About this file
This is a Common Carrier Attestation form for Pinellas County, Florida, required as part of RFP No. 25-0653-RFP for a comprehensive Learning Management System (LMS). The attestation document is a mandatory compliance requirement that contractors must complete and submit as part of their proposal process. The form requires an authorized representative of the contractor to attest, under penalties of perjury, that they are in conformity with Florida Statute 92.525 and will not knowingly provide services to transport unauthorized aliens into Florida during the contract term, except to facilitate detention, removal, or departure. The document must be signed and dated by the contractor's authorized representative, and must include the contractor's name, Federal Work Authorization User Identification Number, and the specific name and number of the Pinellas County contract to which it applies.
The County reserves the right to terminate the associated agreement for cause if the contractor is found in violation of the attestation. This attestation requirement is one of several mandatory legal affidavits that vendors must complete to be eligible for consideration in the LMS procurement, alongside human trafficking certifications and restrictions on contractors from specified foreign countries of concern. Completion and submission of this form is a prerequisite for proposal submission and contract award for the 60-month initial contract term with two optional 12-month renewal periods.
