About this file

This is a Cost Proposal Form attachment for Solicitation AS27-1, issued by the State of Utah Division of Purchasing and conducted by the State of Utah Division of State Parks. The RFP seeks to establish contracts for adaptive mobility devices, which will result in multiple contract awards determined by the highest score for each line item. Three lots are available for bidding: Lot 1 comprises one self-propelled manual device, Lot 2 comprises one electric/track chair, and Lot 3 comprises one assisted mobility device. Each line item is evaluated separately and worth 200 points. The contract term is five years, with vendor pricing guaranteed for one year.

The cost proposal requires vendors to submit the standard baseline MSRP (retail) price for each unit without catalog discounts applied, as well as a guaranteed minimum catalog discount percentage that will apply to all future equipment and accessory purchases throughout the contract term. The scoring formula for product pricing awards 200 points based on the lowest proposed MSRP total, calculated as 200 multiplied by the ratio of the lowest proposed MSRP to the vendor's proposed MSRP. The discount component is worth 100 points, calculated as 100 multiplied by the ratio of the vendor's proposed discount percentage to the highest proposed discount. Additional costs for training and maintenance/repair services are to be submitted for informational and contract purposes only but will not be scored mathematically. Vendor proposals must strictly adhere to the prescribed format, with any deviations potentially resulting in disqualification, and technical requirements and cost proposals must be reflective of one another.
