SUMMARY OF CHANGES
1. Incorporate Addendum 2 to change the APPENDICES under paragraph 5 of the PBWS:
The changes:
a) Replace Appendix A: Table A3 with the new Appendix A: Table A3, Attachment 1. The new table includes a descriptive note at the bottom of table.
Which states, “CHANGE: Note Added: This table addresses locations that always have 40
CY roll-offs on hand. An additional 5, 40 Cubic Yard roll-offs are required to be furnished by contractor. These Roll-offs fall under CLIN 0008 The description unscheduled items and special events (30-40-CY-roll offs) as a drop off and pick up service.”
b) Replace Appendix B: Table B1 with the new Appendix B: Table B1, Attachment 2. The new table has been revised to remove all Government-Furnished Equipment noted for Temporary
Lodging Facilities area from 49 to 0.
“CHANGE: GFE in TLFs reduced from 49 to 0. Government- Furnished Equipment (GFE) in the Temporary Lodging Facilities (TLFs) area: 0”
2. The revised bid sheet dated 19 June 16 failed to be incorporated in the last addendum so it was incorporated with this addendum.
3. It was brought to the governments attention that some questions asked previously had not been answered in the most recent round of Q& A’s.
The answers to those questions are incorporated into Amendment 5.
4. It was brought to the governments attention that the RFP has unrealistic page limits to the following sections:
Executive Summary (3 pages) was revised to (7 pages) in the RFP also language was added to the RFP as follows that requires offerors to acknowledge all amendments to the RFP in their proposal. This language was added in the 3.1 Narrative Summary and is provided below.
“The narrative summary of the entire proposal should be concise, to include addressing the significant risks, and highlighting any key or unique features, excluding cost/price. The salient features should tie in with evaluation factors/subfactors. Any summary material presented here shall not be considered as meeting the requirements for any portions of other volumes of the proposal. Offerors shall include in this section a statement of Acknowledgement for all amendments associated with this Request for Proposal.”
Technical Proposal section of RFP was revised from 20 page limit to 30 pages. This was driven in response to a question asked previously however the change never made it to the RFP.
Past Performance page limit originally stated, “Past Performance 1 cover page for any explanatory notes, & 3 page max for each submitted contract” this was unrealistic as the governments past performance qustionaire was for pages long. As such the language in the RFP was revised as follows, “6.0 Volume IV- Past Performance 1 cover page for any explanatory notes, & 4 page max for each submitted contract, with a total maximum of no more than 3 contracts that may be submitted.”
