EXECUTIVE SUMMARY

The Defense Health Agency (DHA), Contracting Division-Professional Services (CD-PS) is soliciting proposals for the Remote Health Readiness Program-4 contract to provide medical and dental readiness services to the military Service Components of the Reserve Components, Active Components enrolled in TRICARE Prime Remote, DoD Service Civilians, and members of the Department of Homeland Security.

This solicitation will utilize full and open competition to award a firm-fixed-price (FFP) and cost reimbursable (CR) (no fee) single-source, Indefinite Delivery Indefinite Quantity (IDIQ) contract with the ability to issue task orders. The total maximum dollar value (program ceiling) for all task orders placed under this contract is $1,610,000,000. The guaranteed minimum dollar value for the entire contract period of performance is $11,000,000. Because this is an IDIQ contract, the awardee must be capable of performing the entire anticipated requirement of $1,610,000,000.

It is the Government’s intent to award Task Order 0001, for the Transition-in effort, at the execution of the basic contract award. The total period of performance for the RHRP-4 requirement consists of 10 years if all options are exercised.

The basic IDIQ contract will have all pricing for procedures included as an attachment (see Attachment E (Pricing Sheet) provided with the solicitation). Task Orders will be issued per each Service Component’s requirements on an annual basis (except that Task Orders issued during the initial 3–6-month base period will not be annual orders). All pricing will be in accordance with the Attachment E Pricing Sheet. The ordering will be accomplished by each Service Component submitting its funding for services—which are described in the PWS and detailed in the Procedures list in the Pricing Sheet. Each Task Order will be issued for a period of performance (PoP) during which the contractor will provide services.

The contract option periods will be exercised in accordance with FAR 52.217-9 for the initial 3– 6-month base ordering period and each subsequent 12 month ordering period, during which periods DHA will issue task orders for the requirements of each of the Service Components.

Ordering Period

The Government anticipates the ordering period for this IDIQ contract will begin in the first or second quarter of fiscal year 2026 and extend until the last day of the last exercised contract option. All task orders must be issued before the expiration of the IDIQ contract; however, the PoP for individual task orders may extend beyond the base IDIQ period, provided such extensions are permitted under the contract terms.

Authorized Ordering Officials

Only the Defense Health Headquarters (DHA), PS-CD, HT0011 Contracting Office is authorized to issue orders under this IDIQ. The Contracting Officer (KO) will serve as the final authority on all contractual matters and have overall responsibility for contract administration, including order issuance, modifications, and ensuring compliance with contract terms. Any modifications to orders must be executed in writing by the Contracting Officer; no other official will be authorized to make changes to contractual terms or obligations.

Types of Orders

Orders issued under this IDIQ contract may be structured as FFP orders, covering defined services (procedures) at a fixed price; CR orders, which are limited to shipping and travel expenses, as permitted under the contract; or mixed FFP and CR orders.

Order Issuance

All orders will be issued using SF-1449 and will include the contract number, order number, description of work, period of performance, and price or cost structure. Orders will also incorporate the applicable contract clauses and terms outlined in the IDIQ agreement to ensure consistency with contract requirements.

Order Administration

The Contracting Officers Representative (COR) will be responsible for monitoring contractor performance and ensuring compliance with order requirements. The Contractor will submit invoices via Wide Area Workflow (WAWF) in accordance with DFARS 252.232-7006 and the invoicing instructions specified in the IDIQ base contract. The contractor will be required to comply with all reporting and deliverable submission requirements as outlined in the contract.

For shipping and travel-related costs, reimbursement will be based on actual costs incurred, subject to Government review and approval. The Government’s negotiated shipping discounts through carriers such as UPS and FedEx will be extended to the contractor. No burden rates, indirect rates, or G&A may be applied to shipping or travel expenses, and Not-to-Exceed (NTE) cost limits will apply to all travel and shipping CLINs.

To support invoicing for shipping, the contractor will be required to provide copies of shipping invoices and receipts detailing transportation costs, along with shipping manifests or tracking documentation confirming shipment details. Any applicable customs or freight documentation verifying transportation expenses must also be submitted. For travel reimbursement, all expenses should comply with the Federal Travel Regulations (FTR) and contract terms. The contractor will be required to submit a detailed travel invoice that includes the travelers name and title, travel dates and destinations, purpose of travel with justification, itemized expenses, and the total reimbursable amount.

PART 1

1. GENERAL INFORMATION

This is a non-personal services contract to provide medical and dental readiness services to the military Service Components (SCs) of the Reserve Components (RCs), Active Components (ACs) enrolled in TRICARE Prime Remote (TPR), DoD Service Civilians, and members of the Department of Homeland Security (DHS).

1.1 Introduction: The Defense Health Agency (DHA), per Department of Defense Instruction (DoDI) 6025.19, is responsible for executing the Reserve Health Readiness Program.

DHA Procedural Instruction (DHAPI) 6200.06 assigns the Director, DHA responsibility for developing, executing, and managing the DoD’s Periodic Health Assessment (PHA) Program. In 2024, the Reserve Health Readiness Program Core Integrated Product Team changed the name of the program from the Reserve Health Readiness Program to the Remote Health Readiness Program to align more accurately with the population served by the contract. The Remote Health Readiness Program-4th Generation will be referred to as RHRP-4.

1.1.1 The mission of the Remote Health Readiness Program (RHRP) is to support and complement the DoD Individual Medical Readiness (IMR) Program in its efforts to promote and sustain a medically ready force throughout the continuum of military operations and deployments. The RHRP is DHA’s primary support for RC and TPR Service Members (SMs).

The RHRP provides required IMR (Medical and Dental) services, deployment readiness medical services, physical exams (accession, retention, occupational health, and specialty), and RC dental care. These activities support the Services’ efforts to meet or exceed established DoD IMR standards, deployment health-related initiatives, and Force Health Protection (FHP) requirements and goals. The RHRP supports the RC, TPR services members (Army, Navy, and Marine Corps), U.S. Coast Guard SMs (active duty and Reserve) and re-deploying DoD civilians from participating DoD Agencies.

1.2 Background: The RHRP is a recurring requirement since 2001. The optional use contracting vehicle provides capabilities to ensure medical readiness requirements are met for each SC. The requirement for medical readiness support to the Reserve Components has been met through previous contracts such as FEDS HEAL (Federal Strategic Health Alliance), RHRP- I, RHRP-II, and currently RHRP-3. In FY21-FY22, the RHRP contractor provided approximately 2.8M discrete services each year costing, on average, $154M per year.

1.3 Objectives: The Contractor shall provide medical, dental, and behavioral health services in the form of assessments, physical exams, laboratory services, diagnostics, audiology services, optometry services, immunizations, dental exams, and dental treatment. The Contractor shall provide services in the United States (the 50 States and the District of Columbia), the Commonwealths of Puerto Rico and the Northern Mariana Islands, the territories of American Samoa, Guam, and the U.S. Virgin Islands. The government may extend services to additional OCONUS sites, such as in Germany and Japan. Before extending services to any additional OCONUS locations, the Government would first modify the contract’s PWS and CLIN structure to accommodate the requirement. The Contractor shall provide services using the following three modalities: Group Events, In-Clinic appointments, and Virtual Health.

1.4 Scope: The Contractor shall establish and maintain a robust network of credentialed providers in all necessary practice areas to provide on-demand medical and dental readiness services based on SC readiness requirements and as described in Part 5 of this Performance Work Statement (PWS). The Contractor shall provide both face-to-face services and person-to-person services. The Contractor shall create, operate, and maintain both an electronic scheduling capability and a telephonic capability for SMs, SC Leads, and SC delegated personnel to request services by type and modality. The Contractor shall provide assessments, physical exams, laboratory services, diagnostics, audiology services, optometry services, immunizations, dental exams, and dental treatment. The Contractor shall electronically document the results of the services completed into the appropriate system(s) of record and/or DoD/DHS Component health readiness systems for each SM receiving services. (see Exhibit A_RHRP-3 Systems Map)

1.5 Period of Performance (PoP): The PoP begins with a not to exceed 9-month transition plus a 3–6-month base period (depending on the length of the transition-in period), which is the first year (base year) of the contract, and nine 12-month option years, for a total of ten years.

1.6 Transition:

1.6.1 Transition-in period. The transition-in period is defined as the period during which the contractor prepares to meet all contract requirements, ensuring (among other things) that all incoming personnel, to include network providers, are prepared, functionally trained, and qualified by the full performance start date. The transition-in period will not exceed nine months.

The full performance start date is not later than 22 Aug 2026. The Contractor shall provide the Government a detailed incoming transition plan for phasing-in contractor capabilities and performance due no later than (NLT) 10 business days after award. (Deliverable 1). This should be substantially the same transition plan that accompanied the Contractor’s proposal; the Contractor shall execute to the schedule and all associated milestones described in its plan. The Contractor’s transition-in plan shall discuss the activities the Contractor will carry out to (among other things) establish all required capabilities, establish all required IT connections, and ensure a smooth implementation of all aspects of service and support covered under this PWS. In the event that responsibility for fulfillment of ordered or ongoing tasks, either in whole or in part, must be transferred to the Contractor from the outgoing vendor, the Contractor shall participate in meetings with Government staff and representatives of the outgoing vendor to facilitate the hand off and completion of such tasks.

1.6.1.1 The Contractor shall include in its transition-plan the process flows it will employ for data retrieval of a SM’s readiness data to receiving the SM’s request for a scheduled service to publishing the electronic documentation in the appropriate DoD/DHS system once the scheduled service is completed. The Contractor’s transition-in plan shall describe the processes it will use for securely receiving contract data, including government data and CUI, from the outgoing vendor through encrypted transmission (as applicable). The transition-in plan shall contain a communication plan detailing necessary information regarding the new contract, any change in phone numbers, personnel, and websites to provide to the COR.

1.6.1.2 Additionally, the contractor shall ensure appropriate subject matter experts actively participate in systems integration interface meetings to identify all Defense Enrollment Eligibility Reporting System (DEERS) systems interfaces and applications (see PWS paragraph 6.12.2) necessary to meet the requirements of this contract. (For background information about DEERS and interfacing with DEERS, see the TRICARE Systems Manual, Chapter 3 at https://manuals.health.mil/pages/DisplayManualHtmlFile/2025-01- 08/AsOf/TST5/C3TOC.html.)

1.6.2 Transition-Out Period. The Contractor shall, upon the Contracting Officer’s written notice furnish phase-out services for up to 180 calendar days in the final exercised option year of the contract. The Contractor shall provide sufficient experienced personnel during the phase-out period to ensure that the services called for by this contract are maintained at the required level of security and proficiency.

1.6.2.1 The Contractor shall prepare and deliver to the Government a transition out plan that facilitates the accomplishment of a seamless, low-risk transition to the incoming contractor at the expiration of the contract. The Transition Out Plan is due NLT 90 calendar days prior to end of the base year of the contract, and the Contractor shall update it with each option period exercised thereafter (Deliverable 2). The Contractor’s transition-out plan shall describe the process it will use to transfer government data and Personal Health Information (PHI)/Personal Identifiable Information (PII) (e.g., all CUI and other information collected and stored by the Contractor in communicating with SMs and SC representatives and performing services under this contract) to the successor contractor and/or the Government. The Contractor’s plan shall also describe how it will provide data and reports created from the telephonic scheduling process, as well as digital recordings and transcripts (see Paragraph 5.5.4), to the successor contractor and/or the Government.

1.6.2.2 When requested by the Government, in the event that responsibility for fulfillment of the tasks described in this PWS, either in whole or in part, will be transferred to a new contractor or the Government, the Contractor shall participate in transition-out meetings with the PM, project staff, and representatives of the successor contractor and/or Government personnel. The purpose of these meetings will be to review project materials and take preparatory steps to ensure an effective transition in contractor support. When requested, the contractor shall develop, document, and monitor the execution of a Transition-Out Plan that may be used to transition tasks and materials described in this PWS to a new contractor or to the Government.

1.6.2.3 The Contractor shall comply with transition-out requirements of the DHA for contractors who have been issued a Common Access Card (CAC) or who generate “records,” as defined by DoDI 5015.02.

1.7 Administrative specifications

1.7.1 Place of performance: The Contractor shall provide readiness services within CONUS and OCONUS (see Paragraph 1.3) by each of the three modalities: in-clinic, group event, or virtual health.

1.11 Quality

1.11.1 Quality Control (QC): The Contractor shall develop and maintain an effective QC program to ensure services are performed in accordance with this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure nonrecurrence of defective services. The Contractor’s QC program shall document that its work complies with stated requirements. The Contractor shall submit an initial QC Plan (QCP) within 30 days after contract award and an updated QCP within 5 business days after incorporating any updates/changes to the plan (Deliverable 3).

1.11.2 Quality Assurance (QA): The Government will evaluate the Contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). The QASP provides a systematic method for the Government to evaluate performance and to ensure that the Contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

1.12 Contractor personnel

1.12.1 Common Access Card (CAC) requirements. The Contractor shall identify those contractor employees who will require a Government issued Common Access Card (CAC) based on their duties and required level of DHA network access. To obtain CACs, the Contractor Facilities Security Officer (FSO) shall provide the COR the required information per the DHA CAC request process (Exhibit XX). The Contractor shall comply with onboarding requirements of the DHA for contractor employees needing CAC identification, including complying with DoD- and DHA-directed training and forms submission and appropriate background investigations prior to network forms access. (Exhibit YY_DMDC TASS)

1.12.1.1 The Contractor shall return the CACs of departing contractor employees to the COR within three business days after the employees depart and shall return all CACs when the contract ends (Deliverable 4).

1.12.2 Personnel Security. All Contractor employees identified as needing a CAC (see PWS para 1.12.1) shall have a minimum of a TIER 1 adjudicated investigation for Non-Critical, Non- Sensitive positions.

1.12.3 The Contractor shall identify a limited number of personnel who will support the design, development, operations, and security of Information Systems (IS) who shall complete a minimum of a TIER 2 adjudicated Non-Critical, Sensitive investigation.

1.12.4 Contractor onboarding and training. Contractor employees shall complete all requirements, training, and forms (as applicable) prescribed in Exhibit D_Required Training (taking into account the training timing and frequency requirements), as well as the following requirements (as applicable): (PRS #1)

1.12.4.1 Prior to accessing the DHA network, the Contractor shall complete the following:

1.12.4.1.1 Sign a DHA Acceptable User Policy (AUP), and

1.12.4.1.2 Complete DHA-approved training for Health Insurance Portability and Accountability Act (HIPAA) and Privacy Act and DoD Cyber Awareness Challenge initially within 30 days of entry on duty and annually thereafter.

1.12.5 Licensing and Compliance: (PRS #2)

1.12.5.1 The Contractor shall ensure that all Contractor providers (of all types) and nurses who deliver services under this contract (whether as direct Contractor employees or as part of the Contractor’s network of providers) are currently licensed to render professional health care services in each state in which they render services to SMs and/or satisfy the special license portability requirements described in Title 10 United States Code (USC) Section 1094(d)(2) (i.e., relating to providing “an examination or assessment under [10 USC] Section 10206”). The license must be at full clinical practice level to meet this requirement. A temporary license at the full practice level is acceptable. When a state does not license a specific category of individual professional provider, certification by a qualified accrediting organization is required.

Certification must be at full clinical practice level. A temporary certification at the full clinical practice level is acceptable. The Contractor shall ensure that providers comply with any applicable state immunization requirements for health care providers.

1.12.5.2 The Contractor shall instruct any providers rendering telehealth/virtual health services (as that term is defined by applicable state laws) to follow telehealth-specific security, regulatory, licensing, credentialing and privileging, malpractice and insurance laws and rules for their profession in both the jurisdiction (site) in which they are practicing as well as the jurisdiction (site) where the patient (SM) is receiving care, and shall ensure compliance as required by appropriate regulatory and accrediting agencies.

1.12.6 Malpractice Insurance: The Contractor shall determine the medical malpractice coverage required in the state for each of its providers (whether employed by the Contractor or a provider in the Contractor’s network) and ensure that each provider complies with this requirement. In the absence of a state law requirement for medical malpractice insurance coverage, the Contractor shall determine the local community standard for medical malpractice coverage and the Contractor shall maintain necessary documentation evidencing both the standard and compliance by its providers. In no case shall any provider (e.g., physician, dentist, nurse practitioner, physician assistant, advance practice nurse, advance practice registered nurse, audiologist, optometrist, or behavioral health specialist) delivering services under this contract not have the minimum required medical malpractice coverage. The Contractor shall be solely liable for and expressly agrees to indemnify the Government for the costs of defense and any liability resulting from services provided to SMs by its providers.
