About this file

This is a URS Post-Retired Verification Form used by Jordan School District in Utah to verify the employment and retirement status of contractors and independent contractors before engagement. The form is required to be submitted along with a W-9 to the Accounting Department and applies to any contractor, owner, partner, or principal who may have previously retired from Utah Retirement Systems (URS). Bidders must disclose whether they or any associated parties have retired from URS and, if applicable, provide their birthdate and type of business. The form is directly associated with the District Wide Elevator Maintenance and Repair Invitation for Bid issued by Jordan School District in accordance with Utah Code Annotated Chapter 63G-6a and applicable Utah Administrative Rules.

URS retirees who are receiving monthly benefits face specific restrictions on returning to work with participating URS employers, including work performed as employees or independent contractors. Any retiree identified as the contractor, owner, partner, or principal must report re-employment to URS using Form RTRT-27A before commencing work with Jordan District. Failure to report re-employment violates Title 49 of the Utah Code and subjects the retiree to repayment of any benefit overpayments. Retirees are directed to contact URS's Retirement Benefits Department at 801-366-7770 prior to beginning work to ensure compliance with reporting requirements and to complete the necessary Post-Retirement Employment documentation.

If it is discovered that a URS retiree failed to report reemployment to URS on its Post-

Retirement Employment form (Form RTRT-27A) in violation of Title 49, the retiree will be liable to repay URS the amount of any overpayment resulting from the violation to report. See Utah code section 49-11-1207(2).
