering office.
The permit normally takes two Working days to coordinate utility clearances. There is no fee for the permit.
8.3 Security Clearance: The Florida Air National Guard base has 24-hour security and entry control procedures.
Contractors will be briefed on an ‘as needed’ basis concerning traffic routing and work site security restrictions. Base Access Letters for contractors are required and will be obtained at either the preconstruction conference or when requesting a construction permit. Information required for the Base Access Letters shall include individual’s complete name, company name, driver’s license number, project or contract number and title of the project. Base Access Letters are provided by the Base Civil Engineer and approved by The Chief of Security. A 125FW Contractor Badge will be issued to all workers prior to work start. This badge will be required to enter the base and worn at all times while on base. To obtain a 125FW Contractor Badge, all workers must submit a police records check from the Jacksonville’s Sherriff’s Office (JSO) to the 125th Security Forces Squadron Pass and ID Office. This records check must be the original and signed copy issued from JSO. Furthermore, before the request is accepted and processed, each member must present a government issued photo ID to the Pass and ID Office when submitting the request. Each identified contractor will ensure that the identified badge is properly displayed while performing work on the installation.
Also, each individual who is issued a badge must maintain control of his or her assigned badge at all times. If lost or misplaced, it must be reported to Security Forces immediately. Any contractor driving on the installation will be required to have valid driver’s license, proof of insurance, and vehicle registration. Weapons and firearms are not allowed on the base regardless of any issued weapon/firearm permits. One escort badge will be issued to the site super for the purpose of receiving deliveries.
"All contractor employees, to include subcontractor employees, requiring access to Government Installations, Facilities and Controlled Access areas shall complete AT Level I awareness training within 10 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. Certificates of completion for each affected contractor employee and subcontractor employee will be maintained by the COR or the Antiterrorism Officer. 8.4 Burn Permit: Obtain a burn permit prior to any welding processes. This permit can be obtained by contacting the 125th Fire Department at (904) 741-7636 or (904) 741-7637.
SW -7 PERFORMANCE PERIOD
Construction shall be completed within ninety (90) calendar days after notice to proceed is issued.
SW-8 SITE VISIT
A site visit for this project is optional but highly encouraged, as it allows contractors to become familiar with the site and surrounding areas and project scope.
