SAMPLE PROJECT
(While no delivery order is being awarded under this Sample Project scenario, the following represents one option which might be utilized to execute MACC projects. The intent is to show prospective contractors how the process will be implemented and allow the Government an opportunity to evaluate the proposer’s abilities related to understanding and responding to specific project requirements.)
GENERAL INFORMATION AND PROJECT REQUIREMENTS (GIPR)
Construct Storage and Restrooms – Facility 5025 Project # FXBM 01-1000
1. Project Title: Construct Storage and Restrooms – Facility 5025
2. Project Number: FXBM: 01-1000
3. Award Method: RFP, Technically Acceptable “Best Value” (Example 2 per paragraph 2.b of the General Provisions.) Only those proposals which include all the required information and meet the definition of a 95% design as described in paragraph 1.c. of the General Provisions will be determined to be technically acceptable. Proposals not meeting these standards will be considered non-responsive and will not be eligible for further evaluation or award. For the purpose of proposal evaluation and selection of the successful offeror, equal weight will be given to pricing and the number of points attainable as documented on the LEED 2009 New Construction and Major Renovation project checklist. Back-up information must be included, with the proposal, which supports the anticipated points assigned to those items included in the design and resulting in the points referenced on the checklist. It is not the government’s intent to attain a “certified” or higher rating, but rather to reasonably incorporate “green technology” to the extent possible without adding significant costs to the project. (For the purposes of this Sample Project, a proposal which does not include at least two, points-producing items and back-up documentation, will be considered non-responsive.)

7. Construction performance period shall be 180 days from the date the Notice to Proceed for construction is issued.
8. The contractor will be provided with a storage area. Temporary fencing and/or storage trailer will be the responsibility of the contractor. The contractor will be responsible for restoration of the storage area to pre-construction status.
9. Work hours for this project shall be the standard hours as specified in the General Provisions of the MACC Contract.
10. The standard construction warranty as specified in the General Provisions of the MACC Contract will be required.
11. Design submittals required; 95%
12. No restrooms are available for the contractor’s use.
13. A digging permit is required
14. The following is a minimum listing of drawings to be provided by the contractor. Drawings shall be standard D size drawings (24” x 36”).
A. Cover Sheet B. Existing / Final Site & Grading Plan(s) C. Underground Utility Plan * D. Floor Plan * E. Structural Plans and Details * F. Architectural Elevations, Wall Sections, and Details G. Door Schedule and Room Finish Schedule H. Mechanical Plans, Schedules, and Details * I. Plumbing Riser Diagram J. Power Plan and Lighting Plan * K. Fire Protection Plan *
15. As a minimum, the Contractor shall provide the following specification sections; items A & B per the General Provisions, and the remainder in the Master Spec short form:

(* - Drawings and specifications sections for which a letter of certification must be provided as part of the Sample Project 95% submittal package. The letter will state that the corresponding drawing(s) and specifications section(s) were prepared by, or under the supervision of, a person licensed as a professional in that area of expertise in the State of South Dakota. The name of the design professional and their license number shall be included. Any disclaimers or restrictions relating to the use of the documents should also be included in the letter. “Professional Certification” of the plans and specifications, as described in the Definitions section of the General Provisions, is not being requested nor required.)
16. Contractor shall provide a Submittal Schedule with the following submittals to be included, as a minimum. All of the listed items will require Designer of Record (DOR) review.
A. Concrete mix design ** B. Masonry units and mortar mix ** C. Bar Joists D. Roof Decking E. Unit heaters and baseboard heaters ** F. Electrical panel **
(** - Submittal packages for these items are to be included in the contractor’s proposal to assist in the Sample Project evaluation process. Projects executed under the MACC would not typically include a requirement for submittals prior to completion of the 100% design.)
17. A color board for interior finishes is required.

20. Tests Required;
| A. One set of concrete cylinders for each day’s concrete placement |
| B. Bacteria test for waterline |
21. Design calculations for roof joists selection, heating / ventilation loads, and electrical loads shall be submitted with the 95% design submittal
