About this file
This is a bid package for the Royal Oak School District in Michigan for window replacement work at Royal Oak High School. The document provides the standard bidding forms and compliance requirements that contractors must complete and submit with their bids. While the specific scope of work, project timeline, and quantity of windows to be replaced are not detailed in this excerpt, bidders are required to submit warranty information, prompt payment terms as a percentage and number of days, and documentation of background checks conducted on employees.
Bidders must provide a DUNS number if available and submit at least three professional references with contact information. The bid package requires multiple compliance certifications including a notarized familial relationship disclosure statement per Michigan Code MCL 380.1267, confirming no family relationships exist between the bidder's ownership or employees and members of the Royal Oak Schools Board or the Superintendent. Additionally, bidders must submit a notarized affidavit certifying compliance with the Iran Economic Sanctions Act (Michigan Public Act No. 517 of 2012), confirming the contractor is not an "Iran linked business" and will not become one during contract performance. Violations of the Iran sanctions certification carry substantial penalties of up to $250,000 or twice the contract amount, plus investigation costs and attorney fees, along with a three-year bid ineligibility period. Bidders must also certify they are not on the Excluded Parties List that prohibits federal contracting.
