About this file

This is a cost proposal template for RFP 26-87928 concerning the establishment of a Comprehensive Rehabilitative Management Needs Facility (CRMNF), submitted to an unnamed state agency. The project encompasses facility construction and staffing across four cost categories: facility construction (including site preparation, kitchen installation, CMS-compliant sleeping accommodations, furnishings, fire and safety systems, emergency preparedness supplies, phone systems, and office setup), staffing and training (including recruitment, initial staff training across medical and behavioral disciplines, QIDP onboarding, activities director onboarding, nursing staff onboarding, and administrative staff), administrative and licensing (including Medicaid certification and policy development), and ongoing operational costs. Respondents must submit pricing for all components across a four-year initial contract period (2026-2027 through 2029-2030) and must also provide optional pricing for two one-year extension periods. The total bid amount will be evaluated based on the four-year initial term.

The state has allocated $2,000,000 for startup costs distributed across five milestone payments: 20% ($400,000) upon contract award, 20% ($400,000) upon notice to proceed, 20% ($400,000) upon construction completion and Life Safety Survey verification, 20% ($400,000) upon achievement of Life Safety Certification, and 20% ($400,000) upon project completion and closeout. All funds are restricted to allowable non-Medicaid expenses including facility renovation, security upgrades, equipment, and initial staff onboarding. Respondents must provide all-inclusive pricing with no additional costs accepted by the state. Detailed records and verification documentation are required with each payment request, and the state does not guarantee coverage for construction or facility readiness cost overruns.
