About this file
This is a Felony Conviction Notification form required by the State of Texas under Legislative Senate Bill No. 1, Section 44.034, applicable to entities entering into contracts with school districts. The form mandates that any person or business entity contracting with a school district must provide advance notice if the person, owner, or operator has been convicted of a felony, including a general description of the conduct resulting in the conviction. The form requires an authorized representative to certify the accuracy of the information provided and to select one of three options: confirming the firm is a publicly-held corporation (exempt from the requirement), certifying that the firm is not owned or operated by anyone convicted of a felony, or disclosing the names and details of any owners or operators with felony convictions. The form includes signature lines corresponding to each declaration option.
School districts retain the authority to terminate contracts with entities that fail to provide required notice or that misrepresent information regarding convictions, though the district must compensate the entity for services performed prior to contract termination. This notification requirement applies to the Charter Bus Services solicitation being conducted by the Amarillo Independent School District, with proposals due by October 13, 2026 at 2:00 p.m. All prospective bidders must complete and submit this certification as part of their proposal package.
