1. The 720 th Special Tactics Group is in the process of selecting contractors for award of a Joint Terminal Attack Control (JTAC)/Small Unmanned Aerial Systems (SUAS) Program Management contract for the period of one (1) basic year plus four (4) one year options. Detailed information regarding this solicitation may be found at http://www.fedbizopps.gov by searching Reference No.: FA0021-12-R-0014.

2. One of the considerations in proposal evaluation is the verification of the offeror’s Present and Past Performance on contracts, which reflects the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror's performance on those contracts.

3. Our areas of interest in the offeror are summarized in the attached questionnaire, which we have requested be forwarded by the offeror to his customers. We need your written response by 14 Aug 2012. This schedule will allow us sufficient time to analyze the information prior to start of negotiations. It is important that you return the completed questionnaire directly to the Government.

4. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and faxed to (850) 884-2476, Attention: Jonathan Cansdale. Please call Jonathan Cansdale at (850) 884-5186 prior to transmission or if you have any questions. Your completed questionnaire will become a part of the official source selection records.

5. Your assistance is greatly appreciated, and your prompt response will be one of the keys to the successful and timely completion of this award process.

G GREEN/SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Y YELLOW/MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.

R RED/UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
