General BMPs *to be reviewed by all
_____ * All equipment and vehicles must be maintained on paved/gravel surfaces or in the designated work area.
_____ * All materials, vehicle parking, and staging areas shall be designated by CEIE and located at least 25 feet away from drainages and other wetlands. Storage of all construction material/debris shall be kept to the designated storage/staging area. Excess soils temporarily stored on-site during construction, and stockpiles, must be covered with stabilization blankets/tarp and wattles to prevent exposure to the elements and to lessen chances of sedimentation due to storm water runoff and wind erosion.
_____ * All vegetated areas disturbed by construction shall be re-vegetated with a CEIE approved seed and certified weed free straw mulch upon completion. All landscaped areas disturbed by construction shall be restored to their original condition or better, e.g., sod, trees, shrubs and mulch replaced along with any irrigation systems. When the soil is re-vegetated the contractor/shop will remove the erosion control systems. All disturbed areas will be backfilled and returned to normal grade.
_____ * 9 CES/CEIE will be notified in the case of any spills of any size in the project area. 9 CES/CEIE SPILL CELL: 530-301-9413.
_____ * Any worker who inadvertently kills or injures any animal, especially a special status species, or finds one injured or trapped, will immediately report the incident to the CEIE biological monitor at 530-634-2835.
_____ * All open trenches/holes shall be covered with plywood or adequate cover before leaving the worksite to prevent wildlife from falling in and becoming trapped or personnel from becoming injured.
Air Quality
_____ *Air Quality (FRAQMD compliance): The contractor/shop shall comply with the Feather River Air Quality Management District (FRAQMD) Standard
Mitigation Measures for All Projects and Fugitive Dust Control Mitigation Measures during construction to minimize construction related air quality impacts.

_____ *Air Quality (Title 13 CCR): In accordance with Title 13 CCR, Section 2485, all diesel-fueled commercial vehicles operating in the State of California with a gross vehicular weight of greater than 10,000 lbs. and are or must be licensed to operate on highways shall not be idled for greater than five (5) minutes at any location. See Title 13 CCR, Section 2485, Subsection (d) for exceptions.

Natural Resources
_____ * Burrowing owls may be in the project area. No disturbance should occur within 50 m (approx. 160 ft.) of occupied burrows during the non-breeding season of September 1 through January 31 or within 75 m (approx. 250 ft.) during the breeding season of February 1 through August 31.
______ * Hawks may be nesting in the project area. No disturbance should occur within 1/2 mile of an active nest during the breeding season of February 1 through
August 31, unless approved by 9 CES/CEIE.
______ * Nesting birds protected under the Migratory Bird Treaty Act may be present within the construction area. Birds, nests, and eggs are protected by law from
15 February - 31 August (nesting season). A pre-construction site survey must be performed by a qualified biologist at least 30 days before construction to determine whether any protected species are present on or near the site. If protected birds are present and nesting on or near the site, construction may be temporarily postponed until the nesting season is over. Contact CEIE at least 30 days in advance to arrange the pre-construction site survey. Other measures which may be necessary if protected species are found on or near the site during the pre-construction survey include: (1) the contractor may be prohibited from disturbing areas within a specified distance of owl burrows or bird nests; (2) the contractor may be required to shut down or restrict activities during breeding and nesting seasons; (3) construction may be temporarily delayed while birds are encouraged to relocate away from the construction area.
______ * Raptor Protection: See attached notes on ways to make utility equipment safe for animals. New installation of equipment is required to have protection measures installed for safety.
______ * All trees scheduled for preservation shall be temporarily fenced during construction. Fencing shall be installed prior to the commencement of construction and shall remain in place until all construction is completed. As a minimum, fencing shall be located at the edge of the root zone. The root zone is determined to be the entire ground surface located underneath the tree canopy, commonly referred to as the drip line of the tree. Fencing shall consist of chain link or plastic link fence.
Fencing shall be rigidly supported and maintained during all construction periods at a minimum height of 4' - 0", above grade. Fenced areas shall not be used for material stockpile, storage or vehicle parking. Dumping of building materials, chemicals, concrete products or garbage shall be prohibited within the fenced area. Do not attach anything to the tree’s limbs or trunk, or cause injury to the bark. Do not irrigate within the root zone of native oak trees. Fenced areas under trees shall be maintained in a natural condition and soils shall not be disturbed or compacted. All trees required to be fenced shall be clearly marked with a spot of yellow paint.
The marking will notify inspectors that the subject tree or tree(s) are to be fenced at all times during construction.
______ * Vernal pools are nearby the project location. Vernal pools are protected as endangered species habitat under the Endangered Species Act (ESA) and as
Jurisdictional Waters of the US under the Clean Water Act (CWA). No construction activities or disposal of materials may occur within vernal pools and all ground disturbing activities within 250 feet of a vernal pool require 9 CES/CEIE approval.

______ * Cultural Resources: If any cultural remains or artifacts are inadvertently discovered, stop all ground disturbing work at the point of discovery, plus a reasonable buffer exclusionary area and notify the installation’s Environmental Office at 634-2593/2645. Hazardous Materials
______ *Hazmat (contractor): Air Force regulation (AFI 32-7086 § 2.5.5) requires that all hazardous materials obtain prior authorization through the Hazardous
Materials Management Process (HMMP) before the material is brought on base. Usage data of approved materials must be reported for tracking in the Enterprise
Environmental, Safety, and Occupational Health - Management Information System (EESOH-MIS). For any work requiring the use of materials that have an
MSDS (hazmats), contractors should have submitted a Contractor Hazmat Worksheets package and received HMMP approval. If this has not been accomplished, submit the package (attached) to the contract QA/representative for HMMP review. Package must receive HMMP approval prior to start of work. Hazardous Waste
______ *HazWaste: Hazardous Wastes must be disposed properly in accordance with local, state, and federal regulations. The contractor or generating organization must submit hazwaste profiles to include MSDS's and/or sampling results to 9 CES/CEIE at least two days prior to shipping hazardous wastes from Beale AFB.
Hazardous Waste shipments and all shipping documents (manifests, BOL's, LDR's, etc.) will be inspected and signed by 9 CES/CEIE before leaving base. The contractor or generating organization will complete manifests with the following information: EPA ID# CA7570024508; Beale 24 hr emergency #530-634-8675;
generator mailing address- 9 CES/CEIE, 6601 B St., Beale AFB 95903; POC- 530-634-2644 Eric Maresh. Alternate POC phone # 634-4452.

MMRP
On/near Military Munitions Response Program (MMRP) Site(s) ________________________________
______ *Military Munitions Advisory: Past activities at Beale used a variety of munitions and related materials across most of the base property. Because of the base’s history, military munitions related material could be encountered within the project area. If any suspected military munitions related material is found, stop work in the area, move personnel away from the site, and contact the Beale Explosive Ordnance Disposal (EOD) (634-9070/634-9071 or 911 for emergency). Do not touch or attempt to remove any material suspected to be military munitions related.
______ *A Comprehensive Site Evaluation (CSE) was conducted by the U. S. Army Corps of Engineers (USACE). The CSE supports the Military Munitions
Response Program (MMRP). The Report includes evaluations of Munitions Response Sites (MRS) for hazards. The Final Report is available for review.
______ *The proposed intrusive work area shown on the figure attached to the Dig Permit is in an area that has already been disturbed. So long as the intrusive activities are confined to the area already disturbed, the risk of encountering munitions and explosives of concern (MEC) should be low.
______ *Excess digging should be discouraged to minimize the risk of encountering MEC. If MEC is encountered, the Contractor should stop work in the area, move personnel away from the site and contact the Beale Explosive Ordnance Disposal (EOD) flight by calling 911. The Contractor should not touch or attempt to remove any material suspected to be military munitions related. If MEC is discovered a UXO technician support will be required before the project can continue.

______ *P2/QRP/Solid Waste (Debris Diversion Report): Contractor should report the construction and demolition debris disposal data each quarter during the project. The data report should include the tonnage of each type of material disposed of in a landfill and the tonnage of each type of material diverted for recycling.

______ *P2/QRP/Solid Waste (scrap metal): All scrap metal produced from construction and demolition projects on base is recycled as part of the base Qualified

______ *P2/QRP/Solid Waste (Sorted Demo Debris): Ensure sorted demo debris does not remain on site for more than 1 year from date of project completion.

Storm Water
______ * Ground disturbing work on unpaved areas is typically not allowed during the Limited Operations Period (November 1 through April 30). Permission to work during the Limited Operations Period is being granted for this project but is dependent on prevailing conditions and forecasted weather. Contact 9 CES/
CEIE (4-2645) to obtain permission to begin work. Once work has begun, the following restrictions apply: The only work that is allowed 12 hours before or after a storm event is the inspection, installation and/or maintenance of erosion controls/best management practices. If conditions arise that are unfavorable, CEIE may further restrict activities. Contact 9 CES/CEIE for authorization to resume normal activities (530-634-2645).
______ * Ground disturbing work is authorized for this project between 1 May - 31 October. No ground disturbing work is permitted for 12 hours before or after a storm event with the exception of the inspection, installation and/or maintenance of erosion controls/best management practices. Should the project require ground disturbing work to occur during the base wet season (1 November - 30 April), contact 9 CES/CEIE for authorization.
______ * Ground disturbing work on unpaved areas is not allowed during the wet season (usually November through April). Permission to work outdoors during the Limited Operations Period (November 1 through April 30)may be requested from 9 CES/CEIE if weather continues to be fair but is dependent on prevailing conditions and forecasted weather. Contact 9 CES/ CEIE (4-2835) to obtain permission to begin work. 9 CES/CEIE will evaluate and approve or disapprove outdoor work requests during this time on a case-by-case basis, depending on weather conditions, weather forecast, and whether or not activities will damage soil or vegetative cover. Once work has begun, the following restriction apply: The only outdoor work allowed for 12 hours before or after a storm event is the inspection, installation and/or maintenance of erosion controls/best management practices.
______ * Attached is a map of the site drain inlets, drainage ditches, and wetlands in the project work area. Orange barrier fences or pink flags will designate exclusion zones where construction activities cannot take place. Contractor will install orange fence to keep equipment out of wetland areas. Drain inlets within the project area will be protected from sediment laden water.
______ * The contractor/shop is responsible for implementing best management practices to ensure pollutants are not released or discharged to the drainage ditches, storm drains, seasonal wetlands or other water bodies in the project area. Proper erosion and sediment control measures will be installed. The contractor/shop shall install and maintain erosion control systems such as gravel/sand bags, silt fence, straw bale barriers, erosion control/stabilization blankets, straw wattles, or etc. as needed to protect drainage ditches, storm drains, seasonal wetlands and water bodies from sedimentation resulting from construction activity.
______ * Work disturbing more than 1 acre requires a NPDES construction permit. NPDES permits require that a Notice of Intent (NOI) be submitted to the State of California with a Storm Water Pollution Prevention Plan (SWPPP) developed by a certified Qualified SWPPP Developer (QSD). After September 2, 2011 all
SWPPPs must be implemented in the field by a certified Qualified SWPPP Practitioner (QSP). The contractor must comply with all aspects of the State issued permit and general permit guidelines. For more information reference http://www.swrcb.ca.gov/
______ * Concrete/paint/substance washout must be contained and disposed of properly. No water from the rinsing of equipment shall be discharged to the ground surface or drainages. Any type of rinse or wash water discharged to Beale’s storm water drainage system is considered an unauthorized non-storm water discharge and is strictly prohibited by the industrial storm water general permit.
______ * Contractor must coordinate with CEIE staff for access routes/staging areas prior to beginning work. Call 530-634-2835 to arrange.
______ * Above ground work only is authorized. No ground disturbance is allowed for this project. Any ground disturbance created by equipment access must be repaired and BMP's properly installed to prevent sediment movement off site.

EPA NESHAP, OSHA & CDPH: Asbestos/Lead Based Paint
______ *HAZMAT Disturbance Risk: Project work could disturb hazardous materials such as asbestos or lead based paint, if they are present in the work area.
Attached to the 813, 1391, 332, the “Hazardous Materials Survey for Asbestos, Lead Based Paint, PCBs, Mold, Mercury and Recyclable Radioactive Sources” provides guidance for completing a hazardous material survey during the project design phase. The contract must include the requirement for a hazardous material survey, performed by a state certified lead/asbestos and hazardous materials consultant, as a contractor responsibility. This will ensure that hazardous materials, if present, will be identified and properly managed by the contractor.
______ * Health & Safety Work Plan: The contractor must submit a Health and Safety Plan and Abatement Work Plan that includes worker health and safety practices for the abatement activities. The Health and Safety Plan must include valid certificates for each worker that will be involved in the abatement showing they have completed required AHERA and OSHA training requirements. A copy must be provided to the 9 CES/CEIE Toxics Program Manager.
______ * Beale Asbestos M&OP/LBP Plan Compliance: The contractor must comply with Beale AFB Asbestos Management and Operating Plan, Lead Based
Paint Management and Operations Plan, and applicable AHERA, EPA, NESHAP and OSHA laws and regulations when working in areas with asbestos or lead hazards. Contact 9 CES/CEIE at 530-634-2644 for additional information if needed.
______ * Warning Signs Required: The contractor must post warning signs at the entrance of asbestos or lead abatement areas warning of the hazard.
______ * Demolition Written Notification: Written notification of demolition or renovation operations is required. The notification is required for demolition even if there is no asbestos present. Only complete notification forms are acceptable, and incomplete notification can result in enforcement action. The contractor must comply with the NESHAP regulation, and provide copies of all notification forms to 9 CES/CEIE prior to sending them to the regulatory agencies.
______ * Asbestos Abatement Notification: Beale AFB requires notification at least ten days prior to any asbestos abatement project. The Beale AFB notification must go to 9 MDOS/SGOAB, AFGE Local 2025 Safety Officer, and 9 CES/CEIE Toxics Program Manager, 530-634-2644. Notify the Feather River Air Quality
Management District (FRAQMD) five days prior to any demolition, and at least ten working days prior to any project that removes or disturbs regulated asbestos containing materials in excess of specified quantities. Notify Cal-OSHA at least 24 hours prior to removing regulated asbestos containing materials. The contractor must complete all required notifications.

Title: Replace Wheatland Bridges, #221 and #222 Log Number: 11700 Government

Location: Bridges #221 and #222 - located on S. Beale Rd between the Wheatland Gate and Wastewater Treatment Plant.
Description:
***Urgent - Work scheduled to begin 5 June 2017*** The purpose of this project is to remove the existing dilapidated bridges, #221 and #222, and replace with new concrete bridges so the Wheatland gate and the Commercial Vehicle Inspection Area can reopen. Work required for the completion of this project includes but is not limited to; demolition of existing timber bridges and associated structures/components;
installation of steel piles, concrete abutments/wing walls, precast concrete girders, pavement, guardrail, and signs; and the removal of approximately 5 trees. Some earthwork and grading will be required.
Additionally, a crane will be needed for the construction of the new replacement bridges. It should be noted that due to the load restrictions on the bridges, some material and equipment will need to enter through the Wheatland gate. ***We are requesting that the review/approval process be expedited to allow work to begin Monday, 5 June 2017.***
