Dear Ms. Kegel:

Abatement Contractors of Montana, LLC (ACM) is pleased to provide this sampling report for a limited asbestos inspection conducted at the Kamiah Supervisors Offices located at 1023 5th Street (Annex 1) and 1022 5th Street (Annex 2) in Kamiah, Idaho. The asbestos inspection was performed on March 6th, 2020, by James Parker, MTA-5024, a Montana Department of Environmental Quality (DEQ)-accredited asbestos inspector. The results of this inspection can be found in the enclosed report.

INTRODUCTION

ACM collected a total of seventy eight (78) bulk samples representing twenty six (26) homogeneous areas (HA) of suspected asbestos-containing building materials (ACBM) in the concerned area(s) that will be impacted during the planned demolition of the garages and the basements of both buildings within the subject property. Any materials found to contain greater than one percent (>1%) asbestos by weight are considered to be Asbestos Containing Building Materials (ACBM). This inspection considered only the garage and the basement of each building and ACM’s scope of work. This inspection was limited only to the materials that were readily accessible and only included those materials based on the information provided by the client phone conversations. All samples were collected from previously damaged or inconspicuous areas. Although ACM’s Inspector took every effort to evaluate all of the materials that will be impacted during the demolition of the garages and basements, it is possible that other hidden materials may exist.

Therefore, any materials that have not been discovered AND have not been specifically evaluated by bulk sampling in this inspection report must be tested and evaluated for asbestos content prior to disturbance of these materials. A complete list of all samples that were collected is located in Table 1. The detailed laboratory results are presented in Appendix II. A diagram of the building indicating where samples were collected and a map showing where asbestos containing material are located is enclosed within Appendix III. Photographs of materials sampled and asbestos containing materials can be found in Appendix VI.

Some of the samples tested POSITIVE for asbestos. Positive samples are highlighted in BOLD in Table 1.

BUILDING DESCRIPTION

ACM was contracted to perform a demolition asbestos inspection of the garage and basements of 1023 5th Street (Annex 1) and 1022 5th Street (Annex 2) at the subject site. Both buildings are a single story commercial facility housing offices, working spaces, mechanical spaces, kitchens and restrooms. The slab on grade buildings has a basement with steel support beams with a steel cross beam to support the upper http://www.acm-contracting.com/ mailto:clkegel@fs.fed.us

Inspection Number IH 20-022 Page 2 of 8 March 10, 2020 level. The interior walls of the basements are wood formed concrete which are mainly covered with wood framing with textured gypsum coverings. The ceilings are mainly wood framed with textured gypsum sheeting. The floors are concrete in the basements and are covered with carpeting with associated mastics.

The floors in the garages are wood framed with wood sheathing and are covered with carpeting and associated mastics in Annex 2 and laminated wood flooring with foam padding in Annex 1. The interior walls and ceilings are wood framed with textured gypsum sheathing in both garages. Attic insulation of both garages are blown-in cellulose on top of fiberglass batting. The exterior walls are T1-11 sheathing on the street side of the garage and cedar siding on the remaining portions of the garages. There is a tar paper moisture barrier below the exterior siding that is mechanically attached to the wood frame. The pitched roofs of the garages are constructed of composite asphalt shingles over a rolled out felt sheeting. The soffits and fascia boards of the garages are wood.

INSPECTION

The limited asbestos inspection was performed in accordance with Occupational Safety and Health Administration 29 Code of Federal Regulations (CFR) 1926.1101(k), the Montana DEQ Administrative Rules of Montana (ARM) 17.74.354 and National Emission Standards for Hazardous Air Pollutants (NESHAP) 40 CFR 61, Subparts A and M, and applicable portions of the currently recognized standard protocol developed for schools under the Asbestos Hazard Emergency Response Act (AHERA) and as promulgated in Title 40, Code of Federal Regulations (40 CFR), Part 763 and as amended in the Federal Register. The Quest Micro- Analytics Laboratory Analysis Report are enclosed for reference.

Representative bulk material samples were collected from the suspect asbestos-containing surfacing (S) materials, thermal system insulation (TSI) materials and miscellaneous (M) materials from each of the assigned homogeneous areas (HAs) that will be impacted at the time of the demolition. The suspect ACBM were visually identified and touched prior to sampling. The samples were placed in HA groups using a coding method to classify each suspect ACBM by type, texture, color and location. The asbestos inspection included:

Identification of HA suspect ACBM on an area-by-area basis within ACM’s scope of work;

Inspection was non-destructive in nature and bulk samples were collected in discrete locations when possible;

Collection and analysis of bulk samples to confirm or deny the presence of asbestos.

No materials were presumed to be asbestos containing materials

Bulk samples were not collected from any HA suspect ACBM made of fiberglass, wood, foam, glass or rubber.

This building did not reveal Vermiculite insulation within the wall cavities or in the attic of the garages. Due to the nature of Vermiculite and the uncertainties in the variations with testing techniques, it has been assumed that this material contains asbestos. Removal of this material should be accomplished by an accredited abatement contractor who will properly remove the material, containerize it in the recommended containers and dispose of at a certified landfill.

A minimum of three (3) suspect ACBM samples were collected from random locations within the subject area. Selected suspect ACBM were limited to HA that will be impacted at the time of the demolition. When possible, suspect ACBM samples were collected from existing damaged areas. These samples were placed in pre-labeled plastic containers for shipment to the laboratory.

The samples were shipped, under chain-of-custody protocol for standard analyses, to Quest Micro-Analytics Laboratory (NVLAP Lab# 200249) in Dallas, Texas for bulk sample analysis utilizing Polarized Light

Microscopy (PLM) coupled with dispersion staining for asbestos identification. The detailed laboratory results are presented in Appendix II.

Materials in the following table that are highlighted in BOLD were found to be asbestos containing.

Our inspection has revealed the presence of ACM at the subject property. This inspection was generally non-destructive in scope and may not have located all potentially hidden ACBM within the subject property.

However, we have attempted to sample all obvious and accessible suspect materials within the scope of this inspection. Most of these materials appeared to be in a good condition at the time of our inspection.

Limitations

This asbestos inspection summary was prepared based on information gathered during our site visit and interpretations of laboratory results of bulk samples collected during the inspection. This inspection considered only the garage and the basement of each building and ACM’s scope of work. Additional sampling may be required if additional HA’s are exposed during material removal, or the scope of work is expanded to include HA’s that have not been inspected or analyzed for asbestos content.

Any ACBM that might be impacted during renovation / demolition activities must be addressed prior to start of such activities by a qualified and accredited abatement company.

Abatement Contractors of Montana (ACM) greatly appreciates this opportunity to assist the United States Forest Service and look forward to providing our services to you on future projects. If you have any questions regarding this sampling report, would like an estimate for removal, or require additional information of our services, please contact me at 406-549-8489 or via e-mail at jim.parker@acm-contracting.com.

Sincerely, James Parker Montana DEQ-Accredited Asbestos Inspector

Analyst: Jennifer Jaber

Approved Signatory :

Samples are analyzed by layers, and percentages estimated visually during microscopic examination. Individual analysis sheets available upon request. Results may not be reproduced except in full. This test report relates only to the samples tested, and results must not be used to claim product endorsement by NVLAP or any agency of the U.S. Government.

The EPA test method for bulk analysis (EPA/600/R-93/116) states in paragraph 2.2.2. that "the detection limit for visual estimation is a function of the quantity of the sample analyzed, the nature of matrix interference, sample preparation, and fiber size and distribution. Asbestos may be detected in concentrations of less than one percent by area if sufficient material is analyzed. Samples may contain fibers too small to be resolved by PLM (<0.25 micrometers in diameter) so detection of those fibers by this method may not be possible."

Materials containing >1% asbestos are considered by the EPA to be asbestos containing materials, and must be handled as such.
