OFFEROR’S RELEVANT PAST EXPERIENCE AND PERFORMANCE HISTORY

The purpose of this form is to obtain information on the past experience and performance of the Offeror and the Offeror’s subcontractor(s) that is similar in scope and complexity to that which will be provided under this solicitation. This information, coupled with the questionnaire to be completed by the customer who received the service and is identified below, will support the FAA’s evaluation of the Offeror’s proposal.
Each customer cited in a Relevant Past Experience and Performance History form must be provided a Past Performance Questionnaire, Attachment L.2 to this solicitation, to complete and return to the FAA office listed in Section L.9.2 of this solicitation. The Offeror is to provide the information requested in this form for each past or present contract/program relevant to the requirements as set forth in Section C of this solicitation. Provide a separate completed form for each contract/program. The length of any one Relevant Past Performance History form must not exceed three (3) pages. Offerors may submit at maximum of three (3) completed Relevant Past Performance History forms for the prime contractor and no more than six (6) for subcontractors. No more than three (3) completed Relevant Past Performance History forms may be submitted for any one subcontractor.

D. Provide a description of your work effort and clearly indicate if you were the prime contractor or the subcontractor on this contract.
E. If you were the prime contractor on this contract, identify whether you acted as the integration contractor, responsible for fully integrating all services under the contract or just a portion thereof. Describe your roles and responsibilities in integrating all pertinent aspects of the program.
F. Select the applicable factors in Section L.11.2, L.11.3 and/or L.11.4 of this solicitation and explain your relevant experience on this contract and how it applies to those factors.
