About this file
This is a Business Proposal Attachment E document for RFP 26-85939 issued by the State of Indiana, specifically the Indiana Department of Administration (IDOA). The document serves as the instructions and template for respondents to provide comprehensive business proposal information across multiple sections. While the RFP does not specify the particular products or services being procured in this attachment, respondents are required to submit general company information, organizational structure, financial stability documentation, and experience details. The proposal requires submission of three reference contacts from clients with similar service experience, proof of registration to do business with the Indiana Secretary of State, and evidence of authorization for signing representatives. Respondents must also demonstrate their engagement efforts with Minority & Women's Business Enterprises (MBE/WBE) and Indiana Veteran-Owned Small Business (IVOSB) subcontractors prior to proposal submission.
The Business Proposal requires respondents to provide detailed financial information including Dunn & Bradstreet Business Reports or audited financial statements for the two most recently completed fiscal years, with CEO/CFO attestation to the accuracy and completeness of all financial data. Respondents must demonstrate financial stability and corporate responsibility, including separation of audit functions and board integrity measures. Additionally, respondents must address their company's business continuity and disaster recovery planning, technology and security protocols for state information, and experience serving state governments and similar-sized clients. The proposal also requires respondents to indicate acceptance or propose redlined edits to cloud-based Additional Terms and Conditions (IaaS, PaaS, and SaaS) as applicable to their proposed system, with the State strongly preferring no deviations from the provided terms. Respondents must also indicate their willingness to extend pricing to other governmental bodies and provide general company information including Federal Tax Identification Number, employee count, office locations, and revenue data.
