About this file
This is a Task Assignment Change Order issued by the Florida Department of Environmental Protection (FDEP), Division of Waste Management, Petroleum Restoration Program, authorizing a change in scope of work for Contract #GC904. The contractor is Geosyntec Consultants, and the change order applies to Purchase Order #TA-GC904-222A for work at the Coyote Navarre C&D Debris Disposal Facility. The document is designated as a Rush-type change order with 100% FDEP cost share. Administrative and technical approvals were provided by Melissa Brock and Natasha Lampkin on April 6, 2023.
The change order authorizes additional services to address an unanticipated discovery during site reconnaissance mobilization: approximately 250 gallons of petroleum contact water found in a 1,000-gallon aboveground storage tank (AST). The contractor requested authorization for one heavy-duty/stakebed truck round trip (Pay Item 3-3 at $817.93) and transport and disposal of up to 750 gallons of petroleum contact water in bulk (Pay Item 12-14 at $2.26 per gallon for 750 gallons). The total authorized cost for this change order is $2,512.93, representing the sum of $817.93 for the truck round trip and $1,695.00 for petroleum contact water disposal, with the actual amount removed to be invoiced based on actual quantities. No changes were made to deliverable due dates or the overall period of service ending August 2, 2023.
