3rd Generation Multiple Award Construction Contract (MACC) 8(a)
1. The 28th Contracting Squadron is in the process of selecting contractors for the Multiple Award Construction Contract. In our proposal instructions to offerors we requested they provide a brief prospectus of all contracts over the past three years that are similar in size and scope of work as the sample project contained in the solicitation as well as point of contact (POC) information (e.g., contracting agency, phone number, name of primary person responsible for the contract, address, e-mail address, and fax number) as one of our award criteria includes the evaluation of the offeror's past performance on prior contracts and contracts currently in progress. It is for that purpose that your comments are solicited on the attached questionnaire. Please have the questionnaire completed by the person(s) most familiar with the contractor’s performance. A simple “N/A” answer may be appropriate when no evidence is available to you in a particular area. Indicate, based on the definitions provided in the questionnaire, the contractor’s performance on the identified contract. Please provide narrative rationales for your answers that are marked less than "satisfactory" or areas that the contractor clearly exceeded contract requirements, e.g., "exceptional." Hand written responses, printed clearly, are sufficient.
2. Please submit your past performance questionnaire directly to the 28th Contracting Squadron (not to the company) either by fax with a cover sheet marked SOURCE SELECTION SENSITIVE INFORMATION to commercial number 605-385-1734, DSN 675-1759, or via e-mail (which is preferred) to david.goff@ellsworth.af.mil as soon as possible but no later than 2 Feb 11, 4:30 p.m. Mountain Standard Time*.

Questionnaire on Contractor’s Present and Past Performance
Request you provide the following information on the contractor’s present and past performance by placing an X in the space provided by the most appropriate response. If you wish to elaborate on your answer, please provide comments on the lines provided after each question. If more space is needed, continue your comments on a separate sheet of paper and attach it to this questionnaire prior to submittal to the 28th Comptroller and Contracting Squadron. You are asked to provide frank, concise comments regarding your overall assessment of the contractor’s performance on the contract identified. You are urged to supplement your own knowledge of the contractor’s performance with the judgement of others in your organization.
