About this file
File Summary
This is a Q&A Responses document for RFP 27-88343, an Indiana Family and Social Services Administration solicitation for Medicaid Managed Care Organizations to serve four statewide programs: Hoosier Healthwise, Healthy Indiana Plan, Hoosier Care Connect, and Indiana PathWays for Aging, collectively serving approximately 1.5 million beneficiaries. The RFP requests managed care services across multiple programs with proposals due November 6, 2026, at 3:00 PM ET. A pre-proposal conference was held September 10, 2026, with written questions accepted in two rounds: first round due September 11, 2026 with responses provided by October 2, 2026, and second round due October 9, 2026 with responses by October 23, 2026. The contract term is four years beginning January 1, 2029, with two optional one-year renewal periods available at the State's discretion. Proposal discussions, oral presentations, and Best and Final Offers are anticipated in May 2027, with Award Recommendation expected in July 2027. Respondents must be Indiana-licensed health maintenance organizations with a minimum of five years of MCO experience currently operating in at least one state, achieve NCQA accreditation by contract start or no later than the end of the second full calendar year, and maintain administrative offices within five miles of the Indiana Government Center in Indianapolis.
Capitation rates for state fiscal year 2027 include Hoosier Healthwise at $305.42-$304.23 PMPM (serving approximately 589,000 children and pregnant women), Healthy Indiana Plan ranging from $228.72-$1,936.56 PMPM plus maternity case rates (serving approximately 487,000 adults aged 19-64), Hoosier Care Connect at $1,448.47 PMPM composite (serving approximately 80,000 individuals with disabilities or in state custody), and PathWays for Aging at $3,468.29 PMPM composite (serving approximately 115,000 seniors aged 60+). Total estimated base capitation across all programs exceeds $4.9 billion for the initial rating period. Evaluation criteria total 90 points (91 with bonus) including Management Assessment and Quality (75 points), Oral Presentations (5 points), Buy Indiana Initiative preference (5 points), and Indiana Veteran Owned Small Business subcontracting (5 points with 1 bonus point). Contractors must maintain minimum Risk Based Capital ratios of 300 percent, obtain fidelity bond and professional liability insurance, and purchase commercial reinsurance with attachment points not exceeding $2 million and co-insurance responsibility capped at 20 percent. A pay-for-outcomes program will withhold 1.85-2.25 percent of capitation payments annually from 2029-2034, with estimated recovery of 60-70 percent based on performance against HEDIS and outcome measures.

| 22 | RFP Instructions | 3.2 | 21 | Mandatory Requirements | Please confirm that the below reference to "last two calendar years" means the 24 months prior to the proposal due date? 11/6/24 - 11/6/2026 | ||
| "Additionally, a Respondent must not have had any State contract or purchase order terminated or cancelled within the last two calendar years due to Respondent’s failure to deliver products or services timely and in accordance with all terms of an accepted proposal or a State contract." | Yes, this is referring to the 24 months prior to the proposal due date. | ||||||
| 23 | Att E - Business Proposal | 6 | 3 | References | Please confirm that Indiana can not be used as a reference for H1. | Using the State of Indiana as a reference is not recommended, but not prohibited. | |
| 24 | Att F1 - Technical Proposal Response Template | B.4 | 3 | Administrative Requirements and Information Technology Systems | Attachment F instructs Respondents to provide a staffing chart "assuming four awarded contractors, each with approximately equal proportions of members." Please confirm whether the member months and enrollment assumptions underlying the capitation rate certifications in Attachment S should serve as the basis for this calculation, and if not, identify the enrollment figure the State intends Respondents to use. | Yes | |
| 25 | Att F1 - Technical Proposal Response Template | A.3 | 2 | Known litigation, administrative or regulatory proceedings, or similar matters. | Please confirm that consistent with past Indiana RFPs, bidders can (1) exclude from disclosure non-material proceedings, and (2) limit their disclosures to those involving government funded healthcare plans of the bidder and/or any related holding company, parent company, subsidiary, or intermediary company. | It is permissible to exclude non-material proceedings. | |
| 26 | Att F1 - Technical Proposal Response Template | A.3 | 2 | Known litigation, administrative or regulatory proceedings, or similar matters. | The instructions ask that the respondent identify instances in which "you or any related holding company, parent company, subsidiary, or intermediary company…" Please confirm the intent of this clause is to include any company under common control with bidder's parent company that operates a government funded healthcare plan. | Disclosures may be limited to government funded healthcare plans. | |
| 27 | Att F1 - Technical Proposal Response Template | A.3 | 2 | Known litigation, administrative or regulatory proceedings, or similar matters. | The instructions ask for disclosure of "known litigation, administrative or regulatory proceedings, or similar matters." Please confirm that "administrative or regulatory proceedings" refers to enforcement proceedings by state agencies. | Yes | |
| 28 | Att F1 - Technical Proposal Response Template | A.3 | 2 | Known litigation, administrative or regulatory proceedings, or similar matters. | Please confirm the 5-year lookback period noted in question A.3. is from the date the RFP was released on 8/27/26? | Yes | |
| 29 | Att P3 - Exhibit 4 Hoosier Care Connect - Responsibilities of the State | 7 | 230 | Quality Improvement Goals and Member Incentive | Please confirm the State-defined measurable objectives, associated performance measures, and performance targets for each Quality Improvement Goal listed in section 7.0. | As indicated in Exhibit 2, 2029 Pay for Outcomes measures will be defined at a later date, including any program specific measures. | |
| 30 | Att E - Business Proposal | 13 | 8 | Subcontractor Information Requirements | Could you clarify whether the required subcontractor information and Letters of Agreement should be submitted with Attachment J (Attestation Form) or Attachment E (Business Proposal)? | Subcontractor information and letters of commitment should be submitted as separate files. | |
| 31 | Att E - Business Proposal | 6 | 3 | References | The RFP requests five Attachment H1 forms "from states for whom the Respondent has provided services that are the same, or similar, to those services requested in this solicitation." Because this solicitation is for full-risk Medicaid managed care delivered under a direct contract with the single state Medicaid agency, please confirm that each Attachment H1 reference must be provided by a state Medicaid agency with which the Respondent holds a direct, at-risk Medicaid managed care contract, and that references arising from Medicare-only or coordination-only D-SNP arrangements, ACA Marketplace or commercial insurance contracts, third-party administrator engagements, or subcontracted, delegated or partnership arrangements with another managed care entity would not satisfy this request. | The State prefers references from states with programs that are similar to Indiana's program. | |
| 32 | Att H1 - Reference Form | Reference Form | Attachment H1 forms are completed by the reference state and submitted directly to IDOA at idoareferences@idoa.in.gov. Will the State provide a mechanism for Respondents to confirm receipt of their Attachment H1 forms with IDOA prior to the November 6, 2026 deadline? For example, a confirmation of receipt to the Respondent's designated Internal Response Manager, or a status inquiry process through the point of contact identified in the solicitation? | If you wish to confirm reference submissions with IDOA, please reach out to Robert Cohen (rcohen@idoa.in.gov) and Mitch Roelecke (mroelecke@idoa.in.gov). | |||
| 33 | Att K3 - Exhibit 1 Hoosier Care Connect - Statement of Work | 2.3.4 | 31 | Other Required Staff Positions | The description for the Member Advocate/Non-Discrimination Coordinator indicates that this position reports to the Community Initiatives Director. Is the Community Initiatives Director considered a Key Staff or Required Staff position under the RFP requirements? Additionally, could the State please provide a description of the Community Initiatives Director role? | The Community Initiatives Director is not a position. The Member Advocate/Non-Discrimination Coordinator has been edited to remove this reference. Please see Addendum 1. | |
| 34 | Att E - Business Proposal | Section 6.0 | 3 | References | Section 6.0 of Attachment E provides that Attachment H1 must be completed by the reference state and emailed directly to idoareferences@idoa.in.gov, which means Respondents lack visibility into whether their references have submitted the required forms by the due date. During the procurement period, may a Respondent contact the State to confirm that reference documents have been received? If so, we would appreciate guidance on the preferred contact method and timing for requesting such confirmation. Because submission of Attachment H1 rests with third-party references rather than the Respondent, the ability to confirm receipt would allow Respondents to proactively follow up with their references and help ensure the State has the complete information it needs to evaluate proposals. | If you wish to confirm reference submissions with IDOA, please reach out to Robert Cohen (rcohen@idoa.in.gov) and Mitch Roelecke (mroelecke@idoa.in.gov). | |
| 35 | Att F1 - Technical Proposal Response Template | N/A | 1 | Submission Specifications | Will the State confirm that it is permissible to use a font smaller than 10-point Arial for graphics, tables and callout boxes within the response? | Please see response for Question #152 | |
| 36 | Att F1 - Technical Proposal Response Template | N/A | 1 | Submission Specifications | To help delineate sections within the Technical Proposal, will the State confirm section divider pages may be used and will not count toward the 200-page limit? | Please see response for Question #77 | |
| 37 | Att F1 - Technical Proposal Response Template | Section B, Question 4 | 3 | Staffing Chart | Will the State allow respondents to submit the comprehensive staffing chart required in Question B.4 as an attachment that does not count toward the 200-page limit? | The staffing chart is not included in the page count limit. Please see Addendum 1. | |
| 38 | Att F1 - Technical Proposal Response Template | Section B, Question 12 | 3 | IT Systems Diagram | Will the State allow respondents to submit the IT Systems Diagram required in Question B.12 as an attachment that does not count toward the 200-page limit? | Please see response for Question #49 | |
| 39 | Att F1 - Technical Proposal Response Template | Sections B, C, D, E | 2,4,8 | SOW Enhancement Tables | To fully and concisely respond to all technical proposal questions within the 200-page limit, will the State permit the enhancement table(s) required in B.1, C.1, D.1 and E.1 to be submitted as a separate, page-limited attachment that does not count toward the 200-page technical proposal limit? Under this approach, Respondents would provide a complete narrative response for each question within the 200-page limit, while the separate attachment would be limited to the required enhancement tables and could be held to a State-specified page limit per attachment. | Please see response for Question #48 | |
| 40 | Att F1 - Technical Proposal Response Template | Section C, Question 11 | 5 | Children's Mental Health Wraparound | Question C.11 states "Describe your plan to manage the delivery of MRO, 1915(i), and CCBHC services." Children's Mental Health Wraparound (CMHW) is not specifically listed among the carved-in behavioral health services in Section 3.5.5 (Att. K1, K2, K3, K4), but it is included in the definition of 1915(i) services in Section 3.5.5.2, which further states that the Contractor is responsible for reimbursement of 1915(i) services. Can the State clarify whether CMHW is intended to be carved into managed care as with BPHC and AMHH? | Yes, it is carved-in. | |
| 41 | Att F2 - Outcomes Measures Response Template | N/A | N/A | RDM and LDM Outcomes Measures | The Attachment F2: Outcomes Measures Response Template requests reporting of the Race Diversity Measure (RDM) and Language Diversity Measure (LDM). However, the data source available contains multiple race and language subcategories, and we are unable to identify a single aggregate value that corresponds to the requested measure result. Additionally, the response template does not specify a specific race/language category, or results for all available subcategories. |
Can the State please clarify the expected reporting methodology for RDM and LDM, including:
> Whether respondents should report an overall/aggregate measure score or individual category-level results?
> If an aggregate score is required, the specific data element, calculation methodology or category that should be used?
| > Whether all race and language categories available in the source data should be reported separately if no aggregate measure score exists? | Respondents should report the separate, final NCQA Health Plan Rating measure rating for RDM and LDM, rather than individual race, ethnicity, or language category results, consistent with NCQA Health Plan Ratings Methodology. | ||||||
| 42 | Att K4 - Exhibit 1 Indiana PathWays for Aging - Statement of Work | Section 2.3.4 | 31-32 | Community Initiatives Director Position | The role description for Member Advocate/Non-discrimination Coordinator indicates that this position reports to the "Community Initiatives Director." However, no such role is listed in the required staffing model. Can the State please clarify the role of the Community Initiatives Director and confirm if it is a required position? | Please see response for Question #33 | |
| 43 | RFP Instructions | N/A | N/A | Value-Added Services | We are unable to locate a definition or scope of Value-Added Services in the RFP or Statements of Work. Can OMPP define Value-Added Services and clarify the types of offerings that should be classified as Value-Added Services? Additionally, can OMPP clarify how Value-Added Services requested in Technical Proposal Question C. 48 are intended to differ from the enhanced services requested in Question C. 18 and member incentives in Question E.9? | Please see response for Question #4 | |
| 44 | RFP Instructions | Section 1.3 | 7 | HMO Licensure | RFP Instructions, 1.3 and Section 2.1 (State Licensure) of each Statement of Work requires the Contractor to be an Indiana-licensed health maintenance organization (HMO). The corresponding provision in the current Medicaid managed care contracts permits the Contractor to be "an Indiana licensed accident or sickness insurer or an Indiana licensed health maintenance organization (HMO)." Does the State intend to require that the Contractor hold an Indiana HMO license issued under IC 27-13, or will the State also accept an entity that is an Indiana-licensed accident and health insurer, consistent with the current contract terms? | Contractors must be an Indiana-licensed HMO. | |
| 45 | Att F1 - Technical Proposal Response Template | N/A | 1 | Submission Specifications | Attachment F.1 - Technical Proposal Instructions outlines instructions including the use of 10-point font size set in Arial for narrative responses. Is a smaller size permissible for included charts, diagrams, graphics, headers/footers, and tables? May included visuals be presented in the respondent's brand-designated fonts (Helvetica)? | Please see response for Question #152 | |
| 46 | Att F1 - Technical Proposal Response Template | B.2 | 3 | Staffing Plan and Staffing Chart | Attachment F.1, B.2 requests a detailed staffing plan across all four managed plans. Please confirm that the detailed staffing plan requested can be submitted as a separate attachment and not included in the 200-page limit. | Please see response for Question #37 | |
| 47 | Att F1 - Technical Proposal Response Template | Question B.4 | 3 | Comprehensive Staffing Chart | Attachment F.1, B.4 requests a comprehensive staffing chart. Please confirm that the comprehensive staffing chart requested can be submitted as a separate attachment and not included in the 200-page limit. | Please see response for Question #37 | |
| 48 | Att F1 - Technical Proposal Response Template | Question B.1, C.1, D.1, and E.1 | 3, 4, 8, 9 | Tables for Enhancements beyond the Statement of Work Requirements | Please confirm that the tables requested in Attachment F.1, Questions B.1, C.1, D.1, and E.1 regarding enhancements beyond the Statement of Work Requirements can be submitted as separate attachments and not included in the 200-page limit. | Enhancement table(s) are included within the page limit. When making these tables, template rows not relevant to the specific response can be removed. | |
| 49 | Att F1 - Technical Proposal Response Template | B.12 | 3 | IT Systems | Attachment F.1, B.12 requests a general IT systems description and IT systems diagram. Please confirm that the IT systems description and IT systems diagram requested can be submitted as a separate attachment and not included in the 200-page limit. | IT systems description and diagrams are included in the page limit. | |
| 50 | Att F1 - Technical Proposal Response Template | C.22 | 5 | Care Coordination Program Plan | Attachment F, Question C.22 asks Bidders to provide a draft Care Coordination Program Plan (CCPP) that we would propose for Indiana Medicaid. In reviewing each of the four Statement of Works (K.1, K.2, K.3, and K.4), each make reference to the following same statement within Section 4.0 Care Coordination: "Refer to the Care Coordination Program Manual for more information on Complex Case Management." While the Bidders Library (Attachment C) includes policy manuals that include references to care coordination, a Care Coordination Program Manual was not included. Please provide the cited Care Coordination Program Manual. | This language will be removed from the Statement of work "Refer to the Care Coordination Program Manual for more information on Complex Case Management". Please see Addendum 1. | |

Is a master's degree required for complex care social work or only physical and behavioral health? | Please see Addendum 1.
| 53 | Att K4 - Exhibit 1 Indiana PathWays for Aging - Statement of Work | Section 4.7 | 137 | Health Assessment Timeframe | Section 4.7 states that, for NFLOC members receiving HCBS, the Comprehensive Health Assessment Tool (CHAT) must be completed within sixty (60) calendar days of enrollment. However, the subsequent CHAT requirements indicate that a Full CHAT for members who meet NFLOC and are receiving HCBS must be completed within thirty (30) calendar days of enrollment or identified need. |
| Please clarify the required timeframe for completion of the initial Full CHAT for NFLOC members receiving HCBS and indicate which timeframe should govern for proposal and implementation planning purposes? | The CHAT must be completed within sixty (60) calendar days. Please see Addendum 1. | ||||||
| 54 | Att R1 - Exhibit 6 Hoosier Healthwise - Capitation Rates | n/a | 1 | Rate Development | In all Attachmetn Rs (Att R1, Att R2, Att R3, & Att R4), for planning purposes, does FSSA anticipate providing awarded Contractors with additional actuarial and rate-development information in advance of initial and final rate negotiations? If so, please indicate the types of information that may be made available, such as: | ||
| • | Detailed base-data summaries | ||||||
| • | Adjustment exhibits | ||||||

All of the types of information listed in the question would be reasonable items to expect.
| 55 | RFP Instructions | 1.3 | 6 | Knowledge Transfer | To support implementation planning and knowledge transfer for awarded Contractors, does FSSA anticipate facilitating peer-to-peer discussions, transition meetings, or other operational knowledge-sharing opportunities involving incumbent and newly awarded MCOs? If so, please describe the anticipated approach and timing. | Implementation and transition planning with be addressed after award. | |
| 56 | RFP Instructions | 1.8 | 11 | Due Date for Proposals | Please advise if there is a file size limitation on the electronic upload site. | We're not aware of a file size limit in the IDOA Supplier Portal. | |
| 57 | RFP Instructions | 3.2.6 | 24 | Qualified State Agency Preference Scoring | The Qualified State Agency Preference Scoring provision is identified as Section 3.2.6 in the RFP but identified as Section 3.2.8 in Attachment J. Please confirm the correct section number. | 3.2.6 is correct. | |
