About this file

This document appears to be a comprehensive proposal response for an Arizona Department of Transportation (ADOT) solicitation, specifically for an on-call diamond grinding project. The document is a detailed Excel-based questionnaire containing multiple sections covering proposal requirements, including general offer requirements, method of approach, capacity planning, equipment details, subcontractor information, company experience, and pricing terms. The proposal is submitted by WSP, a company seeking to provide transportation infrastructure services.

The solicitation covers diamond grinding project services, with the contractor required to provide detailed information about their operational capabilities, including service crews, equipment inventory, and subcontractor relationships. Pricing elements include a maximum 15% markup for extra work, with the option for prompt payment discounts. The proposal includes attachments demonstrating the company's experience with similar governmental projects, equipment lists, capacity planning documentation, and a comprehensive method of approach. Key submission requirements include completing various forms such as equipment lists, subcontractor details, and compliance certifications related to Israel boycott restrictions and forced labor regulations.

INSTRUCTIONS
English
You can use this file to create and/or update your questionnaire.
Important: This file's format is specific to the questionnaire type it is associated with and cannot be used for another type.
This file contains two data tabs: Questions and Constraints.
*Questions tab: Use this tab to add your questionnaire's sections, sub-sections, and questions.
*Constraints tab: Use this tab to set up conditional behaviors based on respondents' answers.
Questions Tab
An easy way to figure out how to create and set up the various types of questionnaire elements (sections, subsections, and questions) is to create a test item of each type, and then download the MS Excel® file.
In most cases, column values must be selected from a predefined list. This list can be accessed by clicking the cell's down arrow (lower right corner).
In the case of Multiple Choice questions, you must provide the list of possible answers and associated scoring points.
*For a non scored multiple choice question, enter possible answers in the "Choices for answer" column and speficy an associated score of zero for each answer, like so:
Yes=0

No=0

*For a scored multiple choice question, enter possible answers in the "Choices for scoring" column and speficy each answer's associated score, like so:
Audited & Certified quality system, good maintenance=10

Audited & Certified quality system=7 Non-certified Quality system=5 No quality system, but steps are taken=2 No quality system, or missing certificates to back supplier's claims=0

If your questionnaire includes at least one section, then all questions must be attached to a section.
Constraints Tab
You have the abillity to set up conditional behaviors. For instance, you can choose to include some questions only if respondents select a given response, or to make a question, a comment, or a file attachment mandatory, all based on respondents’ answers.
Here is an example to illustrate how to set up a conditional behavior. Let's say you have 2 questions, both located in the same section called "Documentation":
Q14 – Do you provide a manual?
Q15 – Select available languages.
You want to display Q15 only if the answer to Q14 is YES.
This is how you would set up the columns in the Constraints tab:
Section = Documentation
Question = Do you provide a manual?
Operator = Is equal to
Value = Yes
Result = Select available languages.
Constraint type = is visible

*Pour une question à choix multiple non notée, saisissez les réponses possibles dans la colonne "Choix pour réponse" et laissez la note associée à chaque réponse à zéro, comme suit :
Oui=0

Non=0

*Pour une question à choix multiple notée, saisissez les réponses possibles dans la colonne "Choix pour notation" en précisant le nombre de points que vous souhaitez affecter à chaque réponse, comme suit :
Excellent=10

Bon=7 Satisfaisant=5 Moyen=2 Faible=0

Section = Documentation
Question = Fournissez-vous un manuel d'utilisation ?
Operation = Est égal à
Valeur = Oui
Résultat = Sélectionnez les langues disponibles.
Type de constraint = est visible
