About this file
Attachment K is a Foreign Country of Concern Attestation form issued by the Florida Department of Management Services for the Data Management Tools RFP (No. 25-43232300-RFP). The form is a mandatory certification document that must be completed by an officer or representative of an entity submitting a bid, proposal, or entering into a contract that would grant access to personal identifying information. The form requires the bidding entity to attest that they are not owned by, organized under the laws of, or have a principal place of business in a Foreign Country of Concern, and that no such government has a controlling interest in the entity.
The attestation form is part of a comprehensive procurement process for a statewide contract to provide data management tools across four service categories: Data Management, Data Governance, Data Analytics, and Data Interoperability and Integration. The form supports the state's compliance requirements, which include verifying vendors' international business conduct and ensuring that potential contractors do not have connections to countries of concern. By requiring this attestation under penalties of perjury, the Florida Department of Management Services aims to protect the state's data infrastructure and maintain stringent vendor qualification standards for this critical technology procurement.
