PAST PERFORMANCE INFORMATION WORKSHEET
Submit information on recent contracts that you consider relevant in demonstrating your ability to perform the Laughlin Air Force Base (AFB) Base Operation Support (BOS) Re-compete requirement. Include rationale supporting your assertion of relevance. For a description of the characteristics or aspects the Government will consider in determining relevance, see Section M, Table M-5 of the RFP. To be recent, the effort must have been performed during the three years preceding the date of issuance of this RFP, to include at least 12-months of documented performance during the three-year period. Contract performance that meets these criteria will be evaluated in their entirety. Contract performance that does not meet these criteria will not be evaluated.
Offerors may include efforts accomplished to resolve problems encountered on the referenced contract as well as efforts to identify and manage program risk. Merely having problems does not automatically equate to less than Satisfactory rating since the problems encountered may have been on a more complex program, or an Offeror may have subsequently demonstrated the ability to overcome the problems. The Offeror should clearly describe management actions employed in overcoming problems and the effects of those actions, in terms of improvements achieved or problems rectified.
Provide the information requested in this submittal for each contract/program being described. Limit the number of efforts submitted to no more than a combined total of six (6) contracts. The length of each worksheet submission is limited to no more than seven (7) pages. Past Performance worksheets do not count against the page limitation (40 pages) for Volume II - Past Performance.
This cover page should be deleted when submitting the Past and Present Performance Information Sheet.
