About this file
This is a Task Assignment Change Order issued by the Florida Department of Environmental Protection (FDEP), Division of Waste Management, Petroleum Restoration Program. The document authorizes changes to the scope of work for Contract #GC904, Purchase Order #TA-GC904-222A, with contractor Geosyntec Consultants. The facility involved is the Coyote Navarre C&D Debris Disposal Facility, identified by 9-Digit Facility ID #570013332. The FDEP Task Manager is Brittany Anding, with administrative review by Melissa Brock. FDEP maintains a 100% cost share for this project.
The change order addresses revised petroleum storage tank capacities discovered during field work. Initial reported capacities of waste oil and diesel aboveground storage tanks (ASTs) of 1,000 and 3,000 gallons were found to actually total 1,468 and 7,520 gallons respectively based on physical dimensions. Consequently, Geosyntec reduced one line item for tank removal and disposal (SPI 11-2, >1,000 to 5,000 gallons at $5,970.27) and added one line item for larger tank removal (SPI 11-3, >5,000 to 10,000 gallons at $8,358.37). Additionally, 985 gallons of petroleum contact water transport and disposal (SPI 12-14 at $2.26 per gallon) was requested, totaling $2,226.10. A contingent funding allowance (SPI 23-1) of $4,614.20 was offset against these costs, resulting in a net change order total of $0.00. No changes were made to deliverable due dates or project end date of August 2, 2023.

This is an authorization for the costs associated with the change in quantities of services being provided and/or deliverable due dates. In order for these costs to be paid, these changes must be processed through a change order to the purchase requisition and a revised Purchase Order issued by MFMP prior to initiating work.
DESCRIPTION
Remove and Dispose Petroleum Storage Tank - > 1,000 to 5,000 gal.
capacity
Remove and Dispose Petroleum Storage Tank - > 5,000 to 10,000 gal.
capacity
Transport and Disposal of Petroleum Contact Water (bulk)
Contingent Funding - Allowance only to be used as offset for field change orders
Description of Change and Justification: Include complete description of who, what, where, when, how and why.
The reported capacity of the waste oil and diesel ASTs were 1,000 and 3,000 gallons, respectively. However, the dimensions of the waste oil tank were 5 feet in diameter and 10 feet long and the diesel tank was 8 feet in diameters and 20 feet long. The capacities of the waste oil and diesel ASTs are 1,468 gallons and 7,520 gallons, respectively. Geosyntec, is reducing 1 SPI Line Item 11-2 (remove and disposal of storage tank ->1,000 to 5,000 gal) and adding 1 SPI Line Item 11-3 (remove and disposal of storage tank ->5,000 to 10,000 gal).
885 gallons of PCW was pumped from the waste oil AST and 850 gallons of PCW from the diesel AST. Transport & Disposal of 750 gallons of PCW (SPI Line Item 12-14) was included in CO#1. Therefore, Geosyntec is requesting an additional 985 gallons of SPI Line Item 12-14. There is no cost associate with this TACO.
