About this file
This is a Task Assignment Notification Form (Task Assignment No. 4) under DEP Agreement Number GC892, a state contract between the Florida Department of Environmental Protection (DEP) and the City of Jacksonville Environmental and Compliance Department. The task assignment covers the period from July 1, 2019, to June 30, 2020, with a total value of $1,661,607.68. The Local Program Contract Manager is Allene Rachal, and the DEP Contract Manager is David G. Warfield. The contract is managed through Jacksonville's Environmental and Compliance Department and involves work specified in Attachment A of the master contract GC892.
The contract encompasses petroleum cleanup site management services for 582 non-Low Score Assessment (LSA) source properties and zero LSA source properties under a fixed-cost arrangement. The annual fixed base rate for petroleum cleanup site management is $1,560,195.00. Compensation varies by site phase: planning phase sites receive $3,150.00 per year; remediation phase sites receive $6,300.00 per year for the first two years and $3,150.00 thereafter; monitoring phase sites receive $1,890.00 per year; and non-program voluntary cleanup sites receive 75 percent of the applicable level of effort cost. Performance incentives of $750.00 per completed target are available, with an annual maximum of 10 percent of total non-LSA source property compensation. The contract includes a 5 percent monthly retainage from non-LSA source property compensation, with 80 percent release based on four performance metrics (deliverable turnaround times, change request turnaround times, data entry accuracy in the STCM database, and document insertion into OCULUS), and 20 percent release contingent upon achieving a 98 percent site inspection rate. The contract document includes a comprehensive list of source properties requiring inspection during fiscal year 2019-2020.
