This guide is an overview and does not provide a complete description of all benefit provisions. It also includes helpful tips to help you understand your coverage, save time and money on healthcare, reduce taxes, and balance your work and home life.
For more detailed information, please refer to the plan benefit booklets. The plan benefit booklets determine how all benefits are paid. A list of plan contacts is included in the back of this guide.
The benefits in this summary are effective:
March 1, 2025, through February 28, 2026
Medicare Part D Notice
If you (and/or your dependents) have Medicare or will become eligible for Medicare in the next 12 months, a federal law gives you more choices about your prescription drug coverage.
Please see the Important Notices section for more details.
If anything in this summary contradicts with your Plan Document, the Plan Document will prevail.
To watch the helpful videos placed throughout the guide, please review the PDF outside of the CITRIX platform.

Finding a Doctor or Specialist Need to find a doctor or specialist? Want to confirm if your providers are in the network?
Review the steps below to get started.
I. Visit www.aetna.com
II. Select the Find a Doctor feature in the on the homepage tool bar
III. Sign In or Search as a guest with a plan from an employer
IV. Continue as a guest and enter the doctor’s city or zip code and select search
V. On the Select Plan page, select a plan under Aetna Open Access Plans:
I. HMO: Aetna Select Open Access EPO
II. PPO: Aetna Choice POS II (Open Access)
VI. Enter your provider’s name. If results load, the provider is in-network with the selected plan.
Which plan is right for you?
Medical Plan I: Open Access Aetna Select EPO (HMO)
• Not required to have a primary care physician on file
• Choose any PCP from the network
• Visit any doctor or specialist in the network without a referral
• Lower healthcare costs with predictable out-of-pocket costs
• Out-of-network care is not covered except in an emergency.
You are not required to choose a primary care physician (PCP) or to get a referral for specialist visits, but we do recommend that you select a PCP to manage routine care and hospital stays.
Medical Plan II: Aetna Choice POS II (PPO)
• Not required to have a primary care physician on file
• Visit any doctor or specialist in the network without a referral
• You want coverage for out-of-network providers (at a higher cost).
This plan gives you flexibility and choice, for a price. You can go to any doctor without a referral, but you will pay a larger share of the cost if they are not in the plan's network. You must meet an annual deductible before the plan starts to pay.
https://www.aetna.com/individuals-families/find-a-doctor.html
Preventive care You take your car in for maintenance; why not do the same for yourself?
Annual preventive checkups can help you and your doctor identify your baseline level of health and detect issues before they become serious.
Health plans are required to cover a set of preventive services at no cost to you, even if you haven’t met your deductible. The preventive care services you’ll need to stay healthy vary by age, sex, and medical history.
Be aware: Not all exams and tests are considered preventive care
Certain screenings may be considered diagnostic, rather than preventive, based on your current medical condition. You may be responsible for paying all or a share of the cost for those services.
In addition, exams performed by specialists are generally not considered preventive care and may not be covered at 100%.
If you have a question about whether a service will be covered as preventive care, contact your medical plan.
Typical screenings for adults
• Blood pressure
• Cholesterol
• Diabetes
• Colorectal cancer screening
• Depression
• Mammograms
• OB/GYN screenings
• Prostate cancer screening
• Testicular exam
Medical Plan Comparison
You always pay the deductible and/or copayment ($). Referrals are not required for primary or specialist office visits. Under the HMO plan, there is no out-of-network coverage. Under the PPO plan, you may be subject to pay the full cost for services when you seek care out of the network.

Healthcare flexible spending account (FSA)
Set aside healthcare dollars for the year
A healthcare FSA allows you to set aside tax-free money to pay for healthcare expenses you expect to have over the coming year. This program is administered by P&A Group.
How the Healthcare flexible spending account works
• You estimate what your and your dependents’ out-of-pocket costs will be for the coming year. Think about what out-of-pocket costs you expect to have for eligible expenses such as office visits, surgery, dental and vision expenses, prescriptions, and certain drugstore items.
• You can contribute a minimum of $120 up to $3,300, the annual limit set by the IRS. Contributions are deducted from your pay pre-tax, meaning no federal or state tax on that amount. A health care payment card will be provided for easy payment of expenses.
• During the year, you can use your FSA debit card to pay for services and products. Withdrawals are tax-free as long as they’re for eligible healthcare expenses.
• You cannot obtain reimbursement for eligible expenses for a domestic partner or their children unless they qualify as your tax dependents (Important: questions about the tax status of your dependents should be addressed with your tax advisor).
• Keep your receipts and Explanation of Benefits (EOB) statements as proof that your expenses were eligible for IRS purposes.
• You must re-enroll in this program each year to allow use of rollover funds.
Estimate carefully!
If you don’t spend all the money in your account, you can roll over up to $660 to use the following year. Any additional remaining balance will be forfeited.
Potential tax savings
Because FSA contributions are pre-tax, they reduce the total amount of your income the government makes you pay taxes
on. Tax savings vary depending on filing status and other variables.
Are you eligible?
You don’t have to enroll in one of our medical plans to participate in the healthcare FSA.
Find out more:
• Eligible Expenses
• Ineligible Expenses
Important Considerations
There's no "crossover" spending allowed between the healthcare and dependent care accounts.
Expenses must be incurred between 03/01/2025 and 02/28/2026 and submitted no later than 05/29/2026.
Elections cannot be changed during the plan year unless you have a qualified change in family status (and the election change must be consistent with the event).
FSA funds can be used for eligible expenses incurred by you, your spouse, and your tax dependents only.

Prescriptions breaking your budget?
Understanding the formulary can save you money
If your doctor prescribes medicine, especially for an ongoing condition, don’t forget to check your health plan’s drug formulary. It’s a powerful tool that can help you make informed decisions about your medication options and identify the lowest cost selection.
What is a formulary?
A drug formulary is a list of prescription drugs covered by your medical plan. Most prescription drug formularies separate the medications they cover into four or five drug categories, or “tiers.” These groupings range from least expensive to most expensive cost to you. “Preferred” drugs generally cost you less than “non-preferred” drugs.
Get the most from your coverage
To get the most out of your prescription drug coverage, note where your prescriptions fall within your plan’s drug formulary tiers and ask your doctor for advice. Generic drugs are usually the lowest cost option. Generics are required by the Food and Drug Administration (FDA) to be as effective as brand-name drug equivalents.
To find out if a drug is on your plan’s formulary, visit the plan’s website or call the customer service number on your ID card.
CVS Pharmacy Mail Service Order
Quick. Without the hassle. Get your regular medicines through CVS Caremark Mail Service Pharmacy.
1. Call CVS Pharmacy, go online, or download the mobile app
2. Request Mail Service
3. Get refills your way
Depending on your plan, you may pay less for medicine you get through home delivery than a retail pharmacy. Visit aetna.com and visit the Pharmacy section to get cost estimates for generic or name brand drugs.
Need help? Call toll-free, 24/7 at (888) 792-3862.

Tollway -provided Basic Life and AD&D insurance
Basic Life and AD&D
If you have loved ones who depend on your income for support, having life and accidental death insurance can help protect your family's financial security and pay for large expenses such as housing and education, as well as day-to-day living expenses.
Basic life insurance pays your beneficiary a lump sum if you die. AD&D (accidental death & dismemberment) coverage provides a benefit to you if you suffer from loss of a limb, speech, sight, or hearing, or to your beneficiary if you have a fatal accident. The salary class you fall in will determine which benefit is available to you.
The cost of coverage for this benefit is paid in full by the Tollway and offered to you by MetLife.
CLASS ONE: Salaries less than $30,000
CLASS TWO: Salaries of $30,000 - $50,000
CLASS THREE: Salaries more than $50,000
What’s guaranteed issue?
If you select coverage above a certain limit (the "guaranteed issue") or after your initial eligibility, you will need to provide additional information about your health status to qualify for the requested amount of coverage.
A note about taxes
Company-provided life insurance coverage over $50,000 is considered a taxable benefit. The value of the benefit over $50,000 will be reported as taxable income on your annual W-2 form.
Basic Life Amount $50,000
Basic AD&D Amount $50,000
Basic Life Amount $75,000
Basic AD&D Amount $75,000
Basic Life Amount $100,000
Basic AD&D Amount $100,000
Name Your Beneficiaries
If the worst happens, your beneficiary—the person (or people) on record with the life insurance carrier—receives the benefit. Make sure that you name at least one beneficiary for your life insurance benefit, and change your beneficiary as needed if your situation changes.
Voluntary Life and AD&D insurance
Protecting those you leave behind
Voluntary Life insurance allows you to purchase additional life insurance to protect your family's financial security. Voluntary AD&D Insurance allows you to purchase additional accidental death and dismemberment insurance to protect your family's financial security in case you suffer from loss of a limb, speech, sight or hearing or if you die in an accident.
MetLife offers coverage for your spouse and children if you purchase coverage for yourself.
Employees whose spouses are also employed by the Tollway may not enroll their spouse or dependents under dual coverage. Only one spouse-employee may elect voluntary life insurance for their spouse and/or children. It is the responsibility of the employees to coordinate their elections to ensure compliance with this policy.
Guaranteed issue
If you purchase life insurance coverage above a certain limit (the "guaranteed issue" amount) or after your initial eligibility period, you will need to submit a Statement of Health (Evidence of Insurability) with additional information about your health for the insurance company to approve the amount of coverage. You can increase your election each year by $10,000 increments without completing a Statement of Health if your election is less than the guaranteed issue amount listed.

Paying for daycare? Make it tax-free!
Dependent Care FSA—up to $5,000 per year tax-free
A dependent care Flexible Spending Account (DCFSA) can help families save potentially hundreds of dollars per year on day care. This program is administered by P&A Group.
Here's how the Dependent Care FSA works
You set aside money from your paycheck, before taxes, to pay for work-related day care expenses. Eligible expenses include not only childcare, but also before- and after-school care programs, preschool, and summer day camp for children younger than 13.
The account can also be used for day care for a spouse or other adult dependent who lives with you and is physically or mentally incapable of self-care.
You can set aside up to $5,000 per household per year.
Members can pay for dependent care expenses directly from their FSA account by using their debit card, submitting a claim through the participant portal, or mailing or faxing a paper claim.
All caregivers must have a tax ID or Social Security number.
This information must be included on your federal tax return.
If you use the dependent care reimbursement account, the IRS will not allow you to claim a dependent care credit for reimbursed expenses. Consult your tax advisor to determine whether you should enroll in this plan.
Estimate carefully!
You can’t change your FSA election amount mid-year unless you experience a qualifying event. Money contributed to a dependent care FSA must be used for expenses incurred during the same plan year. Any unused dependent care funds will not rollover, therefore the funds will be forfeited. It is very important that you plan carefully before making your election.
Every opportunity to save
The biggest deduction from your paycheck is likely federal income tax. Why not take a bite out of taxes while paying for necessary expenses with tax-free dollars?

Commuting to work
Transportation savings accounts—up to $325 per month tax-free
Do you have out-of-pocket commuting expenses for public transportation, van pooling, or for worksite parking? If so, you can save on taxes by enrolling in our transportation savings account, administered by P&A Group.
The account lets you set aside money—before it's taxed— through payroll deduction. You may enroll in or stop this program at any time. Money in the account can be used in future months or plan years.
Set aside up to $325 per month for work-related parking expenses and up to $325 per month for work-related commute expenses.
Reimbursement can take 3-5 business days after P&A receives the claim as long as there refunds available in your account.
You will receive a reimbursement check but you have the option to sign up for direct deposit reimbursement.

Deferred Compensation — State of Illinois 457-retirement plan
The Deferred Compensation Plan is a 457-retirement plan, offered by the State of Illinois, which allows you to save a portion of your pay through payroll deductions, or “contributions.” These contributions are then invested and allowed to compound on a tax-advantaged basis until you make qualified withdrawals, typically in retirement.
The plan offers tax savings NOW through pre-tax contributions and/or tax savings after you retire through a Roth after-tax option.
The Plan automatically enrolls new State employees; 3% of your before-tax pay will be deposited into your Plan account.
You may change your contribution rate or opt out within 30 days for a full refund. Funds managed by Empower.
What are your plans?
Many of us can’t plan past the next paycheck, never mind thinking about a retirement nest egg. Our retirement and pension plans will help you set a retirement savings goal and stick to it.
The important thing is to start now and set aside what you can, even if you think it’s too small an amount.
Maximum 2025 contribution
(IRS limits are evaluated annually)
Up to $23,500. If you’re 50+ years old, you can save an extra $7,500.
Total contribution limit for aged 50+ is $31,000.
How to Enroll or Contribute to the Deferred Compensation Plan
To enroll in Deferred Compensation, please contact Employee Benefits at:
EmpBenefits@getipass.com.
If you are already an existing member and would like to make changes to your account, please contact Empower at:

Important differences of a Roth 457(k)
• You pay taxes when you contribute, at your current tax rate.
• Account interest and dividends are not taxed if you meet certain criteria.
• Like a traditional 401(k), you can withdraw money without penalties when you reach age 59½, but you must have held the account for at least 5 years.
• You are not forced to take distributions at age 70½.
You can keep the money in your Roth account as long as you want.
