A/C MAINT Aircraft Maintenance
Includes all aspects of aircraft maintenance (e.g., corrosion control, machine, AGE, NDI, repair and reclamation, structural repair, off and on equipment etc.) not previously addressed.
Supplier Will:
1. Support owner of real property similar equipment, as defined in applicable regulations, with field or intermediate-level maintenance when the logistics community cannot support the equipment and the maintenance is existing skill within civil engineering. This includes 150 LFs and 15 MAFs.
Receiver Will:
1. Identify requirements identify off base priority.
2. The 54 HF request supplier provide the following "on-equipment" maintenance services for assigned
UH-IN helicopters:
a. Limited hydraulic support consisting of manufacturing hoses/lines/tubing as required.
2. Identify requirements. Coordinate and provide equipment-requiring NDI
3. Supplier maintenance personnel should be qualified to perform all tasks of their respective specialties as it pertains to UH-1N aircraft.
3. Identify requirements.
4. 5 MXS/AGE will provide maintenance and inspections as required on four H1 Heaters, one -86 Hobart
Generator, one Hydraulic Test Stand, one Wash Cart, and one 600 gallon Fuel Bowser assigned to the
54th Helicopter Flight.

5. Provide interim/short term support for receiver, as requested, whenever the receiver's organic capability is limited.

6. 5 MXS/AGE will provide maintenance and inspections as required on 17 ea H-1 Heaters and two MD-
2 Motor Generators assigned to the 91st Maintenance Squadron and RIVET MILE.
6. Process all required maintenance through CAMS and obtain a work order prior to delivering parts for repair. All pertinent information will be filled out on an AFTO form 350, Repairable Item Processing
Tag.
7. 5 MXS/FAB will provide machining, sheet metal, and non-destructive inspection services in support of on/off equipment maintenance beyond the capability of the receiver unit support shops and personnel. All requested maintenance must be in compliance with current technical orders and directives.
7. Complete all necessary coordination when requesting a local manufacturer IAW MAFBI 21-106.
Receiver is responsible for obtaining all drawings, blueprints, or other specific data.
8. Request depot/engineer guidance for any repairs or manufactures that are not covered by current technical data.
9. Route any priority request through the Maintenance Operations Center.
ADMIN Administrative Services
Includes records management, personnel locator, document control and handling, forms and publications, and maintenance of official publications reference libraries.
1. Provide administrative communications/official mail distribution service through the Base Information
Transfer System (BITS) IAW DoD 4525.8-M/AF Sup.
1. Operates a distribution office to receive and dispatch official communication through the BITS.
Coordinate with Supplier on address symbols, location, and other changes thereto. Maintain central control of all mail and classified documents, including classified destruction. Comply with the Supplier's established policies and procedures. Prepare all documents and packages IAW DOD 4525.8-M/AF Sup.
2. Provide technical assistance and staging area support. Provide records management orientation.
2. Manage unit records under supervision of parent activity.
3. Provide assistance and training on obtaining electronic departmental, field, and installation publications. Provide electronic copies of departmental field and installation publications and forms if means to obtain them are not available.

3. Comply with applicable directives.
4. Supply periodicals for 91 OSS/OSKE Intelligence Library and MAFs.
4. Provide information to the base locator as required by the supplier.
5. Maintain receiver's required publication in the Base Master Publications Library.

6. Process all Freedom of Information Act (FOIA) and those Privacy Act (PA) requests denying access for Space Wing (AFSPC) records by assigning an AFSPC identifier case number to the requests.
Maintain files IAW applicable regulations. Collect and deposit applicable fees. Collect and maintain reportable data and prepare annual reports for submission to AFSPC for consolidation. Forwards all recommended denials through 91 SW to HQ AFSPC for final determination.

Includes data processing services and systems analysis. Also, includes design, development, execution, and life cycle maintenance of data processing systems.

1. Assign a Work Group Manager to provide hardware/software installation, troubleshooting, and minor system administration.
2. Provide technical assistance and network management for all small computer systems on Automated
Data Processing Equipment (ADPE) inventory.
2. Identify requirements and comply with supplier's directives and policies.
3. Process Communications-Computer System Requirement Documents (CSRD).
3. Provide POCs for CSRDs. Provide for computer systems requirements upon approval of unit commander, fund purchases, and forward all CSRD requirements to 5 CS.

AIRFLD OPS Airfield Base Operations
Manage airfield support activities and facilities and provide service for preflight planning and flight plan processing.
1. Manage US Air Force airfield support activities. Coordinate with the FAA and other applicable agencies in developing local procedures for air and ground control of all aircraft traffic. Be responsible for other functions prescribed by applicable directives except airspace management.
1. Comply with established procedures and hours of operation. Advise supplier of any unusual requirements and establish required operational procedures with base operations for clearances and ground control.
2. Maintain 91 SW helicopter airfield waiver for contingency conditions.
2. Comply with supplier guidance.
AMMO Ammunition
Provide for the supply, maintenance, and repair of devices charged with explosives, propellants, pyrotechnics, initiating composition of nuclear, biological, or chemical material for use in connection with defense or offense including demolitions, ceremonial, or non-operational purposes.

1. Perform Munitions Accountable Systems Officer (MASO) duties.
1. Comply with supplier directives.
2. Order, store, inspect, issue and maintain all ammunition, explosives, and grenades for receiver Security
Forces, and Maintenance Group.
2. Provide TDY funds when available to 5 MUNS personnel for HQ AFSPC sponsored programs and training.
3. Provide repair, maintenance, security, and storage for the MK-12A Reentry Systems (RS), Reentry
Vehicles (RV), and their associated components while on base.

4. Provide maintenance and repair support for RSs in the field when requested by 91 SW/MXG and authorized by technical data.

BASE PLANS Base Plans
Develops and monitors the application of base-level operations and contingency plans not specifically assigned by directive to other functional areas. Provides personnel for on-base emergency contingencies where the Receiver does not have a capability.
1. Plans: Develop and monitor the application of base-level operations and contingency plans not specifically assigned by directive to other functional areas.

CHAPEL Chapel and Chaplain Services
Includes pastoral ministries, worship services, religious rites, pastoral visits, spiritual counseling, and religious education.
1. Provide comprehensive pastoral ministry including opportunities for worship, religious rites, and religious education. Also provide adequate staff to support pastoral care, spiritual counseling and visitation over an 8500 square-mile missile complex.
1. Coordinate chaplain activities and support requirements and comply with supplier's directives and policies.
CIV PERS Civilian Personnel Services
Includes recruitment, classification, staffing, pay administration, personnel management, employee relations, awards, equal opportunity programs, and career development.
1. Provide full civilian personnel administration services for all functions related to classification, data management, affirmative employment, employee management relations, labor relations, employee career development and training and resource management in accordance with all governing laws, rules, and regulations.
1. Advise supplier of civilian personnel requirements and services and submit necessary forms and paperwork normally associated with requesting such services.
2. Provide formal and informal MEO counseling to civilian personnel.
2. Coordinate with civilian personnel to schedule MEO counseling as required.
CLUBS Clubs
Includes collocated club operations (Jimmy Doolittle Center and J.R Rockers), community and other recreational clubs.
1. Provide access to collocated club operations, community and other membership activities. Also, includes golf course, bowling center, and related services.

(a) Assigned personnel will pay any required dues at the prevailing rates as established by the supplier base.
CMD POST Consolidated Command Post
Provide command and control facilities and services for receiver units. Supplier is responsible for command post coordination of operations.
1. Provide command and control facilities and services for receiver unit. Submit voice and record copy operational reports for receiver according to Air Force Instructions. Provide voice advisory of operational reports to receiver higher headquarters and include receiver higher headquarters addresses on record copy as provide by receiver. Provide weather advisories to receiver for the supplier area, and alert receiver of severe weather conditions, to include delayed reporting, early release and base closure as specified by receiver. Notify receiver of FPCON implemented by supplier.
1. Support supplier in discharging command and control responsibilities, including consolidation activities. Provide supplier with information for submission of operational reports, and provide supplier with higher headquarters DSN numbers and addresses for record copy reporting. Provide supplier with current recall rosters for notifications and alerting. Provide supplier with sever weather notification requirements. Notify supplier of FPCON implemented by receiver. Coordinate with supplier to deconflict call signs.
2. Provide command and control (C2) for receiver assists. This includes but is not limited to, Emergency
War Orders (EWO), CJCS, USSTRATCOM, AFSPC, and emergency action messages affecting the receiver. Make notifications to wing CC/CV, and group CCs, staff agencies, and the squadron command posts, as required. Monitor and operate equipment installed in the Minot Command Post in support of
AFSPC requirements. Provide the required reporting, including SORTS and OPREP-3, per appropriate
CJCS, USSTRATCOM, USAF, and AFSPC regulations for the receiver.
2. Maintain and provide Supplier with current points of contact for receiver activities requiring notifications. Receiver will ensure that 91 SW/CC, CV, and group commanders notify supplier of contact names and numbers and when not available, alternate contact names and numbers.
3. Notify 91 SW/CC, group and/or squadron commanders of incidents involving AFSPC personnel.
3. Coordinate with 5 BW on incidents involving AFSPC personnel.
4. Provide specialized EWO, codes, plans and other training to assigned controllers to support receivers' requirements.
4. Provide, as required, a Command Representative (COMREP) IAW AFI 10-207 to provide command-unique expertise in Receiver requirements and procedures.

5. Provide radio/telephone conference capability and Skyhook to receiver operations/aircraft and higher headquarters.
COMM Communication Services
Includes dedicated communications services and telephone equipment. May also include leasing of communication equipment, lines, and special communications-electronics equipment services.
1. Provide and maintain all common user/administrative telephone service with access to the local calling area and DSN when requested and justified.
1. Identify requirements and comply with supplier's directives and policies to include appointment of a primary and alternate Telephone Control Officer (TCO). The TCO will provide the supplier with an annual certification/inventory that all installed telephone equipment has been physically inventoried to include the type of set (i.e., single line, multi-line, or STU-III) with the five-digit telephone number assigned to each set.
2. Provide and maintain unconditioned on-base circuits.
2. Program, budget, and fund for the following:
(a) All official toll and directory assistance charges.
(b) Specialized/unique communications (I.e. multi-line phones, voice mail, business lines, and tariffed circuits).
(c) Provide direct fund cites for land mobile radios and cellular phones required by the receiver.
(d) Request and fund all telephone service orders that add, change, or disconnect service.
3. Process orders for tariffed circuits to local off-base locations.
3. Provide operational concept, in sufficient detail, to allow development of a satisfactory engineering design solution by the supplier.
4. Provide communications engineering support for DSN, FTS, etc.
4. Provide documentation to the supplier detailing interface equipment of all specialized/unique communications systems operated/maintained by the receiver.

5. Provide land mobile radio maintenance IAW contract for all nets to include helicopter operations radio net.
5. Comply with supplier directives and policies.
6. Communication Security:
A. Provide personnel to maintain the 91 OSS/OSKE COMSEC account.
B. Pick up required COMSEC from base account
C. Issue required COMSEC material.
6. Communication Security: Submit C4 requirements IAW AF instructions, and comply with procedures outlined in those instructions.
7. Provide training and guidance to unit Education, Training, and Awareness program (SATE) managers, and perform SATE inspections as required.
7. SATE: Appoint a unit SATE manager in writing to 5 CS/SCBS. Attend periodic training sessions/meetings, provide required initial/recurring SATE training, and comply with guidelines established in AFI 32-304.
8. Communication/Electronic Equipment: Provide or arrange for organic maintenance, contract maintenance or other support for authorized maintenance of base common user equipment, systems, facilities, and services. Provide a listing of personnel requiring access to ELAB to perform maintenance on communications equipment. Assist receiver in planning for installation of C4 equipment to safeguard against possible interference with existing operations or facilities. Provide or arrange for maintenance of communications equipment at LCCs, LCEBs, LFs, and missile procedures trainer. Systems maintained include: Strategic Automated Command and Control (SACCS), Military Strategic and Tactical Relay
(MILSTAR), ICBM SHF Satellite Terminal (ISST), Air Force Satellite Communications (AFSATCOM), Survivable Low Frequency Communication System (SLFCS), Higher Authority Communications/Rapid
Message Processor Equipment (HAC/RMPE), and the Meridian digital mini-switch for dial lines. This also includes the Support Information Network (SIN) and the hardened intersite cable system (HICS).
8. Communication/Electronic Equipment:
A. Provide maintenance and/or funding of receiver-specific communications equipment, systems facilities and services. In conjunction with supplier, plan any receiver proposed installation of C4 requirements. Program and obtain appropriate approval and funding. Budget and fund for land mobile radios and related maintenance.
B. Provide access to maintenance crews in order to perform maintenance on communications systems.
Allow the communications squadron to use the test stations in ELAB to check out communications equipment. Identify requirements and provide maintenance support as necessary.

COMMAND SPT Command Support
Includes oversight and management provided by the installation commander and the command element office staff.
1. Command: Command supplier personnel who support the receiver unit. Direct supplier personnel to provide receiver support as outlined by this agreement. Authorize receiver senior leadership direct liason authority with supplier organizations as long as supplier senior leadership remains fully informed of any special requests or identified shortfalls in supplier service.
1. Command: Command receiver personnel. Coordinate with the supplier in carrying out the receiver mission. Receiver units must not take any action that adversely affects the inherent overall authority and responsibility of the supplier or the Air Force image at the base, particularly in such areas as general military discipline, conduct and appearance. The receiver observes pertinent publications issued by the supplier to support the command responsibilities mentioned above.
2. Protocol: Provide receiver units with protocol guidance, as related to Air Force and ACC, AFSPC, regulations, directives, and policies.
2. Protocol: Provide protocol guidance on receiver unit matters, in coordination with supplier as related to Air Force and MAJCOM regulations, directives, and policies.

COMMON INFRA Common Use Facility Construction, Operations, Maintenance, and Repair
Includes construction, alteration, operation, maintenance, and repair of common use infrastructure (e.g., roads, grounds, surfaced areas, structures, real property and installed equipment). Also includes common benefit signs, energy consumption, snow removal, street cleaning, and installation beautification projects.
1. Furnish and provide for the operation, maintenance, repair and minor construction or alteration of common use infrastructure, roads, grounds (to include parks and athletic fields, surfaced areas, structures, real property, and installed equipment. Includes common benefit signs, energy consumption, and beautification projects).
1. Comply with Civil Engineering requirements for the operation and maintenance of common use facilities.

ENVIR COMPLY Environmental Compliance
Includes recycling and resource recovery programs, pollution prevention, environmental compliance, and programs aimed at management and control of hazardous materials -- does not include clean-up and disposal of hazardous materials.
1. Provide support to ensure receiver complies with all Air Force directives and policies; Federal and local environmental laws, regulations and standard; and executive orders dealing with environmental and natural resource protection and quality.
1. Provide personnel, materials and equipment needed (i.e., drums, labels, over-packs, etc.) to handle, store, and label and transport hazardous material/waste.
2. Provide training and guidance to personnel on Hazardous Waste Accumulation Point procedures, provide funds for Hazardous waste disposal.

5. Provide recycling of POL materials that meet the criteria set forth in the FGS for Used Oil for Energy
Recovery. Only solvent that can be recycled is PD680. All other solvent waste must be managed as
Hazardous Waste.
5. Initiate AF Form 813 and assist with the development of environmental studies and reviews when deemed necessary to support the receiver's proposed actions. Provide support to the Environmental
Impact Analysis Process when required.
6. Complete the Environmental Impact Analysis Process (EIAP) in accordance with the National
Environmental Policy Act (NEPA), governing host regulations, wing policy and other DOD regulations.
Any action a federal agency may take that has potential environmental impact must be analyzed before the decision is made to take such action. The proponent of such an action must initiate the EIAP. The proponent completing Section I of AF Form 813 usually does this. 5 CES/CEV manages the process and evaluates and completes the AF Form 813 with the assistance of the Environmental Planning Function
(EPF). Obtain technical data to support the EIAP and prepare the preliminary environmental survey and prepare or oversee the preparation of environmental documents. The proponent of the action is responsible for funding the survey required to provide for the baseline data information.
6. Comply with Air Force standards for elimination of OZONE Depletion Substances (ODS).
7. Monitor refrigerants usage and inspect equipment for leakage. Ensure acceptable substitute refrigerants are used as they become available.
7. Designate to the supplier a representative to serve on the installation EPC.
8. Include receiver as a member of the Installation Environmental Protection Committee (EPC).

FACIL CONSTR Facility Construction and Major Repair
Includes minor construction, alterations, additions, and major repairs required to modernize, replace, expand or restore real property. Also, includes related planning and design.
1. Provide planning, programming, design, and construction management for maintenance, repair and minor construction of receiver facilities and equipment. Budget and fund for all maintenance and repair projects, including minor construction projects costing less than $2,000.
1. Make requirements known through the submittal of AF Form 332, Base Civil Engineer Work Request.
Coordinate with supplier on development of requirements into project programming and design documents. Budget and fund cost of Minot construction projects costing $2,000 or more.
2. Maintain real property accountability and submit reports for receiver-funded construction, improvements, or modifications that, on completion, are located on the supplier base.
2. Participate in the yearly commanders' facility assessment program, identifying facility needs in categories Level I, (unsatisfactory); Level II, (degraded); and Level III, (satisfactory).
3. Advise the receiver that new facility requirements will be satisfied by:
(a) Re-assigning existing facility usage, or
(b) Including receiver facility deficiencies in military construction programs.
3. Appoint a primary and alternate real property building manager for each building or facility assigned.

4. Use existing facilities effectively and economically to meet real property requirements of all activities on an equitable basis.

FACIL REPAIR Facility Maintenance and Minor Repair
Includes routine and cyclical preventive maintenance and minor repairs required to preserve or restore real property so it may be used for its designated purpose.
1. Provide missile-engineering support for maintaining "as built drawing" and system upgrade drawings.
1. Request services.
2. Provide site topical surveys when problems arise during transporter erector leveling procedures.
2. Request services.
3. Perform space utilization studies/inventories periodically on all facilities.
3. Not change the use/occupancy of any facility prior to approval of base Facilities Utilization Board

5. Send AF Form 332 to supplier through the building manager for action after the receiver commander or a designated representative validates it.

FIRE Fire Protection
Includes all normal services related to fire protection and fighting operations, alert service, and rescue operations. Also, includes inspections for fire hazards, servicing of portable extinguishers, and related training programs.
1. Perform fire protection and suppression as required. Perform fire prevention inspections as needed and required. Provide fire safety education as required by Air Force Instructions (AFIs), AFOSH Standards and other applicable guidance.
1. Comply with base fire regulations, Air Force and DOD directives, and National Fire Codes. Conduct a fire prevention program for all employees within the guidance set by fire department personnel. Notify fire departments of location of all facility Material Safety Data Sheets (MSDS) so that pre-fire plans may be annotated.
2. Recharge, test, and perform routine maintenance on all Air Force Owned fire extinguishers.
