About this file

This is a Business Proposal Attachment E document for RFP 26-87847 issued by the State of Indiana, specifically the Indiana Department of Administration (IDOA). The attachment serves as the template and instructions for respondents to submit comprehensive business proposal information as part of their RFP response. While the document does not specify particular products or services, it establishes the framework requiring respondents to demonstrate their qualifications, organizational structure, financial stability, and experience serving governmental entities. The proposal must include company information, financial documentation, references from three similar clients, evidence of Indiana Secretary of State registration, and details regarding business continuity and disaster recovery capabilities. Respondents are required to identify their proposed system type and indicate acceptance of or proposed modifications to the state's standard cloud-based terms and conditions for Infrastructure-as-a-Service (IaaS), Platform-as-a-Service (PaaS), and Software-as-a-Service (SaaS) systems.

The proposal process requires respondents to demonstrate financial responsibility through Dunn & Bradstreet Business Reports or audited financial statements for the two most recently completed fiscal years, with certification by the CEO and/or CFO regarding the accuracy of submitted financial information. Respondents must complete reference documentation on Attachment H, which must be submitted directly by references to idoareferences@idoa.in.gov by the proposal due date specified in RFP Section 1.24. Additionally, the document addresses diversity subcontracting requirements, specifically requiring respondents to explain their engagement process with Indiana Veteran Owned Small Business (IVOSB) subcontractors and to provide rationale for any IVOSB partnerships not proposed. Respondents must also indicate their willingness to extend awarded pricing to other governmental bodies and provide documentation of authorization for contract signature, along with information regarding their experience serving state governments and similar-sized clients.
