About this file

This is a URS Post-Retired Verification Form for independent contractors and 1099 employees required by Jordan School District in Utah. The form is used to determine whether any individual serving as a contractor, owner, partner, or principal of a contracting business has retired from the Utah Retirement Systems (URS). Contractors must disclose their retirement status, provide their birthdate if applicable, identify the type of business being provided, and submit the completed form along with a W-9 to the Accounting Department. The form applies to all work or services performed as an employee paid through payroll or as a contract worker paid through Accounting for any participating URS employer.

URS retirees receiving monthly benefits face significant restrictions when returning to work and are required to report any reemployment to URS on Form RTRT-27A in compliance with Utah Code Section 49-11-1207(2). Failure to report reemployment constitutes a violation that may result in the retiree being held liable to repay URS any overpayments received. Before beginning work with Jordan School District, any retiree must contact URS's Retirement Benefits Department at 801-366-7770 and complete the Post-Retirement Employment form as directed.
