About this file
This is an attestation form (PUR 1355) required by the Florida Department of Management Services for the Contact Center Services RFP (Solicitation No. 26-83111500-RFP). The form must be completed and signed by an officer or representative of any entity submitting a bid or proposal for this solicitation that would grant the entity access to personal identifying information. The form requires the entity name, printed name of the signatory, title, signature, and date, and is executed under penalties of perjury in accordance with Rule 60A-1.020, Florida Administrative Code.
This attestation is a mandatory compliance requirement for all respondents to the Contact Center Services RFP, which seeks comprehensive contact center solutions including automatic call distribution, interactive voice response, reporting and analytics, call recording, and workforce management services for Florida state agencies. Proposals must be submitted electronically through MyFloridaMarketPlace by 10:00 AM Eastern Time on March 17, 2026, with evaluation anticipated from March 31 through April 14, 2026, and a Notice of Intent to Award planned for May 4, 2026. The contract will have an initial three-year term with potential renewal options. Vendors must also comply with other mandatory requirements including avoiding suspension or conviction vendor lists, certifying no offshoring and E-Verify compliance, and submitting drug-free workplace and foreign country of concern attestations. The Department intends to make multiple statewide awards but reserves the right to award to one or multiple respondents by region or make no award based on the state's best interests.
