About this file
This is a Hawaii Bidder/Offeror Preference Certification Form (Form SPO-081) related to the Office of Hawaiian Affairs' request for proposals for Monday.com consulting, implementation, optimization, and training services. The form requires bidders to certify their eligibility for Hawaii's local business preference program under HRS 103D-1001. Bidders must maintain a valid commercial place of business in Hawaii that has been staffed for at least two consecutive years prior to proposal submission, submit their bid under their authorized business name in the State, and commit to performing at least 51 percent of total direct labor hours within Hawaii. For joint ventures, all parties must meet these same requirements.
Bidders who are certified as Hawaii preferential bidders must acknowledge their obligation to maintain qualification status throughout contract performance and report any changes in eligibility to the contract administrator or purchasing agency contact. The form explicitly warns that failure to maintain the required qualifications will trigger a price adjustment equal to the value of the preference applied and may result in debarment under HRS 103D-702. Authorized officers signing the form certify under penalty of perjury that all statements are true and agree to comply with these requirements for the duration of contract performance.
