About this file
This document is a Mandatory Features Checklist for a Group Dental Insurance Request for Proposal (RFP) issued by the Brevard County Board of County Commissioners, effective January 1, 2026. The checklist outlines nine critical mandatory requirements for potential dental insurance proposers, including submission of specific documentation, licensing requirements, and service capabilities. Proposers must have at least five years of experience in Florida providing group dental insurance, with a proven track record of servicing groups with 4,000 or more employees. The RFP requires proposers to offer dental DHMO and PPO plan options with specific coverage parameters, including no minimum participation requirements and the ability to continue coverage for retired employees and their dependents.
The document emphasizes that any proposal failing to meet any of the nine mandatory features will be immediately disqualified from consideration. Key requirements include submitting three hard copies and one electronic copy of the proposal, using specified file formats (MS Word and MS Excel), and being the prime contractor performing at least 80% of contract work. The proposer must provide a single contract with separate billing for each agency and match the County's definition of dependents, which includes children up to age 30. Proposers are expected to provide benefits that meet the current in-force coverage limits and must note any deviations or enhancements in their proposal. The document concludes with a signature section requiring authorization of the proposal's accuracy by an authorized representative of the proposing firm.
