About this file

This document is a Standard Form 30 Amendment of Solicitation/Modification of Contract for a Department of Veterans Affairs project at the Network Contracting Office (NCO) 10 in Cincinnati, OH. Amendment 0005 provides clarifications to multiple Requests for Information (RFIs) related to a construction project, including details about lead-based paint abatement, equipment installation, commissioning responsibilities, and demolition of headwall units. Key clarifications include: all existing doorframes requiring lead abatement, assumptions that OFCI equipment is plug-in type, confirmation that Levin Porter and Associates is contracted directly by the VA for commissioning, and specifications for demolishing second-floor wall-mounted patient headwalls while protecting third-floor ceiling-mounted booms.

The amendment also addresses technical questions about cable management for an EHRM project and potential cable pathway modifications. The contractor is instructed to bid according to the construction documents as presented, with the understanding that any significant deviations or additional costs may need to be addressed through change orders. Two attachments are included: a General Commissioning Specifications document (01 91 00) and a First Floor Plan for the ED Project. The bid opening date, time, and call-in information remain unchanged from previous amendments, and the document is signed by the Contracting Officer from the VA's Network Contracting Office.

CLARIFICATIONS FOR RFIs

RFI #2 asked about LBP abatement. Response is to strip paint on the beige door frames in the MICU. Which frames are beige? The survey only identifies the one sampled as “North Doorway into Pharmacy Room in

MICU”. Plans do not identify a pharmacy. There is A277 Medication with a door to the North that goes to A298 Office. Even if we assume that door is the door mentioned, which other doors are beige? There are two other metal door frames identified in the survey (A299 and A389 (A389 is not in the project area)) and there is nothing distinguishing about any of these three types to be able to determine the other doors that match each type.

Perhaps for the sake of leveling bids, the COR could state a certain number for all bidders to include and any deviation may be addressed as a Change

Order.

Answer: All doorframes existing to remain in scope to require lead abatement.

RFI #15 asked for installation details for OFCI equipment. Response states to coordinate with COR. Without details, we cannot include the labor costs associated with installing any of this equipment. Please provide details or we will need owner to pay additional as change order.

Answer: Contractor should assume equipment is plug-in type only.

Several RFI ask who is responsible for Commissioning – meaning who pays for it. Is Levin Porter and Associates contracted and paid for directly by the VA under separate contract, or is the GC to include their cost in our bid? (RFI 32, 85, 129, and 163)

Answer: Refer to attachment for commissioning specification section.

Yes...Levin Porter and Associates are contracted and paid for directly by the VA under a separate contract. The phone number is 937-224-1931.

RFI #145 – asked about layout of the area indicated by Note 1.05 and A101-

N that states that area has a project underway. The response said to refer to RFI 13 which is about the laydown area. Please provide a response to this question.

Answer: Layout is provided in attachment. Project is currently under construction.

RFI #147 – asked about headwall units not shown on plans. Response stated we are to demo those. We cannot quantify the cost of this without knowing locations, quantity, etc. Please provide more information or this must be addressed as a change order as unforeseen.

Answer: Second floor wall mounted patient head walls shall be demoed.

Assume each patient room has (1) head wall each. Third floor ceiling mounted booms shall remain and contractor to protect in place and work around.

RFI #168 states that a separate EHRM project is underway and is providing racks and cable management and that the GC for this bid is responsible for terminating those cables. This is highly unusual to require a subsequent contractor to work another contractor’s system. We can’t know if they ran the correct wires, ran them properly, provided adequate labeling, etc. They should be responsible for all their own terminations. If this is to be on our contract, we need a detailed list of all drops, cable types, lengths, etc. as well as commitment that any errors introduced by them shall not negatively impact this GC for contract time or material expense.

Answer: Contractor to bid as shown in the construction documents.

RFI 169 asked if a different cable type could be used to address extended cable runs. Response asked us to provide new drawings proposing new pathways. We cannot ascertain the cost of that option not knowing if it will be approved. Please advise what we should include in our bid amount: 1 – use GameChanger for longer runs, 2 – bid as shown and owner will fund change order for either using GameChanger cables or developing designs and all costs associated with new pathways.

Answer: Contractor to bid as shown in the construction documents.
