About this file
The document is an Attachment K Foreign Country of Concern Attestation form (PUR 1355) associated with Florida Administrative Code Rule 60A-1.020, designed for entities submitting bids, proposals, or entering contracts with governmental entities that would provide access to Personal Identifying Information. The form requires an officer or representative of the bidding entity to attest that their organization is not owned by, organized under the laws of, or principally located in a Foreign Country of Concern, and that no foreign government has a controlling interest in the entity.
The attestation form is a legal declaration made under penalties of perjury, requiring the submitting entity's representative to print their name, provide their title, and sign and date the document. By completing this form, the entity affirms its compliance with specific regulations regarding foreign ownership and business relationships, which appear to be part of a broader strategy to protect sensitive government information and maintain security standards for contract opportunities.
