About this file

This is a State Corporation Commission Form attachment for a Virginia Works janitorial services contract opportunity at the Cedar Bluff office location in Cedar Bluff, Virginia. The form establishes registration and business entity requirements for bidders participating in this procurement.

The form requires bidders to indicate their business status relative to Virginia State Corporation Commission registration. Bidders must confirm one of four conditions: registration as a Virginia corporation or business entity with an SCC identification number; operation as a non-corporate entity exempt from SCC registration; status as an out-of-state business entity with no regular and continuous business presence in Virginia; or submission of legal counsel opinion explaining out-of-state contacts that do not constitute transacting business in Virginia under applicable state code. The form also provides an option for bidders with pending SCC applications to request a waiver allowing submission of the identification number after the bid due date, subject to Commonwealth discretion.
