About this file
This is a Pre-Proposal Network Opportunities Form submitted by RADcube LLC in response to Request for Proposal 27-88343 issued by the Indiana Family and Social Services Administration's Office of Medicaid Policy and Planning. The RFP seeks managed care organization (MCO) proposals to provide statewide risk-based Medicaid services across four programs—Hoosier Healthwise, Healthy Indiana Plan, Hoosier Care Connect, and Indiana PathWays for Aging—collectively serving approximately 1.5 million Hoosiers. The form was submitted by Adam McKnight (Contact: 317-350-0080, bidmanagement@radcube.com) on behalf of RADcube LLC, located at 1119 Keystone Way, Suite 302, Carmel, Indiana. The pre-proposal conference is scheduled for September 10, 2026, with the formal proposal deadline of November 6, 2026. The contract term is four years with two optional one-year renewals available at the state's discretion, allowing for a potential six-year engagement.
Respondents must demonstrate current MCO service in at least one state and possess at least five years of prior MCO experience. The estimated contract value exceeds $4.9 billion in base capitation across all four programs for the initial rating period, with state fiscal year 2027 capitation rates varying by program: Hoosier Healthwise at $304.23–$305.42 per member per month (PMPM), Healthy Indiana Plan ranging from $228.72–$1,936.56 PMPM, Hoosier Care Connect at $1,448.47 PMPM, and PathWays for Aging at $3,468.29 PMPM. Evaluation criteria total 90 points (91 with bonus), including Management Assessment and Quality (75 points), Oral Presentations (5 points), Buy Indiana Initiative preference (5 points), and Indiana Veteran Owned Small Businesses subcontracting goal (5 points plus 1 bonus point). Contractors must be Indiana-licensed health maintenance organizations achieving NCQA accreditation by contract start or no later than the end of the second full calendar year of operation, maintain administrative offices within five miles of the Indiana Government Center in Indianapolis, and satisfy minimum Risk Based Capital ratios of 300 percent per NAIC standards. A pay-for-outcomes program will withhold 1.85–2.25 percent of capitation payments annually from 2029–2034, with estimated recovery of 60–70 percent based on performance against HEDIS and outcome measures.
