Jim Barber 909-801-5780 The purpose of this contract is to have a contractor to furnish all labor, materials, equipment and supervision necessary to repair and install for not to exceed 122 fire doors at the VA Loma Linda Healthcare System. Contractor will use attached Door Discrepancies list with goal to resolve all known fire discrpeancies to be brought up to code to meet TJC and NFPA life safety and environment of care requirements.
The solicitation and any amendments are available electronically on FedBizOpps Website (http:/www.fedbizopps.gov).
Bidders are cautioned that it is their responsibility to access the website for any amendments that may be issued under this solicitation. Pricing shall be submitted utilizing SF 1442, Page 2, Box 17 of the solicitation.
IAW VAAR 836.204 the magnitude for construction for this project is between $250,000 and $500,000.
All requests for information about this solicitation must be in writing and received prior to 2:00 p.m. March 20, 2019 to allow for amendment issuance before bid due date. Requests for Information should be submitted via email to james.barber@va.gov and include return contact information.
It is strongly recommended that bidders send a representative to attend the pre-bid site visit on Thursday March 14, 2019 at 10:00 AM at the VA Loma Linda Medical Center, meet at Front Main Entrance sidewalk, Loma Linda, CA 92357.
Individual site visits will not be conducted.
This is a 100% Service Disabled Veteran Owned Small Business Set-Aside pursuant to 38 USC 8127(d) and PL 109-461. The SDVOSB status must be verified prior to submitting a bid in accordance with VAAR 819.7003.

INFORMATION REGARDING BIDDING MATERIAL, BID GUARANTEE AND BONDS
(a) Bidding materials may be obtained by general (prime) contractors interested in submitting bids direct to the Department of Veterans Affairs and by suppliers and subcontractors for their use in preparing sub-bids for general (prime) contractors from the Federal Business Opportunities (FedBizOpps) website at https://www.fbo.gov under solicitation number 36C26219B0001. By registering at FedBizOpps, prime contractors, suppliers and subcontractors will have access to the solicitation and amendments.
(b) A Bid Guarantee is required for this solicitation. A bid guarantee is required in an amount not less than 20 percent of the bid price but shall not exceed $500,000.00 Bid Bond is to be executed on a SF 24. Failure to furnish the required bid guarantee on the proper form and amount, by the time set for the opening of bids, will require rejection of the bid in all cases except those listed in FAR 28.101-4, and may be cause for rejection even then.
(c) The bidder to whom award is made will be required to furnish two (2) bonds, a Payment Bond to be executed on the SF 25A and a Performance Bond to be executed on the proper SF 25, each in the penal sum as noted in the General Conditions of the Specification. Copies of the SF 25 and SF 25A can be obtained upon application to the issuing office

INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO BIDDERS/OFFERORS
1. PROJECT DESCRIPTION: Contractor shall provide construction services to furnish and install labor and materials for not to exceed (122) fire doors at the VA Loma Linda Health Care System (VALLHCS) using the provided noted discrepancies (See Attachment 1 Fire Door Discrepancies). All VALLHCS Fire Doors need to be brought up to code to meet TJC and NFPA life safety and environment of care requirements.
The goal is to resolve all known fire door discrepancies and services will comply with all DVA, local, state, and federal policies and regulations. All construction services at the VA Loma Linda Healthcare System, VA Loma Linda Medical Center, 11201 Benton Street, Loma Linda, CA 92357.

9. SITE VISIT: An organized site has been scheduled for: Thursday, March 14, 2019 at 10:00 A.M.. Participants will meet at VA Loma Linda Medical Center, Front Main Entrance sidewalk area, 11201 Benton Street, Loma Linda, CA 92357. Please arrive early if necessary as parking is limited at VA Loma Linda Medical Center. Attendance is not mandatory, however all offerors are strongly encouraged to attend.
10. BID CONTENTS:
a. Standard Form 1442. (Box 14-20 completed and signed acknowledging all amendments; do not fill in anything in “Award” section, from box 21 down). All Bidders are to fully complete blocks 14 through 20 of SF 1442, unsigned bids will be rejected.
b. Bid Guarantee/Bond (SF24).

13. OFFER GUARANTEE (BID BOND): An offer guarantee is required. Per FAR Clause 52.228-1 – Bid Guarantee – paragraph (c), the amount of the bid guarantee shall be 20 percent of the bid price or $3,000,000, whichever is less.

PAYMENT AND PERFORMANCE BONDS: If the contract will exceed $150,000 (see FAR 28.102-1 for lesser amount), the bidder to whom award is made will be required to furnish two bonds, a Payment Bond, SF 25A, and a Performance Bond, SF 25, each in the penal sum as noted in the General Conditions of the Specification. REQUEST FOR INFORMATION: All Requests for Information (RFIs) regarding this IFB must be received no later than Wednesday, March 20, 2019 @ 2:00 P.M. local time via e-mail (james.barber@va.gov) to allow a reply to reach all prospective bidders before the submission of their bids. Responses to questions will be posted as an amendment to the solicitation and the government reserves the right to consolidate questions and responses. 19. Amendments to the solicitation must be acknowledged (in writing only) in the space provided in Block 19 of the SF1442 or by returning a signed copy of the amendment by the bid due date otherwise a bid may be determined nonresponsive.

24. BASIS OF AWARD - SEALED BIDDING - CONSTRUCTION
Per FAR 52.214-19, the Government will evaluate bids in response to this solicitation without discussions and will award a contract to the responsible bidder whose bid, conforming to the solicitation, will be most advantageous to the Government, considering only price and the price-related factors specified elsewhere in the solicitation.
25. This solicitation is 100% Set-Aside for Service Disabled Veteran Owned Small Business (SDVOSB). Public Law 109-461, 38 USC Sec. 8127 provides SDVOSB and VOSB Set Aside authority to the VA. Businesses must be registered and verified in the VIP database at Vendor Information Pages (VetBiz).

STATEMENT OF WORK
I. TITLE:
Fire Door Compliance
II. BACKGROUND INFORMATION:
All VALLHCS Fire Doors need to be brought up to code to meet TJC and NFPA life safety and environment of care requirements.
III. OBJECTIVES:
The purpose of this contract is to have a contractor to furnish all SERVICES to furnish and install labor and materials for not to exceed (122) fire doors at the VA Loma Linda Health Care System using the provided noted discrepancies. The goal is to resolve all known fire door discrepancies.
Services will comply with all DVA, local, state, and federal policies and regulations.
PERIOD OF PERFORMANCE:
Approximately eight (8) weeks or less. All work must begin within (48) hours of contract award. Work will be done during working hours; specifically, Monday through Friday from 8am-3:30pm. Work will be coordinated with all required parties, including, but not limited to, contractor, VALLHCS, GEMS Coordinator, Safety, COR, Chief, Maintenance and Operation, Supervisor, Structures, etc. Work timelines may be subject to change due to VALLHCS operational requirements. Contractor must be able to perform work with no additional charges within (48) hours of contact by the designated VALLHCS Contracting Officer Representative (COR).

V. LICENSING REQUIREMENTS:
a. Offeror must possess a 30-hour Occupational Safety and Health Administration (OSHA) safety card.
b. Offeror must possess fitted N-95 respirator masks, gloves, safety eye wear, knee pads, safety toe boots, helmets, and safety vests.
VI. SPECIFIC REQUIREMENTS:
a. Schedule with Contracting Officer Representative (COR) or Designee (48) hours in advance prior to start of work.
b. Prior to start of job, all personnel shall have a valid Loma Linda PIV badge flash badge.
c. Prior to working, ALL energy sources (electrical, water, etc.) shall be locked and tagged out, and upon completion of work, will be re-energized and verified.
d. Prior to work each day, workers or a designated lead or supervisor are/is to check in with the Graphics Control in the plant.
e. Contractor will complete a VALLHCS interstitial work permit request prior to starting work and upon work completion. Permit must be kept on site and easily accessible for work duration.
f. Contractor shall conform to the following standards, policies, regulations, governing agency, and any other local, state, or Federal guidelines that are not listed here. Contractor will coordinate with the local VA Loma Linda Health Care System GEMS Coordinator.
g. Contractor responsible for clean-up, safe removal, and off-site disposal of items used during the project in accordance with all OSHA, GEMS, VA, etc. This includes, but is not limited to, clean up materials used during the project, discarded and non-compliant doors, door parts, etc.
h. Contractor shall perform work in accordance with existing, relevant standards and codes to minimize the possibility of injury to personnel, damage to government property/equipment.
i. Contractor will not damage existing interstitial infrastructure and/or healthcare system property/spaces (e.g., surfaces, stairwells, corridors, panels, walls, elevators, service areas, floors, carpet, road, pavement, etc.), including but not limited to, duct work, electrical work, conduit, low voltage conductors, piping, CAT V cable, drywall, flooring, ceiling, plumbing work, lighting, and/or blocking ingress/egress routes.
j. Damage to government property may result in additional action.
k. Every effort will be made to safeguard and care for all human life and property.
l. Clean Up briefings will occur daily, weekly at 3:15pm to provide a briefing of progress and/or findings to address red flag issues, action items, and/or review items that may require additional discussion of options.
m. The Contractor shall furnish all labor, materials, supplies, tools, equipment, vehicles, removal devices, carrying containers, carts, flashlights, etc.
n. Provide before and after photos to be submitted with final, close out report.
Phase 1. Conduct a thorough site survey to include dimensions, pictures, and written scope of work for each line item. Vendor will take measurements, and order all required items to include doors, hardware, etc. All doors will be clearly labeled as certified or not certified. Certifications will be provided. Non-certified doors will be repaired to meet certification requirements.
Phase 2. Vendor will install all doors and/or hardware to ensure compliance. The vendor will provide all required oversight. Daily project logs will be completed and turned in at the end of each day for COR review. Weekly reports will also be submitted documenting project progress, line items completed, in progress, parts status, pending completion, upcoming schedules, and/or issues.
NFPA reports and checklists will be completed and submitted daily and weekly. Doors which are not listed as rated will be rated. Doors listed as rated which are no longer visible will be addressed and fixed to ensure labeling.
In the event that the vendor discovers a door which are listed as rated but not actually rated will be addressed on a case-by-case basis.
No changes will be made without COR consent.
The vendor will provide patch and paint and carpentry, if needed.
This is a not to exceed quote and any not required services will not be charged to the customer by the vendor.

(a) The offeror's attention is called to the Equal Opportunity clause and the Affirmative Action Compliance Requirements for Construction clause of this solicitation.
(b) The goals for minority and female participation, expressed in percentage terms for the Contractor's aggregate workforce in each trade on all construction work in the covered area, are as follows:

(End of Provision) A.6 52.225-10 NOTICE OF BUY AMERICAN REQUIREMENT—CONSTRUCTION MATERIALS (MAY 2014)
(a) Definitions. "Commercially available off-the-shelf (COTS) item," "construction material," "domestic construction material," and "foreign construction material," as used in this provision, are defined in the clause of this solicitation entitled "Buy American—Construction Materials" (Federal Acquisition Regulation (FAR) clause 52.225-9).
(b) Requests for determinations of inapplicability. An offeror requesting a determination regarding the inapplicability of the Buy American statute should submit the request to the Contracting Officer in time to allow a determination before submission of offers. The offeror shall include the information and applicable supporting data required by paragraphs (c) and (d) of the clause at FAR 52.225-9 in the request. If an offeror has not requested a determination regarding the inapplicability of the Buy American statute before submitting its offer, or has not received a response to a previous request, the offeror shall include the information and supporting data in the offer.
(c) Evaluation of offers.
(1) The Government will evaluate an offer requesting exception to the requirements of the Buy American statute, based on claimed unreasonable cost of domestic construction material, by adding to the offered price the appropriate percentage of the cost of such foreign construction material, as specified in paragraph (b)(3)(i) of the clause at FAR 52.225-9.
(2) If evaluation results in a tie between an offeror that requested the substitution of foreign construction material based on unreasonable cost and an offeror that did not request an exception, the Contracting Officer will award to the offeror that did not request an exception based on unreasonable cost.
(d) Alternate offers.
(1) When an offer includes foreign solicitation in paragraph (b)(2) of the clause at FAR 52.225-9, the offeror also may submit an alternate offer based on use of equivalent domestic construction material.
(2) If an alternate offer is submitted, the offeror shall submit a separate Standard Form 1442 for the alternate offer, and a separate price comparison table prepared in accordance with paragraphs (c) and (d) of the clause at FAR 52.225-9 for the offer that is based on the use of any foreign construction material for which the Government has not yet determined an exception applies.
(3) If the Government determines that a particular exception requested in accordance with paragraph (c) of the clause at FAR 52.225-9 does not apply, the Government will evaluate only those offers based on use of the equivalent domestic construction material, and the offeror shall be required to furnish such domestic construction material. An offer based on use of the foreign construction material for which an exception was requested—
(i) Will be rejected as nonresponsive if this acquisition is conducted by sealed bidding; or
(ii) May be accepted if revised during negotiations.

A.7 52.228-1 BID GUARANTEE (SEP 1996)
(a) Failure to furnish a bid guarantee in the proper form and amount, by the time set for opening of bids, may be cause for rejection of the bid.
(b) The bidder shall furnish a bid guarantee in the form of a firm commitment, e.g., bid bond supported by good and sufficient surety or sureties acceptable to the Government, postal money order, certified check, cashier's check, irrevocable letter of credit, or, under Treasury Department regulations, certain bonds or notes of the United States. The Contracting Officer will return bid guarantees, other than bid bonds—
(1) To unsuccessful bidders as soon as practicable after the opening of bids; and

(c) The amount of the bid guarantee shall be 20 percent of the bid price or $500,000.00, whichever is less.
(d) If the successful bidder, upon acceptance of its bid by the Government within the period specified for acceptance, fails to execute all contractual documents or furnish executed bond(s) within 10 days after receipt of the forms by the bidder, the Contracting Officer may terminate the contract for default.
(e) In the event the contract is terminated for default, the bidder is liable for any cost of acquiring the work that exceeds the amount of its bid, and the bid guarantee is available to offset the difference.

(ii) No entity owned or controlled by the Offeror has engaged in any activity that contributed to or was a significant factor in the President’s or Secretary of State’s determination that a foreign country is in violation of its obligations undertaken in any arms control, nonproliferation, or disarmament agreement to which the United States is a party, or is not adhering to its arms control, nonproliferation, or disarmament commitments in which the United States is a participating state. The determinations are described in the most recent unclassified annual report provided to Congress pursuant to section 403 of the Arms Control and Disarmament Act (22 U.S.C. 2593a). The report is available via the internet at https://www.state.gov/t/avc/rls/rpt/; or
(2) The Offeror is providing separate information with its offer in accordance with paragraph (d)(2) of this provision.

For clarity, references to the report in this section refer to the entirety of the annual unclassified report, including any separate reports that are incorporated by reference into the annual unclassified report.
