About this file
This is a Pre-Proposal Network Opportunities Form submission for Indiana's Medicaid Managed Care Organizations Request for Proposal 27-88343, submitted by UnitedHealthcare Community Plan to the Indiana Family and Social Services Administration's Office of Medicaid Policy and Planning. The RFP seeks managed care organization proposals to provide statewide risk-based Medicaid services across four programs—Hoosier Healthwise, Healthy Indiana Plan, Hoosier Care Connect, and Indiana PathWays for Aging—collectively serving approximately 1.5 million Hoosiers. UnitedHealthcare Community Plan submitted this optional attachment form by the September 10, 2026, 1:30 PM Eastern Time deadline via email to rfp@idoa.in.gov, with contact provided through Chad Piper, Chief Executive Officer, located at the company's Indianapolis office at 115 W. Washington Street. The full proposal deadline is November 6, 2026, with a pre-proposal conference occurring on September 10, 2026. The contract term is four years with two optional one-year renewals available at the state's discretion, for a potential total of six years.
The total estimated base capitation across all four programs exceeds $4.9 billion for the initial rating period, with state fiscal year 2027 rates varying by program: Hoosier Healthwise composite PMPM rates of $305.42 (for-profit) and $304.23 (not-for-profit); Healthy Indiana Plan rates ranging from $228.72 to $1,936.56 PMPM; Hoosier Care Connect composite rates of $1,448.47 PMPM; and PathWays for Aging composite rates of $3,468.29 PMPM. Respondents must be Indiana-licensed health maintenance organizations with current MCO service in at least one state and at least five years of prior MCO experience, achieving NCQA accreditation by contract start or no later than the end of the second full calendar year. Administrative offices must be located within five miles of the Indiana Government Center in Indianapolis. The Buy Indiana Initiative provides a mandatory 5-point preference, and Indiana Veteran Owned Small Businesses subcontracting carries a 3 percent goal with 5 points plus 1 bonus point available. Contractors are subject to a readiness review prior to implementation and a pay-for-outcomes program withholding 1.85-2.25 percent of capitation payments annually from 2029-2034, with estimated recovery of 60-70 percent based on HEDIS and outcome measure performance. Written questions are due September 11, 2026, with proposal discussions and oral presentations anticipated in May 2027 and Award Recommendation in July 2027.
