About this file
This is a Bidder Response Form (Part 1 of 3) for a competitive solicitation issued by the Washington State Department of Health (DOH) Office of Drinking Water. The solicitation (RFQQ DOH26-004C) seeks a qualified contractor to manage the transition from the DOH's Sentry data platform to the U.S. Environmental Protection Agency's Safe Drinking Water Information System (SDWIS) State application suite. The initial contract period is expected to extend for approximately six years. The form functions as a standardized template requiring bidders to complete four sections: a bid submittal checklist, bidder responsiveness and responsibility assessment, bidder certification, and Executive Order 18-03 compliance response. Bidders must submit all required attachments including a bid submission letter and bidder profile (Attachment A), diverse business inclusion plan (Attachment B), bidder certifications (Attachment C), and any general terms and conditions change requests (Attachment D), along with minimum and desirable qualifications and technical qualifications documented in Excel workbooks 9 and 10.
The form establishes evaluation criteria that include bidder capacity, character, integrity, reputation, and past contract performance. DOH reserves the right to request financial statements, credit ratings, references, and facility inspections to assess bidder capability. Small businesses located in Washington with 50 or fewer employees or gross revenue under $7 million may receive five percent additional points if self-identified in Washington's Electronic Business Solution (WEBS). Certified Washington Veteran-Owned Businesses meeting specific ownership and certification requirements through the Washington Department of Veterans' Affairs also receive five percent additional points. Bidders certifying that employees are not required to sign mandatory individual arbitration clauses or class action waivers as a condition of employment receive five percent additional points. Bidders must ensure all responses are legible, complete, and signed by authorized representatives; alterations to the form beyond requested information will result in bid disqualification.
