About this file
This is an attestation form (PUR 1355) required by the Florida Department of Management Services for the Contact Center Services RFP (26-83111500-RFP). The form must be completed and signed by an authorized officer or representative of any entity submitting a bid or proposal for this statewide contract opportunity. The attestation certifies under penalties of perjury that the responding entity is not owned by, organized under the laws of, nor has its principal place of business in a Foreign Country of Concern, and that no such foreign government maintains a controlling interest in the entity. This compliance requirement applies to all vendors seeking to enter into, renew, or extend a contract with the State of Florida that would grant access to personal identifying information. The form references Rule 60A-1.020, F.A.C., which defines the applicable terms and establishes the regulatory framework for this attestation requirement.
The Foreign Country of Concern Attestation is a mandatory responsiveness requirement for the Contact Center Services procurement, which seeks vendors capable of providing single-tenant and multi-tenant contact center solutions including automatic call distribution, interactive voice response, call recording, workforce management, and related services. Proposals are due March 17, 2026, at 10:00 AM Eastern Time through the MyFloridaMarketPlace Sourcing Event, with a public opening at 11:00 AM the same day. The Department intends to make multiple statewide awards with an initial three-year term and potential for renewals. Evaluation will be based on 1,160 total points (660 technical and 500 cost), with an anticipated Notice of Intent to Award on May 4, 2026. Additional mandatory compliance certifications include Drug Free Workplace Certification, E-Verify requirements, and verification that the vendor is not on any suspended, convicted, discriminatory, antitrust violator, or forced labor vendor lists.
