About this file
This is a vendor disclosure and notification form issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, serving as official notice regarding contract performance outside the United States or State of Colorado. The form is required to be completed and returned by vendors bidding on solicitation number DQ 2027000119 and must be signed by the business president, board chairperson, or authorized contract signatory. The document requires vendors to disclose whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if applicable, to identify the specific type of services, locations where services will be performed by country and state, and justification for performing work outside these jurisdictions.
The form serves as compliance documentation with Colorado Revised Statutes 24-102-206 and 24-102-205, establishing a formal notification process for the Department of Personnel regarding vendor intent to perform contract work externally. Vendors must indicate within the procurement timeline whether services will be performed outside the specified geographic boundaries and provide detailed explanations of service types and geographic locations. The procurement office retains responsibility for recording contract type, services performed outside the U.S. or Colorado, and verification that the vendor provided notice within 20 days of contracting or deciding to contract outside these areas. No specific pricing, term length, or award information is provided in this disclosure form, as it functions solely as a notification and compliance mechanism for vendors.
