About this file

This document is a comprehensive insurance certificate and policy endorsement package for Amazing Grace Arizona Families LLC, covering multiple insurance lines including commercial general liability, automobile liability, and workers' compensation. The insurance is provided by Philadelphia Indemnity Insurance Company, NGM Insurance Company, and CopperPoint Insurance Company, with Woodruff Leavitt Insurance Agency as the producer. The primary certificate holder is the State of Arizona Department of Child Safety, Office of Procurement & Contracts, located in Phoenix, Arizona, for a contract related to Family Connections & Nurturing Parenting (Contract #CTR077249).

The insurance policies provide coverage from February 1, 2025 to February 1, 2026, with various liability limits including $1,000,000 per occurrence for commercial general liability, $1,000,000 for sexual abuse and molestation coverage, and $3,000,000 aggregate for professional liability. The policies include additional insured status for the State of Arizona and its departments, agencies, boards, commissions, universities, officers, and employees. Special provisions cover liability arising from the contractor's activities, with waivers of subrogation and specific endorsements for things like vehicle usage, employee coverage, and protection against unintentional errors or omissions. The policies are designed to comprehensively protect the contractor and the state entity during the performance of services under their contractual agreement.
