About this file

This is a Performance Relevancy Survey form used to evaluate offeror past performance for a federal contract to renovate a radiation oncology department (Project No. 586-26-100, Solicitation No. 36C25626R0072).

Part 1 requires offerors to submit up to three project references completed within five years prior to the solicitation issue date. For each project, offerors must provide: project/contract title and number; project location; whether they served as prime contractor or subcontractor; original and current/completed contract values; actual start and end dates; on-time completion status; any liquidated damages assessed; CPARS availability; and a detailed description of work performed with a relevance rating (Very Relevant, Relevant, or Somewhat Relevant). Very Relevant projects involve essentially the same scope, magnitude, and complexity as the current solicitation; Relevant projects involve similar scope and magnitude; and Somewhat Relevant projects involve some of the required scope and complexity. Offerors must also identify a point of contact for each reference who will complete a detailed questionnaire/survey, and they are responsible for ensuring their references submit completed questionnaires prior to solicitation close.

Part 2, which need only be provided once in the submission package, requires additional company-level past performance information including: assessment of any liquidated damages on contracts within the past five years; discussion of problems encountered on relevant projects and corrective actions taken, including explanations of any marginal CPARS ratings; professional/industry awards and recognition received within five years of the solicitation release date; unique skills and accomplishments with supporting documentation; and technical areas uniquely relevant to the radiation oncology renovation project, specifying work performed by the prime firm versus major subcontractors and whether those subcontractors will be retained on this project.
