2. IDENTIFICATION NUMBER

3. DESCRIPTION/PURPOSE

To provide a current inventory of all hazardous materials. This information is required by the Fort Hood Department of Public Works (DPW) who provides the database template that must be completed to meet the requirements of this DID.

This DID is applicable to the U.S. Army Operational Test Command and Fort Hood to collect information on hazardous material maintained by the Contractor.

Distribution Statement A: Approved for public release; distribution is unlimited.

The public reporting burden for this collection of information is estimated to average 110 hours per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. Send comments regarding this burden estimate or any other aspect of this collection of information, including suggestions for reducing the burden, to Department of Defense, Washington Headquarters Services, Directorate for Information Operations and Reports (0704-0188), 1215 Jefferson Davis Highway, Suite 1204, Arlington, VA 22202-4302. 10. PREPARATION INSTRUCTIONS

10.1 The Government will provide a database to be completed by the contractor. The contractor shall provide a current Hazardous Material Inventory in the provided format. The inventory shall include the following information:

a. NSN

b. Nomenclature

c. Manufacturer

d. Unit of Issue

e. Quantity at the beginning of the month

f. Quantity received during the month

g. Quantity on hand at the end of the month

h. Material Safety Data Sheet (MSDS) number

i. Location of the hazardous material
