(Insert your company’s name) is currently participating as an offeror in response to the Government’s Request for Proposal Number FA5000-15-R-0001 for the Civil Engineer Support Services Firm-Fixed contract at Joint Base Elmendorf/Richardson, Alaska.
The government will be selecting contractors for this project based on selection factors. One of the selection factors to be rated is contractor past performance. It shall be a key factor in the government’s final decision. To assist the government in its assessment of contractor past performance, we are required to identify previous projects and clients who are familiar with our past work.
As one of our prior/current customers for past contracting work on (name project here), we are requesting that you complete the attached Past Performance Questionnaire regarding our past performance. This letter also authorizes you to discuss our past performance with the government and to release information regarding our past performance to the government.
Your prompt attention in completing the attached Past Performance Questionnaire will assure we receive full consideration for the contracts to be awarded. Please return the completed form to the government representative POC identified below, to arrive no later than the proposal due date.
