About this file

This is a Contractor Safety Evaluation Form issued by the City of Lakeland, Florida, Risk Management and Purchasing Department for contractors bidding on public safety projects. The form requests comprehensive safety information from prospective contractors, including company background, accident statistics compared to national Bureau of Labor Statistics averages for the contractor's industry classification, Experience Modification Rate (EMR), and documentation of safety practices. The form requires contractors to detail their safety meeting schedules, employee training and orientation programs, written safety policies, and field safety inspection procedures. Prospective contractors must acknowledge understanding of Process Safety Management (PSM) Standard requirements, which mandate that all contract employees receive training on fire, explosion, and toxic release hazards specific to their assigned work, with documentation including employee identity, training dates, and verification of comprehension. Additionally, contractors must complete a Pre-Work Safety Assessment (PSA) or Job Hazard Analysis (JHA) for all work on PSM-covered systems, with required review by all personnel involved at the project's beginning and as needed throughout execution, and any modifications must be approved by the senior contractor supervisor and either the City of Lakeland Safety Manager or North McIntosh Plant Superintendent.

The evaluation form requires submission of an officer's signature confirming the contractor's understanding of and commitment to PSM Standard compliance requirements. Contractors must provide detailed explanations of their intended compliance methods and may submit additional information they deem relevant to demonstrating their safety program's adequacy. The form serves as a critical qualification document for contractors seeking to work on the City of Lakeland's public safety scheduling and timekeeping software project, which carries a budgetary estimate of $150,000 for year-one licensing and setup costs and $75,000 for annual recurring license renewals, with proposals due by April 23, 2026.
