About this file

This document is a dental classification and triage system established by the Maryland Department of Juvenile Services (DJS) for the Baltimore City Juvenile Justice Center (BCJJC), which provides dental services to male youth detainees aged 12 to 20 years old referred by DJS medical staff. The triage system categorizes dental conditions into five priority classes to guide treatment scheduling and resource allocation. Class 1 encompasses emergency conditions requiring immediate dentist assessment or urgent emergency room referral, including suspected jaw fractures, knocked-out permanent teeth, dental abscesses with systemic symptoms, and acute cellulitis from dental infections. Class 2 is subdivided into Class 2a (urgent scheduling with patient bumping capability) for conditions such as abscesses with moderate-to-severe pain, symptomatic impaction, and acutely fractured teeth, and Class 2b (expedited scheduling) for other dental pain and severe periodontal problems. Class 3 addresses moderate priority conditions scheduled as time allows, including asymptomatic caries, gingivitis, and completion of initial dental examinations required within 60 days of admission. Class 4 designates low-priority preventive care such as prophylaxis with fluoride treatment every six months and annual comprehensive examinations. Class 5 identifies care that can be deferred until after discharge, including initial orthodontic care, asymptomatic impacted wisdom teeth, and cosmetic dental work not affecting function.

The triage system also establishes protocols for off-site dental referrals from DJS facilities with on-site dentists, categorizing specialized care needs by provider type. Emergency care referrals follow the Class 1 criteria outlined above. Oral surgeon referrals are indicated for complicated extractions, jaw fractures, and tooth intrusion or rotation. Endodontist referrals are specified for posterior or multi-rooted root canal procedures and complicated pulpotomy or pulpectomy cases. Orthodontic care requiring acute and follow-up treatment is also designated for external referral. All dental services are performed on-site at the BCJJC dental suite.
