INSTRUCTION FOR COMPLETING THE AFMC FORM 807
Quality Assurance provisions must include all inspections (by reference, where appropriate) to be performed to ensure that the item offered conforms to the technical requirements. The technical authority is responsible for providing contract quality requirements for all buys lAW FAR Part 46. Quality provisions as annotated on the AFMC Form 807 will not be downgraded without coordination from the technical authority.
NOTE: The technical authority is the chief engineer of the responsible system or lead engineer of the end-item. Technical authority may be delegated by the chief engineer of the responsible system or lead engineer of the end-item to those having technical competence and capability to perform the required activities. The end-item is defined as equipment that can be used by itself to perform a military function (AFI 63-1201).
The technical authority will complete the AFMC Form 807 as follows: Blocks not identified are self·explanatory.
(Block 3): Point of Inspection - Must be source or destination, where inspection means examining and testing supplies or services by a government representative to determine whether they conform to contract requirements. This will be the location of the inspection or the location of potential inspection by the government as the government always retains the rights to inspect even if you make the contractor responsible for inspection (i.e.
applying FAR 52.246·1). Note: Per DFARS 246.402, on contracts less than 250K, the following three requirements must be met before source inspection can be considered. (1) Significant product technical requirements (e.g., drawings or performance requirements) are in the contract. (2) Critical characteristics or acquisition issues are identified in either a quality assurance letter of instruction (QAU) which describes how to identify or determine if these conditions have been met or they are described in the technical requirements portion of the contract (3) Contract must be for a manufacture or producer and have critical characteristics identified and specific Government verification identified as necessary and feasible to perform at these non manufactures or non producers. If the contract is less than $250K, and all three criteria have not been met, there are no waiver procedures as no waivers are allowed. The only exception is when source inspection is required by 000 regulation or required by Memorandum of Agreement between the acquiring dept or agency and the contract administration agency.
(Block 4): Point of Acceptance - Must be either source or destination. Cannot be both. This is the point of government acceptance, where acceptance means act of an authorized representative of the government by which either the government, or itself or as agent of another, assumes ownership of existing identified supplies tendered or approves specific services rendered as partial or complete performance of contract.
(Block 7): Contract Quality RequiremenVSpecial Inspection Requirements.
Select One: (Note: The appropriate requirement should be selected based on technical description, complexity, application and FAR Part 12, paragraph 12.301, and FAR Part 46, paragraph 46, 402, and 46.403).
CERTIFICATE OF CONFORMANCE FAR 52.246-15: Allows the contractor to ship materiel without or in lieu of Government source inspection. Requires authorization by cognizant contract administration office in writing to ship any supplies for which the contract would otherwise require inspection at the source.
CONTRACTOR RESPONSIBILITY FOR INSPECTION, FAR 52.246-1: Should be used on all small purchase contracts where the contracting officer has determined that the Government does not have a need to inspect the supplies or services in advance of their tender for acceptance. Under this clause we still have a recourse with the contractor if nonconforming materiel is delivered and the Government always retains the rights to inspect, even i~there is no initial desire or identified need.
STANDARD INSPECTION FAR 52.246-2, -3, 04, -5 OR -6 AS APPLICABLE TO CONTRACT: Used when the purchasing office determines there is a need for government inspection of supplies or services. Can be used either in conjunction with higher-level contract quality requirements or by themselves. Should be used when a Commercial Technical Description is used for a complex or critical item or when a Military-Federal Technical Description is used for a noncomplex or noncritical item.
HIGHER LEVEL CONTRACT QUALITY REQUIREMENTS, FAR 52·246·11: NATO AQAP 2070, AS9100, ISO 9001:2000 (2000 version or equivalent) Quality Systems - ISO 9001:2000 specifies requirements for a quality management system for any organization that needs to demonstrate its ability to consistenly provide product that meets customer and applicable regulatory requirements and aims to enhance customer satisfaction. The 2000 version of ISO 9001 incorporates the1994 versions of ISO 90001, 9002 and 9003 and the 1995 versions of NATO ACAP 100, 120 and 130. However, the 1994/1995 versions are no longer valid as they expired on 15 Dee 2003. AS9100 is the quality standard for the aerospace industry. NATO AQAP 2070 is the Allied Quality Assurance Publication for use with NATO participants.
COMMERCIAL ITEM INSPECTION, FAR Part 52.212·4: Should be used when items meet description of a commercial item as defined in FAR Part 12.
OTHER: Provide details in block 8. Such as when a higher level of contract quality requirements is desired, but when these higher requirements do not include ISO or equivalent requirements. A Quality Assurance Letter of Instruction (QAU) or specific requirements to be placed in the contract must be identified in block 8 or attached. These are developed by the technical authority and describes critical characteristics or performance requirements and provides guidance on how to identify or address these issues.
(Block 8): Identify if the item is a critical safety item (CSI). If the item's criticality code is "E", F", or ·S". If item is CSI, a QAlI must be generated for the item.
(Block 8a): For this block (Ozone Depleting Substance (ODS», the engineering entity signing the IMT 807 must mark one of the the two blocks.
Technical entity must decide if an ODS Statement is required or if an ODS Waiver is required. If an ODS Waiver is required, please refer to AF FARS 5323 for appropriate action. If no waiver is required, the engineer must mark the ODS Statement.
(Block 9): Quality Pre-Award Survey Recommended - FAR Part 9, paragraph 9.104-1 requires a contracting officer to determine that a contractor is responsible before awarding a contract. A preaward survey may be requested and used whenever there is not sufficient information to determine responsibility.

• AFMC Participation Recommended. May be recommended if AFMC participation on the preaward survey would be beneficial.
