About this file
This is a Federal Reporting List attachment to a Request for Qualifications and Quote (RFQQ) issued by the Washington State Department of Health (DOH) Office of Drinking Water (ODW) for the transition from the DOH data platform (Sentry) to the EPA Safe Drinking Water Information System (SDWIS) State application suite. The document provides a comprehensive catalog of National Primary Drinking Water Regulation (NPDWR) violation codes and contaminant codes that must be reported to federal authorities. The attachment specifies 315 distinct violation items organized by regulation type—including radiological contaminants, nitrates, Phase II/V chemicals, lead and copper, turbidity, disinfection byproducts, and total coliform rules—each with corresponding SDWIS contaminant codes, violation descriptions with Code of Federal Regulations citations, and violation type classifications (MCL, M&R, TT, MRDL, or OTHER). The document also includes a comprehensive NPDWR Groups-Codes reference table listing analyte codes for contaminants across multiple regulatory categories, including inorganic chemicals, synthetic organic chemicals, volatile organic compounds, radionuclides, disinfection/disinfection byproducts, nitrate/nitrite, surface water treatment, lead and copper, and revised total coliform rules.
This Federal Reporting List serves as the technical specification document for the data migration project, establishing the standardized violation codes and contaminant classifications that the new SDWIS system must accommodate during the transition from Sentry. The document does not contain pricing information, set-asides, or funding details, as it functions as a reference guide for technical compliance requirements rather than a pricing or procurement document. The initial contract period is expected to be approximately six years, though the document does not specify contract award timelines, renewal options, or other procurement-specific details. This attachment ensures that the contractor selected through the RFQQ 26-004C process will understand the complete scope of federal reporting requirements and violation classifications that must be accurately mapped and maintained in the new SDWIS platform.

276 RTCR 3D 8000 A system that fails to use the 100mL standard sample volume required for analysis, regardless of analytical method used for a routine sample. [141.852(a)(1)]
Monitoring
277 RTCR 3D 8000 A system that fails to determine the presence or absence of total coliforms and E. coli for a routine sample. [141.852(a)(2)]
Monitoring
278 RTCR 3D 8000 A system that fails to keep the time from sample collection to initiation of test medium incubation to 30 hours or less for all samples. [141.852(a)(3)]
Monitoring
279 RTCR 3D 8000 A system that provides chlorination, fails to add sufficient sodium thiosulfate to the sample bottle before sterilization in order to neutralize any residual chlorine in the water sample if the water has residual chlorine (measured as free, combined, or total chlorine) for all samples.
[141.852(a)(4)]
Monitoring
280 RTCR 3D 8000 A system that fails to conduct total coliform and E. coli analyses in accordance with one of the analytical methods in the table referenced in 141.852(a)(5) or one of the alternative methods listed in Appendix A to Subpart C of Part 141 for all samples. [141.852(a)(5)]
Monitoring
281 RTCR 3D 8000 A systerm that fails to have all compliance samples, required under the Revised Total Coliform Rule (RTCR), analyzed by a laboratory certified by EPA or a Primacy Agency to analyze drinking water samples. The laboratory used by the system must be certified for each method and associated contaminants used for compliance monitoring analyses. [141.852(b)]
Monitoring
282 RTCR 3D 8000 A system that fails to collect a replacement routine sample from the same location as the original sample within 24 hours of being notified that the laboratory must invalidate total coliform sample (unless total coliforms are detected), if the sample produces a turbid culture in the absence of gas production using an analytical method where gas formation is examined, produces a turbid culture in the absence of an acid reaction in the Presence-Absence coliform test, or exhibits confluent growth or produces colonies too numerous to count with an analytical method using a membrane filter. The Primacy Agency may waive the 24 hour time limit on a case by case basis.
Alternatively, the Primacy Agency may implement criteria for waiving the 24 hour time limit to use in lieu of case by case extensions. [141.853(a)(3), 141.853(c), 141.853(c)(2), and 141.860(c)]

283 RTCR 3D 8000 A system that fails to collect replacement routine samples to meet the minimum monitoring requirements of the Revised Total Colfirom Rule (RTCR) when a total coliform-positive sample is invalidated because of conditions listed in 141.853(c)(1)(i-iii). The laboratory establishes that improper sample analysis caused the total coliform-positive result. The Primacy Agency determines the total coliform-positive sample resulted from a domestic or other nondistribution system plumbing problem. The Primacy Agency has substantial grounds to believe that a total coliform-positive result is due to a circumstance or condition that does not reflect water quality in the distribution system.[141.853(c), 141.853(a)(3), 141.860(c), and 141.853(c)(1)(i-iii)]
Monitoring
284 RTCR 4A 8000 A system that fails to submit a completed assessment form after a system properly conducts assessment in a timely manner within 30 days. [141.860(d)(1) and 141.861(a)(3)]
Reporting
285 RTCR 4B 8000 A system that properly conducts monitoring, but fails to submit the monitoring report in a timely manner. [141.860(d)(1)]
Reporting
286 RTCR 4B 8000 A system that fails to notify the Primacy Agency within 10 days about the monitoring violation after the system fails to comply with a coliform monitoring requirement, in which case the system must notify public in accordance with Subpart Q of Part 141. [141.861(a)(4)]
Reporting
287 RTCR 4C 8000 A system that conducts seasonal system start-up procedures, but fails to submit certification of completion of Primacy Agency-approved start-up procedures or certify prior to serving water to the public that it has complied with the Primacy Agency-approved start-up procedures.
[141.860(d)(3) and 141.861(a)(5)]
Reporting
288 RTCR 4D 8000 A system that has an E. coli -positive routine or repeat sample, fails to notify the Primacy Agency by the end of the day when the system is notified of the test result, unless the system is notified of the result after the Primacy Agency office is closed and the Primacy Agency…
