Q1. Is the Bid due by end of business on the 30th or is there a specific time you want the bid turned in by? pamphlet
A1: Proposal due date was extended under Amendment 00003.
Q2. The 14th and 30th are the dates already showing on the Solicitation. Would you clarify? Are the dates perhaps changing from the 14th to the 17th and the 30th to the 31st?
A2: Proposal due date was extended under Amendment 00003.
Q3. I saw on Construct Connect that there is a specification section for Cementitious Decks on the VA Medical Center renovation in Grand Junction.
A3: That is accurate
Q4. Is there a link to plans and specs we can review by any chance?
A4: They are available via FBO as attachments to the solicitation.
Q5. A Table of Contents was not provided for the Specifications for this solicitation. Will the VA provide the Specifications Table of Contents so that the Contractor can verify that all of the applicable Sections were transmitted?
A5: See attachment to this Amendment.
Q6. RFP, Page 7 of 69, Special Conditions #2 states that hours of construction will be 4:00 p.m. to midnight MST. Will these hours be enforced for the entire project or only the work that impacts the floors above or below this project?
A6: For bid purposes, theses hours will be enforced for the entire project. All work that creates noise above 120 dB must cease after 10 PM.
Q7. Specification Section 28 23 00 – Video Surveillance does not supply the information for either the existing Video Management Software platform nor the basis of design for the required system. Will the VA provide the basis of design or the existing system’s manufacturer’s name and the actual system required for this application?
A7: This is a stand-alone patient CCTV system, and it just need to be IP based with IP cameras, software management system, new NVR, new PoE switches, 4 PR UTP and related equipment’s as specified in the specs and shown on the drawings.
Q8. Specification Section 28 23 00 – Video Surveillance does not supply the information for either the existing surveillance cameras nor the basis of design for the cameras required for this project. Will the VA provide a basis of design manufacturer’s name and model number for the cameras required for this project?
A8: This is a stand-alone patient CCTV system, and it just need to be IP based with IP cameras, software management system, new NVR, new PoE switches, 4 PR UTP and related equipment’s as specified in the specs and shown on the drawings.
Q9. The scope of work for this project requires the installation of patient lifts, including the above-ceiling Unistrut structural supports. In order to provide a cost for the structural supports, the Contractor requires information on the construction structural ceiling details and the height of the deck above the ACT ceiling. Will the VA provide as-built drawings that depict the structural make-up of the deck and the sections depicting the ceiling height?
A9: Bottom of Concrete deck is approx. 12’-8” AFF, Ceilings in Patient rooms are 8’-8” Unless noted Otherwise.
Please Note: Structural drawings are attached to this Amendment as “Original Structural 1-31” and “Original Structural 1-32.”
Q10. The VA provided drawings indication the asbestos test locations and the areas of required lead/asbestos abatement for this project. Will the VA provide the formal Hazardous Material Survey that corresponds to these 2014 drawings?
A10: The most recent asbestos survey is attached as “Asbestos Inspection Report.” Please note that lead was not done as part of this survey, there is a separate report attached as “Lead Paint Final Report.”
Q11. Drawing G-102 shows placement of hard wall barrier note below grid line 10. Can we assume this to be our ICRA barrier for both Phases allowing both phases to be conducted simultaneously?
A11: Yes, for bid purposes. All ICRA barriers will be coordinated with COR in the field prior to start of construction. Patient and employee traffic routes must be maintained from main elevators to ICU, PACU and OR.
Q12. Regarding project phasing, is it the intent of the government allow work to be done and release for construction the whole east wing (Phase A) as noted on the drawings re Phase Limits until completed then release the West Wing (Phase B) upon completion or will it be on a room by room basis?
A12: Work can be done on both wings simultaneously but patient and employee flow must be maintained from the main elevators to ICU, PACU and OR.
Q13. In regards to Fire Alarm Systems (Item #10 in the solicitation/scope of work) and contractor coordination, according to the referred to document, this coordination will be with Simplex Grinnell. Can you please confirm that Simplex Grinnell is indeed the Fire Alarm System provider/ proprietary vendor and any pertinent contact information to allow communications and request a proposal for any changes and modifications required?
A13: The fire alarm system is a Notifier System NOT Simplex Grinnell
Q14. Regarding Hazardous Materials (primarily ACM’s) is there any additional information available as to the Building Survey findings regarding the state of said ACM’s such as friable and / or non-friable as well as the possibilities of lead paints.
A14: The information we have is in the Asbestos survey report, see Attached report.
Q15. On sheet A102 in the “Specific Floor Plan Notes” note A7 calls for “Existing Glazing to be painted with dark paint”. Is this method of blacking out the windows something that the government would want to pursue or would the government want to seek a more sustainable option?
A15: Do NOT paint over or cover the windows in any fashion.
Q16. Regarding the realignment of fire sprinkler heads in rooms to be renovated, will the engineering and drawings for said realignments require review from the local authority (Grand Junction Fire Department (GJ FD)) or will that review/approval come from the VA and / or its affiliates?
A16: GJ FD will not be reviewing. The fire sprinkler system submittal will be provided with a fire protection engineer stamp.
Q17. Do all walls within the current facility go to deck?
A17: No.
Q18. Where are the plumbing shut off valves locates to isolate main riser runs?
A18: Domestic water shut off valves are located in the crawlspace of Building 1, see Attached documents, “Original Domestic Water 1-47” and “Original Domestic Water 1-47 (2)”.
Q19. Drawing A202/3 does not show how to address window height above the RCP planned elevations. Current ACT is adjusted to fit the higher window elevation. How does the government want to address this situation?
A19: Adjust the ACT to fit the higher window elevation.
Q20. Drawing A202 corridor has ACT elevations at 8’2” AFF. Existing ACT is 7’11” AFF. Will there be enough room to raise the ceiling height with all of the MEP systems in the plenum or does the government want us to raise these systems to allow for this added height?
A20: Design expects MEP trades to install components as required to accommodate architectural dimensions.
Q21. During the site visit it was mention that the existing floors are poured up against the walls. Will The contractor be responsible for the infill? If so, please quantify the LF of repair and provide material specification.
A21: Yes, refer back to drawings originally attached in the solicitation to calculate LF of repair.
Q22. Refer to the details on Sheet A-101. Please confirm the existing expansion joint cover is to be replaced per specification.
A22: Yes, Contractor to provide new expansion joint covers
Q23. Confirm the location for the Med-Gas shut off in each wing. Are there isolation valves anywhere in the system or will the entire floor (including areas not within the construction boundaries) that need to be shut down and re-filled/calibrated each time the system is worked on?
A23: Isolation valves for all med gas lines in 3 East and 3 West are located in corridor 3406. These valves separate the construction zones from the rest of the med gas system on the 3rd floor. The isolation valves shall remain closed throughout the construction period and only opened after 3 East and 3 West lines have been tested and calibrated. This has been field verified by VHA Maintenance Technicians.
Q24. At the bid walk thru it was stated that the existing terrazzo was poured after the walls were erected. During a similar project on the 6th floor this process created ledges and troughs of varying depths around the track/studs and an extremely uneven base that required significant prep for the new wall layout and flooring including grinding, chipping, and leveling – should it be assumed that this condition would be similar on the 3rd floor?
A24: Yes.
Q25. Specification section 087100 Door Hardware paragraph 3.5 and the Door Scheduled Plan Page A801 includes references to several doors that are not shown in the plans. 3052, 3053, 3054, 3055, 3056, 3057, 3401, 3431, 3432, 3441. Confirm scope related to these openings.
A25: These doors were omitted from the Scope of work. Doors Indicated on Sheet A801 are correct
Q26. Doors 3431 and 3421 are shown on A103 but not included in the door schedule or door hardware section – confirm these doors are existing and no new doors/frames and hardware are required for these locations.
A26: Existing Doors, Frames, and Hardware to remain.
Q27. Confirm height of doors 3435 and 3436 of 78” (6’-6”) meet ADA clearance requirements.
A27: Provide all height of doors 7’ - 0”.
Q28. Confirm no latching device is required for HW set 01 at doors 3435 and 3436 rated at 45 mins.
A28: Provide Surface mounted vertical rod device.
Q29. Door 3403A lists HW set 02 in the door schedule but specifications says set 03 – confirm which set.
A29: Provide HW set 02, Door 3400A is to have Set HW3.
Q30. Bid deducts listed in the Solicitation page 6 reference “interior finishes”. Please be more specific as to what is considered an “interior finish”.
A30: Refer to specification section 09 06 00 Schedule for Finishes.
Q31. Asbestos - the removal of the asbestos plaster (7535 Sq Ft), floor tile and mastic (653 Sq Ft + 2622 Sq Ft), transite (456 Sq Ft), and pipe insulation (490 Sq Ft) are depicted on the drawings and quantities for these removals are also listed in the asbestos specifications. Are we to bid this project based on the quantities listed in the specifications or depicted by the drawings? The quantities listed for plaster appear to be about 30 % low compared to the drawings. There is no way to know how much asbestos pipe insulation is inside the walls.
A31: Transite is in the windows, reference most recent asbestos survey, see attached.
Q32. Asbestos - the demolition contractor will have to remove all the ceilings and non asbestos partition walls prior to the abatement contractor in order to expose asbestos insulated pipes for the abatement contractor to remove. Is this correct?
A32: Yes.
Q33. Asbestos - the drawings show that the plaster is to be removed from the outside walls (this is one layer), and the hallway walls (this is 2 layers or 2 sides). Removal of plaster from the hallway walls will therefore require twice the labor and disposal. Is the quantity for the hallway walls (both sides) already included in the quantities shown in the specifications - or will a modification be provided when the final quantity is calculated?
A33: The intent of the project is to abate asbestos containing materials according to the Asbestos Inspection Results Summary, building 1 dated March 2018 on drawings and documents as follows: Figure 03B, Summary Sheet for Figure 03B, Figure 03D, Summary Sheet for Figure 03D, Figure 04B, Summary Sheet for Figure 04B, Figure 04D, and Summary Sheet for Figure 04D.
Q34. Asbestos - the drawing shows that the plaster is to be removed along the stair cases on the east and west ends of the building. Will we be removing only the plaster on the patient room side of the these walls bordering the stair cases, or will we be removing the plaster on both sides of these end walls which will open up the stair cases to the work area?
A34: The intent of the project is to abate asbestos containing materials according to the Asbestos Inspection Results Summary, building 1 dated March 2018 on drawings and documents as follows: Figure 03B, Summary Sheet for Figure 03B, Figure 03D, Summary Sheet for Figure 03D, Figure 04B, Summary Sheet for Figure 04B, Figure 04D, and Summary Sheet for Figure 04D.
Q35. Asbestos - the drawing does not show any plaster ceiling removal or removal of partition walls between rooms. The only removal of plaster that is shown is the outside walls, and only the hallway walls in the west half. Is this correct?
A35: The intent of the project is to abate asbestos containing materials according to the Asbestos Inspection Results Summary, building 1 dated March 2018 on drawings and documents as follows: Figure 03B, Summary Sheet for Figure 03B, Figure 03D, Summary Sheet for Figure 03D, Figure 04B, Summary Sheet for Figure 04B, Figure 04D, and Summary Sheet for Figure 04D.
Q36. Asbestos - The safest way to take the waste out of the hospital without spills, will be to place the asbestos (which is in double lined 6 mil disposal bags) into covered carts, and then to roll these carts to the elevator, take the carts down to the first floor, and then roll them down the west hallway to the exit door and then roll them outside to an enclosed 40 yard roll off dumpster located near the south driveway. Will this route be acceptable? These bags cannot be dropped down a chute or walked down public staircases.
A36: This is an acceptable route as long as it is done 4 pm - Midnight
Q37. Asbestos - Colorado requires that certified asbestos workers take a 32 hours training class and asbestos supervisors take a 40 hour training class. These classes include OSHA training as part of the curriculum. Both disciplines require annual refresher training and State testing to maintain State certification. Confirm this is sufficient to meet the requirements of 02 08 10, para 1.5, D 5.
A37: All asbestos abatement will follow state of Colorado Regulation 8.
Q38. Asbestos - There is no asbestos worker classification listed in the Davis Bacon wage determination. What labor rate and fringe rate are we to use for asbestos workers?
A38: See U.S. Departmen of Labor Prevailing Wage Resource Book 2010, https://www.dol.gov/whd/recovery/pwrb/Tab14DBConformances.pdf. Use Laborers, IAW above link, as long abatement removal does not include removal from pipes or boilers.
Q39. There is a lead paint removal specification for this project, but it is unclear if lead paint exists on this project. Is there any lead paint removal on this project?
A39: The latest lead is attached as “Lead Paint Final Report.” It is known that there is lead paint on all the door jams in the area.
Q40. Sheet A002 shows demolition of existing RCP. D12 says “Contractor to remove existing ceiling tile, grid and tees to perform plumbing mechanical and electrical work above ceiling. Following completion of above ceiling work, install new grid and ceiling tile to match existing ceiling. Field verify scope of work”. Sheet MD-102 General Demolition Note #2 states remove, store, and replace ceiling tile as necessary.
A40: A002 Keynote D12 is correct as the existing ceiling is beyond repair/replacement. All ceiling tile, grid and tees to be replaced on 3rd floor. Contractor to remove existing ceiling tiles, grid and tees as needed to complete work on 2nd floor and only replace the ceiling components that are beyond repair. New ceiling components must match existing ones.
Q41. Is the intent only to remove and replace tiles “as needed” in these areas of the 2nd floor or is the entire area reflected by D12 to be replaced with new?
A41: Only replace as needed (damaged) on 2nd floor
Q42. Sheet A002 Demolition Ceiling Legend depicts two scenarios – existing ceiling to be removed (faint lines) and existing ceiling to remain (darker lines). However, between column lines 10 and 11 there are dark lines as well as the D12 symbol. Please confirm scope in this area or confirm replace as needed.
A42: D12 symbol is correct in this location.
Q43. Sheet A002 West Wing shows D12 symbol in area between column lines 2 and 3 but the entire area between column lines 1 and 4/A and D have faint lines. Clarify scope in this area or confirm replace as needed.
A43: Symbol D12 applies to the room in its entirety.
Q44. Sheet MH101 notes #1 states to clean supply and return main ducts. Do these ducts have internal or external insulation.
A44: External insulation, internal insulation is NOT allowed in the hospital.
Q45. Sheet PP101 General Note #2 states the entire project area will be protected by a wet pipe sprinkler system. Is the intent to have a temporary fire sprinkler system during the construction phases?
A45: A sprinkler system will be active throughout demolition and construction. Contractor/Vendor shall maintain all NFPA requirements throughout contract construction period.
Q46. Lighting Controls: Confirm that Wireless/ Ethernet wiring control system (such as Horton/Acuity controls) may be proposed.
A46. Yes, they may be proposed.
