SUBJECT: Request for Present and Past Performance Information, Solicitation No. FA0021-15-R-0004

1. Headquarters Air Force Special Operations Command (HQ AFSOC) is in the process of selecting contractors for award of a Systems Engineering and Technical Assistance (SETA V) Service contract. The estimated total value of this acquisition is $45 million to all awardees over the life of the 5-year contract. Detailed information regarding this solicitation may be found at http://www.fedbizopps.gov by searching Reference No.: FA0021-15-R-0004.

2. One of the considerations in proposal evaluation is the verification of the offeror’s Present and Past Performance on contracts, which reflects the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror's performance on those contracts.

3. Our areas of interest in the offeror are summarized in the attached questionnaire, which we have requested be forwarded by the offeror to his customers. We need your written response by tbd. This schedule will allow us sufficient time to analyze the information prior to start of negotiations. It is important that you return the completed questionnaire directly to the Government.

4. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and scaned/emailed to, Attention: Kurt Buchanan at kurt.buchanan.1@us.af.mil . Your completed questionnaire will become a part of the official Source Selection records.

5. Your assistance is greatly appreciated, and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.

Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:

CODE PERFORMANCE LEVEL

B BLUE/EXCEPTIONAL - The contractor’s performance meets contractual requirements and exceeds many (requirements) to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

P PURPLE/VERY GOOD - The contractor’s performance meets contractual requirements and exceeds some (requirements) to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

G GREEN/SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Y YELLOW/MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.

R RED/UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
